The pedicle screw fixation with vertebroplasty augmentation in the surgical treatment of the severe osteoporotic spines.

STUDY DESIGN: Retrospective clinical study. OBJECTIVE: To present the early clinical results of pedicle screw fixation augmented by vertebroplasty using polymethylmethacrylate in severely osteoporotic patients requiring spine surgery due to the neurologic deficit. SUMMARY OF BACKGROUND DATA: It is postulated that combining a formal vertebroplasty-that is, maximum filling of the trabecular space with polymethylmethacrylate-with pedicle screw placement in osteoporotic vertebrae could result in resistance to pullout forces significantly. METHODS: Between the years 2003 and 2006, pedicle screw …

The relationship between the cervical spinal canal diameter and the pathological changes in the cervical spine.

A congenitally narrow cervical spinal canal has been established as an important risk factor for the development of cervical spondylotic myelopathy. However, few reports have described the mechanism underlying this risk. In this study, we investigate the relationship between cervical spinal canal narrowing and pathological changes in the cervical spine using positional magnetic resonance imaging (MRI). Two hundred and ninety-five symptomatic patients underwent cervical MRI in the weight-bearing position with dynamic motion (flexion, neutral, and extension) of the cervical spine. The sagittal c…

Percutaneous core excision and radiofrequency thermo-coagulation for the ablation of osteoid osteoma of the spine.

Percutaneous radiofrequency ablation is the treatment of choice for osteoid osteoma of the appendicular skeleton. However, difficulties in localizing the lesion in the spine and its proximity to neural elements have yet to make it the prevalent treatment for spine. This study assesses the safety and effectiveness of two percutaneous techniques for ablating osteoid osteoma of the spine. Seven patients were treated between 1998 and 2005. Four patients underwent percutaneous radiofrequency coagulation. The lesions were located at the articular processes of L3 and L4, the lamina of L3 and in the h…

[Cauda equina syndrome leading to diagnosis of malignant non-Hodgkin lymphoma of the spine].

INTRODUCTION: Malignant non-Hodgkin lymphoma is a lymphoid proliferation. Peripheral neuropathies are observed in only 2-8% of cases. CASE: We report a case in which cauda equina syndrome in a 29-year-old man led to the diagnosis of a painless non-Hodgkin lymphoma of the spine. Combined chemotherapy and radiation therapy produced spectacular improvement. DISCUSSION: Infiltration of the cauda equina roots is rare in malignant non-Hodgkin lymphoma and raises problems for positive diagnosis. The good outcome under treatment underlines the interest in considering this cause in disorders affectin…

An analysis of different types of surgical fixation for avulsion fractures of the anterior tibial spine.

This is a retrospective study of 25 patients, looking at the outcome of open reduction and fixation of displaced anterior tibial spine avulsion fractures with absorbable and non-absorbable materials. The mean period of follow-up was 44 months (range, 21 to 88 months). The results of surgery were assessed clinically and radiologically. Outcome was evaluated by using the Lysholm knee scoring system. Anterior cruciate ligament laxity was assessed by using a KT-1000 arthrometer and range of movements with a goniometer. Overall, children did better than adults, but age per se did not appear to affe…

Orientation and position of head posture, scapula and thoracic spine in mouth-breathing children.

OBJECTIVE: Mouth-breathing is a common clinical condition among school-age children and some studies have correlated this condition with quality of life and postural alterations. Therefore, the objective of this study was to investigate the orientation and position of the scapula, thoracic spine and head posture among mouth-breathing (MB) children and nasal-breathing (NB) children. METHODS: Twenty-one male MB children and 21 male NB children between 8 and 12 years of age participated in the study. Data were obtained through a stereophotogrammetry system that uses passive markers over anatomic…

Pain and osteolysis of the thoracic spine--a case of a rare monostotic fibrous dysplasia manifestation.

We describe a case of a young farmer from Central Macedonia, Greece suffering of a mild back pain more than one year. His medical history included hypercholesterolaemia (IIa type) and two episodes of spontaneous pneumothorax of unknown origin two and three years ago respectively. A full imaging survey revealed a single osteolytic lesion at the seventh thoracic vertebra. A CT guided needle biopsy was performed. Diagnosis based on clinical, imaging and histological findings was monostotic fibrous dysplasia of the thoracic spine. We discuss the clinical features and treatment of this non neoplast…

Quantitative analysis of spinal curvature in 3D: application to CT images of normal spine.

