THE SYMPATHETIC MECHANISM IN THE ISOLATED PULMONARY ARTERY OF THE RABBIT.

The nature of postganglionic sympathetic nervous transmission to vascular muscle in vitro was studied using the recurrent cardiac nerve-pulmonary artery preparation of the rabbit. Experiments, similar to those which in other tissues have provided evidence to support a role for acetylcholine at the sympathetic postganglionic nerve-effector cell junction, were carried out. The contractile response of the isolated artery to acetylcholine was blocked completely by atropine. High concentrations of acetylcholine and of hemicholinium had no effect on the contractile response to sympathetic nerve stim

A Temporal Reliability and Sustainability Score Framework for Ranking Heart Rate Variability Metrics During Pharmacological Autonomic Blockade in Rats.

BACKGROUND: While Heart Rate Variability (HRV) is a cornerstone of non-invasive autonomic assessment, the sensitivity and sustainability of specific HRV parameters remain debated. This study introduces a "sustainability score" to systematically validate which metrics best reflect autonomic nervous system (ANS) modulation following pharmacological blockade. METHODS: Ten male Sprague-Dawley rats were implanted with ECG electrodes. After recovery, rats underwent randomized atropine (1 mg/kg, intraperitoneal) and propranolol (10 mg/kg, intraperitoneal) sessions separated by at least 3 days. Basel

Acute and 14-day effects of tiotropium/olodaterol on cardiopulmonary function in COPD.

BACKGROUND: Lung hyperinflation in COPD adversely affects cardiac function and may influence autonomic nervous system activity. The onset of cardiac improvement with dual bronchodilator therapy remains uncertain. METHODS: In this randomised, placebo-controlled, investigator-blinded, mechanistic, single-dose crossover trial with an open-label 2-week extension we assessed tiotropium/olodaterol (T/O) effects on cardiopulmonary function and muscle sympathetic nerve activity (MSNA) in hyperinflated COPD patients. 32 participants received saline (placebo) and T/O (5 µg/5 µg) in a crossover design,

Clinical characteristics, diagnostic challenges, and outcome of paroxysmal sympathetic hyperactivity in pediatric patients: a retrospective cohort study in a tertiary hospital setting.

INTRODUCTION: Paroxysmal Sympathetic Hyperactivity (PSH) is an under-recognized condition in pediatric patients, particularly those with non-traumatic brain injuries, often leading to delayed diagnosis and suboptimal management. The condition features episodic increases in sympathetic nervous system activity, which creates significant diagnostic and therapeutic challenges. This study aims to comprehensively characterize the clinical presentation, diagnostic challenges, and treatment outcomes of pediatric PSH in a tertiary care setting. Additionally, we investigate factors contributing to delay…

The Effect of Soft Tissue Release at the Thoracolumbar Junction in a Patient with Bilateral Leg Symptoms: A Case Report.

BACKGROUND: Manual therapists mostly see patients with bilateral leg symptoms. Pain, pins and needles, fatigue, heaviness, lower limb coldness, and loss of neurological conduction are the patients' symptoms. It is hypothesized to be caused by the sympathetic nervous system. Few publications cover its pathophysiology, diagnosis, and treatment. Limited research has examined the consequences of soft tissue release (STR) at the thoracolumbar junction. This case describes STR in a patient with bilateral leg symptoms. CASE REPORT: A 39-year-old female presented with bilateral leg symptoms, especial…

Diagnostic Efficacy of 123Iodo-Metaiodobenzylguanidine SPECT/CT in Cardiac vs. Neurological Diseases: A Comparative Study of Arrhythmogenic Right Ventricular Cardiomyopathy and α-Synucleinopathies.

Background/Objectives: 123Iodo-metaiodobenzylguanidine single photon emission computed tomography/computed tomography (123I-MIBG SPECT/CT) is used to evaluate the cardiac sympathetic nervous system in cardiac diseases such as arrhythmogenic right ventricular cardiomyopathy (ARVC) and α-synucleinopathies such as Parkinson's diseases. A common feature of these diseases is denervation. We aimed to compare quantitative and semi-quantitative cardiac sympathetic innervation using 123I-MIBG imaging of ARVC and α-synucleinopathies. Methods: Cardiac innervation was assessed using 123I-MIBG SPECT/CT in …

Immune Status of Individuals with Traumatic Spinal Cord Injury: A Systematic Review and Meta-Analysis.

Individuals with spinal cord injury (SCI) have higher infection rates compared to those without SCI. In this review, the immune status difference between individuals with and without traumatic SCI is investigated by examining their peripheral immune cells and markers. PubMed, Cochrane, EMBASE, and Ovid MEDLINE were searched without language or date restrictions. Studies reporting peripheral immune markers' concentration and changes in functional capabilities of immune cells that compared individuals with and without SCI were included. Studies with participants with active infection, immune dis

Long-term safety and efficacy of renal sympathetic denervation in atrial fibrillation: 3-year results of the AFFORD study.

BACKGROUND: Atrial fibrillation (AF) is the most common sustained arrhythmia which has been associated with increased sympathetic nervous system activity and hypertension. Recent evidence indicated that renal sympathetic denervation (RDN) could safely contribute to an improvement in AF burden. OBJECTIVE: To investigate the long-term safety and efficacy of radiofrequency RDN in hypertensive patients with symptomatic AF. METHODS: This pilot study included patients with symptomatic paroxysmal or persistent AF (European Hearth Rhythm Association class ≥ II) despite optimal medical therapy, offic…

Reduction of Cardiac Autonomic Modulation and Increased Sympathetic Activity by Heart Rate Variability in Patients With Long COVID.

Although several clinical manifestations of persistent long coronavirus disease (COVID-19) have been documented, their effects on the cardiovascular and autonomic nervous system over the long term remain unclear. Thus, we examined the presence of alterations in cardiac autonomic functioning in individuals with long-term manifestations. The study was conducted from October 2020 to May 2021, and an autonomic assessment was performed to collect heart rate data for the heart rate variability (HRV) analysis. The study participants were divided into the long COVID clinical group, the intragroup, whi

The contribution of autonomic mechanisms to pain in temporomandibular disorders: A narrative review.

BACKGROUND: Temporomandibular disorders (TMD) are diagnosed based on symptom presentation and, like other functional pain disorders, often lack definitive pathology. There is a strong association between elevated stress levels and the severity of TMD-related pain, which suggests that alterations in autonomic tone may contribute to this pain condition. OBJECTIVES: This narrative review examines the association between altered autonomic function and pain in TMD. METHODS: Relevant articles were identified by searching PubMed and through the reference list of those studies. RESULTS: TMD suffere

Cardiac sympathectomy for refractory ventricular tachycardia in arrhythmogenic right ventricular cardiomyopathy.

BACKGROUND: The sympathetic nervous system plays an important role in arrhythmogenesis in arrhythmogenic right ventricular cardiomyopathy (ARVC). Sudden cardiac death commonly occurs during exertion, and β-blockers are associated with a reduction in arrhythmia burden. Bilateral cardiac sympathetic denervation (BCSD) has been shown to reduce implantable cardioverter-defibrillator (ICD) shocks in patients with structural heart disease and refractory ventricular tachycardia (VT); however, data in ARVC are sparse. OBJECTIVE: The purpose of this study was to evaluate the role of BCSD in patients w…
