Intratendinous strain fields of the supraspinatus tendon: effect of a surgically created articular-surface rotator cuff tear.

Articular-surface partial-thickness rotator cuff tears play a significant role in shoulder pathology, but the role of the articular-surface tissue is poorly understood. This investigation assessed the effect of an articular-surface partial-thickness rotator cuff tear on intratendinous strain fields. A magnetic resonance imaging-based technique quantified intratendinous strains in healthy cadaveric shoulders at 15 degrees, 30 degrees, 45 degrees, and 60 degrees of glenohumeral abduction. A second set of magnetic resonance images was acquired after an articular-surface partial-thickness tear was

Effect of superior capsule and coracohumeral ligament release on strain in the repaired rotator cuff tendon. A cadaveric study.

Twelve cadaveric shoulders were used to determine the effects of release of the superior capsule and the coracohumeral ligament on the strain in the repaired rotator cuff tendon. A rotator cuff tear (2 cm wide and 1.5 cm long) was created and repaired under a 3-kg tensile force. The strain in the repaired tendon was measured with use of linear transducers with the arm in 50 different positions. Release of either the superior capsule or the coracohumeral ligament diminished the tension of the repaired rotator cuff by an average of 25% with the arm in adduction. Release of both of these structur

Neer Award 1999. Overuse activity injures the supraspinatus tendon in an animal model: a histologic and biomechanical study.

Overuse activity has been implicated as an etiologic factor in injury to the rotator cuff and to the supraspinatus tendon in particular. Due … aracteristics, and the mechanical properties of the tendon tissue were evaluated. The supraspinatus tendons in the exercised animals demonstrated significant changes as a result of overuse at all time points compared with the normal group. There was an increase in cellularity and a loss of the normal collagen fiber organization consistent with what has be …

Desmotomy of the accessory ligament of the superficial digital flexor tendon in the horse with use of a tenoscopic approach to the carpal sheath.

OBJECTIVE: To describe a tenoscopic approach to the carpal sheath for desmotomy of the accessory ligament of the superficial digital flexor tendon. STUDY DESIGN: The surgical procedure was developed with use of normal forelimbs from equine cadavers and experimental horses. ANIMALS OR SAMPLE POPULATION: Twelve equine cadaveric forelimbs, 4 forelimbs from 2 horses anesthetized for terminal surgical laboratories, and 10 forelimbs from five experimental horses were used. METHODS: The limbs were positioned lateral side up with the carpus slightly flexed. After distention of the carpal sheath, a

Calibration and application of an intra-articular force transducer for the measurement of patellar tendon graft forces: an in situ evaluation.

The objective of this study was to evaluate two calibration methods for the "Arthroscopically Implantable Force Probe" (AIFP) that are potentially suitable for in vivo use: (1) a direct, experimentally based method performed by applying a tensile load directly to the graft after it is harvested but prior to implantation (the "pre-implantation" technique), and (2) an indirect method that utilizes cadaver-based analytical expressions to transform the AIFP output versus anterior shear load relationship, which may be established in vivo, to resultant graft load (the "post-implantation" technique).

Flexor tendon repair in a rabbit model using a "core" of extensor retinaculum with synovial membrane. An experimental study.

In this histological and biomechanical study in two groups of rabbits, a piece of the extensor retinaculum with its synovial membrane was inserted as a biological "core" into a hole at the centre of both stumps of a severed tendon, which was repaired with interrupted sutures. In the other group, the tendon was sutured without a "core". In the "core" group, proliferation and migration of fibroblasts from both tendon surfaces and the "core" surface toward the deep layer of the suture site was seen 2 weeks after operation. New collagen fibres, aligned parallel to the long axis of the tendon, coul

Interposition of the split flexor hallucis longus tendon after resection of a coalition of the middle facet of the talocalcaneal joint.

Ten consecutive patients (fourteen feet) who had a painful coalition of the middle facet of the talocalcaneal joint with restricted motion of that joint were managed with a resection of the coalition and interposition of a split flexor hallucis longus tendon in 1992, 1993, or 1994. Initial nonoperative treatment of all of the feet had failed. According to the ankle-hindfoot clinical rating system of the American Orthopaedic Foot and Ankle Society, there were eleven excellent results and one good, one fair, and one poor result at a mean of fifty-one months (range, thirty-two to sixty months) af

Closed reduction and tendon transfer for treatment of dislocation of the glenohumeral joint secondary to brachial plexus birth palsy.

Dislocation of the glenohumeral joint developed, in the first few years of life, in eight children who had brachial plexus birth palsy. The palsy involved the fifth and sixth cervical nerve roots in six children and the fifth, sixth, and seventh cervical nerve roots in two. All of the children had a release of the insertions of the pectoralis major, latissimus dorsi, and teres major followed by a closed reduction of the glenohumeral joint. The latissimus dorsi and the teres major were then transferred to the rotator cuff. All of the children had a well located glenohumeral joint with at least

Controlled active motion following primary flexor tendon repair: a prospective study over 9 years.