The purpose of this study is to present a framework for quantitative analysis of spinal curvature in 3D. In order to study the properties of such complex 3D structures, we propose two descriptors that capture the characteristics of spinal curvature in 3D. The descriptors are the geometric curvature (GC) and curvature angle (CA), which are independent of the orientation and size of spine anatomy. We demonstrate the two descriptors that characterize the spinal curvature in 3D on 30 computed tomography (CT) images of normal spine and on a scoliotic spine. The descriptors are determined from 3D ve…

Computed tomography alone for cervical spine clearance in the unreliable patient--are we there yet?

BACKGROUND: Injuries to the cervical spine (CS) occur in 2% to 6.6% of blunt trauma patients. Studies have suggested that computed tomography (CT) alone is sufficient for CS clearance in unreliable patients based on follow-up magnetic resonance (MR) imaging not altering management. We hypothesized that an admission cervical spine CT with no acute injury-using new CT technology-is not sufficient for CS clearance in an unreliable patient. METHODS: The trauma registry was used to identify all patients with blunt trauma who had CS imaging with a CT and MR between August 2004 and December 2005. Du…

Intradural extramedullary primary hydatid cyst of the spine in a child: a very rare presentation.

Spinal hydatid cyst is a serious but fortunately uncommon manifestation of the parasite Echinococcus, involving less than 1% patients with hydatid disease. Intradural hydatid cysts are extremely rare compared to other types of spinal hydatid cysts. We report a rare case of intradural, extramedullary spinal hydatid cyst in a 9-year-old male boy, who presented with weakness of both lower limbs for the last 4 months that was confirmed histopathologically; a better understanding of this rare but clinically challenging disease is intended by reporting this case.

Reactive changes in the adolescent porcine spine with disc degeneration due to endplate injury.

Degenerative and reactive structural alterations occurring after experimentally-induced disc degeneration were evaluated using a porcine model. A cranial perforation was made through the L4 vertebral endplate into the nucleus pulposus. Three months later, the lumbar intervertebral disc and adjacent vertebrae were dissected, fixed in formalin and further processed for histopathological analyses. The results showed that there were nucleus pulposus fragments, rather than a distinct border between the nucleus and annulus fibrosus. The central lamellae were distorted and delamination of the outer a

[Upright posture impact on spine susceptibility in scoliosis and progression patterns of scoliotic curve].

OBJECTIVE: To investigate the impact of upright posture on spine susceptibility in scoliosis and scoliotic curve progression patterns. METHODS: There were 3 groups designed in the experiment: bipedal rats group (n = 26), quadrupedal rats group (n = 26) and sham operation bipedal control (n = 12). Taking the advantage of bipedal rats' upright-posture simulating effect, operations were exerted with posterior central approach between scapulas. Along the spatial of erector spinae muscle and trunk muscles, left side upper (T2-3 or T3-4) and lower (T10-11 or 11 - 12) ribs were tethered with type4 t…

[Aneurysmal bone cysts. First description of the extensive destruction of the upper cervical spine].

Aneurysmal bone cysts are not counted among the classic malignant tumors, although they are destructive locally as blood filled reactive bone lesions. Typically, they are found in the metaphysis of the long bones, while localizations on the spine are rare. A 16-year-old female patient presented with unspecific, progressive neck pain which had been present for half a year. The initial x-ray showed no noticeable pathology whatsoever. Subsequently, the complete destruction of the first cervical vertebrae was found. The tumor had completely infiltrated and completely surrounded the spinal chord. A…

Pain regulation and health-related quality of life after thoracolumbar fractures of the spine.

Fractures of the thoracolumbar spine rank among the severest injuries of the human skeleton. Especially in younger patients they often result from high-energy accidents. Recently, a shift in paradigm towards more aggressive treatment strategies including anterior procedures could be observed. However, so far only few data exist reflecting the quality of life (QoL) after such injuries. The aim of this study was to evaluate medium-term QoL and further to identify factors that influence the clinical outcome in patients with fractures of the thoracolumbar spine. Data of 906 patients who were treat…

[A surgical approach to tuberculosis of the thoracic spine and kyphotic deformity and compression of the spinal cord].

In patients with tuberculosis of thoracic vertebrae, kyphotic deformity and compression of the spinal cord involves resection of the involved portion and revision of the vertebral column. Resection was performed with a chisel that has an edge stop. Thus, 2 anterior and 2 posterior incisions (grooves) and a cross bed between them are prepared after resection on adjacent surfaces. The anterior epidural space is revised, by setting the spinal cord free from compressing agents. Three-four posterior rib ends are preliminary crossed above or below the apex of the hump at the level of the ends of tra