One hundred and thirty patients with 339 divided flexor tendons affecting 208 fingers were studied prospectively between 1988 and 1996, to assess a regime of primary flexor tendon suture and active postoperative motion, combined with a modified Kleinert dynamic traction splint. The tendon suture technique used was a high-strength multistrand technique using a modified Kessler core and a Halsted peripheral stitch. The results were influenced by the zone in which the tendon was divided, by the physiotherapy and to a lesser extent by the grade of surgeon operating. Overall results by Strickland c…

Morphological changes of the triceps surae muscle-tendon unit during passive extension: an in vivo rabbit model.

OBJECTIVE: To elucidate the morphological and biomechanical manifestation of the triceps surae muscle-tendon unit during passive extension. DESIGN: The instantaneous changes within the load-deformation curve of muscle-tendon unit were analyzed by an in vivo rabbit model. BACKGROUND: Although muscle strains occur more frequently than complete failures, the failure mechanism of these sub-failure injuries is rarely investigated. Monitoring of the instantaneous changes in the load-deformation curve allows correlation with the morphological changes that occur during passive extension. METHODS: Afte

[Functional radial tendon replacement surgery at the Accident Hospital of the occupational association in Hamburg. Indications--technique--results].

Tendon transpositions serve to restore functions which were lost through an injury of nerves or muscles. At the Department of Hand Surgery and Plastic Surgery at the Berufsgenossenschaftliches Unfallkrankenhaus Hamburg, 116 tendon transpositions were carried out for radial paralysis between 1960 and 1989. The follow-up for 85 patients is presented and the evaluation scheme developed by Haas (1991) further modified and our scoring system compared to others such as Zachary (1946) modified by Neumann and Partecke (1980). The results were good, also when evaluated according to Riordan (1959) and M

Normalisation of slow-relaxing tendon reflexes (Woltman's sign) after cardiac pacing for complete heart block.

A patient whose slow-relaxing tendon reflexes returned to normal after cardiac pacing for complete heart block is reported. Since the patient was euthyroid, these observations add further weight to previous suggestions that intrinsic catecholaminergic mechanisms, as well as thyroid status, may play a role in the pathogenesis of slow-relaxing reflexes.

Giant cell tumor of tendon sheath and pigmented villonodular synovitis: immunophenotype suggests a synovial cell origin.

Giant cell tumor of tendon sheath (GCTS) and pigmented villonodular synovitis (PVNS) are common synovial "tumors." Their immunohistochemical profile, however, has not been well characterized, and uncertainty exists regarding their histogenesis and relationship to fibroma of tendon sheath. In an effort to clarify these uncertainties and to better define the immunohistochemical profile of GCTS/PVNS, we examined formalin fixed tissue from 35 specimens of GCTS, 12 specimens of PVNS, and three cases of reactive synovitis using avidin biotin complex (ABC) and streptavidin immunohistochemical methods

Reconstruction of extensor mechanism after trauma and infection by transposition of the Achilles tendon: report of technique and four cases.

High-energy injuries to the knee often involve extensive soft tissue loss as well as fractures. In certain situations, the patellar tendon may be lost due to initial injury or post-injury complications such as infection. Use of the gastrocnemius flap is a popular method of covering the defect as well as controlling infection and promoting healing by increasing local vascularity. We present a method of reconstructing the patellar tendon by extending the use of the gastrocnemius flaps using its Achilles tendon and calcaneal insertion. The distal insertion of the Achilles tendon is fixed to the t

Closed rupture of the flexor hallucis longus tendon in a long distance runner: report of a case and review of the literature.

A unique case report of rupture of the flexor hallucis longus tendon at the great toe metatarsal head level is reported in a well-trained marathon runner. The literature regarding interruptions of the flexor hallucis longus tendon is reviewed. There is an identifiable trend: Closed ruptures of the tendons when repaired, do not permit pull-through of the tendon, whereas open lacerations have some potential to restore this function.

Physiologic consequences of surgical lengthening of extensor carpi radialis brevis muscle-tendon junction for tennis elbow.

Sarcomere length was measured intraoperatively using a laser diffraction method before and after surgical lengthening of the human extensor carpi radialis brevis muscle (ECRB) in five subjects treated for lateral epicondylitis. Based on measured sarcomere and filament lengths, we previously established the length-tension curve for this muscle and the normal sarcomere length range as a function of wrist joint angle. Preoperative measurements indicated that the ECRB developed near-maximal isometric force at full wrist extension, decreasing to 20% maximum at full wrist flexion. Stair-step surgica

Secondary hyperparathyroidism, and not beta 2-microglobulin amyloid, as a cause of spontaneous tendon rupture in patients on chronic hemodialysis.

Spontaneous bilateral rupture of the extensor mechanisms of the knees, without significant history of trauma, has been reported rarely, generally in association with chronic metabolic disorders, such as chronic renal failure and secondary hyperparathyroidism. We report spontaneous tendon rupture in two patients on chronic hemodialysis for 4 and 11 years, with documented severe secondary hyperparathyroidism. One patient had spontaneous bilateral rupture of his quadriceps and partial avulsion of the left triceps tendons. The other patient had spontaneous rupture of his right quadriceps tendon. B…
