Electrical stimulation of sacral nerve roots is called neuromodulation. In urology, neuromodulation has been used for the treatment of frequency syndrome, urinary incontinence and urinary retention. Faecal incontinence and constipation are the most significant indications for the use of neuromodulation in gastroenterology. Carefully selected patients should be evaluated by trial test during which the symptom diary will be filled before implantation. Permanent generator implantation can be done for patients with significant objective relief of symptoms. Generator setting can be adjusted by a re…

OBJECTIVE: To introduce a new internal fixation system of spine and its characteristics.
METHODS: To review 16 patients with lumbosacral instability who were their clinical outcomes and radiographic evaluation.
RESULTS: Fifteen patients gained complete recovery from their preoperative symptoms. One patient who had experienced two operations before and with problems of urinary and fecal incontinence and walking difficulty still had lower limb pain, muscle weakness and urinary incontinence after operation. There is no evidence of spine glide on X-ray, implant failure, neural complication or in…

Urethral pressure profile (UPP) measurements were recorded during stress using the method of Brown and Wickham in 981 patients with lower urinary tract symptoms. This procedure was used to identify patients with genuine stress incontinence (GSI). A clinical stress test in the supine and erect position served to confirm urinary leakage. GSI was diagnosed in 661 patients. (67%), whereas no incontinence was demonstrable in 320 (33%). UPP during stress has a high sensitivity (93.3%) and specificity (82.5%). Compared with other urodynamic investigations, the ease with which it can be performed at t

PURPOSE OF REVIEW: Mesh erosions following previous synthetic sling/mesh surgery for stress urinary incontinence (SUI) have become increasingly common. This systematic review provides evidence for the role of laser excision as a first-line management in patients with eroded mesh.
RECENT FINDINGS: Fourteen articles (173 patients) were included for the final review. Among these, 138 patients (79.8%) were submitted to trans-urethral laser excision of eroded urethral/bladder mesh over a median time to presentation of 36.6 months. Over a median follow-up of 23.6 months, 88 (63.7%) reported a compl…

BACKGROUND: Enhorning's pressure transmission theory (PTT), though mortally wounded by multiple invalidations from the 1990 Integral Theory of female urinary incontinence (IT), like Rasputin, continues to survive as a theory for continence and incontinence.
AIM: To examine the questions: How has the PTT survived? What is its contribution to knowledge?
METHODS: Eleven different invalidations are presented based on images, pressure readings, clinical examples, experiments by the author, and others, for example, flow mechanics, finite element models, and surgical operations.
RESULTS: Each of t…

OBJECTIVES: The anatomic integrity of the female mid urethra is an important factor for urinary continence. However, the associations between the urethral volume and potential affecting factors still remain uncertain. The aim of this study was to investigate the impact of potential factors, including age, height, weight, body mass index (BMI), parity number, delivery mode, and menopause, on the midurethral volume to assist in more-accurate evaluations of urethral anatomy and urinary continence mechanisms in clinical practice.
METHODS: A total of 244 women were included in this study. All pati

Unrecognized urinary retention may be a factor in the development of recurrent urinary tract infections, urinary incontinence, bladder dysfunction, and upper urinary tract disease. Because urinary retention is often asymptomatic in elderly women, with amounts of up to 1,500 mL retained in the bladder often causing little or no discomfort, nurses need to have a high index of suspicion about its occurrence. A post-void residual is the best way of determining the presence of urinary retention. The goal of treatment for urinary retention is bladder decompression via catheterization followed by voi…

Treatment selection for localized prostate cancer requires balancing oncologic control against urinary and sexual function. We retrospectively compared 32 men undergoing laparoscopic bilateral nerve-sparing radical prostatectomy (RP) with 18 undergoing focal irreversible electroporation (IRE), using unweighted and inverse probability of treatment weighting analyses. IRE was associated with shorter operative time and less frequent early urinary incontinence and sexual function decline. At 12 months after weighting, urinary incontinence occurred in 6.0% after RP and 0% after IRE, while sexual fu…

We have reviewed 78 patients who underwent "clam" enterocystoplasty as part or all of their lower urinary tract reconstruction. Nearly half (32) were non-neuropathic; the remainder were either overtly neuropathic (mainly spina bifida) or suspected neuropathic (mainly the older primary enuretics). Only 5 were non-walkers--all neuropathic. Most patients were operated on for incontinence but 12 had upper tract damage related either to urinary diversion or to poor bladder compliance in the early phase of filling; 69 patients became dry, voiding spontaneously (30), by activation of an artificial ur

Urinary leakage is prevalent in men. To contain and manage this leakage, men are often advised about urinary devices. However, sometimes this advice does not include penile sheaths. Penile sheaths are a good way of managing urinary leakage if an individual is suitable for sheaths. This article will explore this suitability, advantages to using a sheath, reasons why sheaths may not be suitable, and will discuss optimum fitting to ensure the sheath is a secure drainage device.

PURPOSE OF REVIEW: This paper reviews the literature over the last 3 years on bladder outlet procedures for children with urinary incontinence. The multitude of procedures and need for concomitant bladder augmentation indicates that there is no ideal surgical option for these children.
RECENT FINDINGS: The results of bladder neck slings with or without bladder augmentation have been reported in some key studies during this review period. Several modifications of the bladder wrap around sling and modifications of the Kropp-Salle procedure are described. Long-term results of bladder neck inject…

INTRODUCTION: Historically a positive Bonney maneuver indicates the need for surgical management of stress urinary incontinence. This maneuver is regularly used in French daily practice without formal proof of its predictive value.
MATERIAL: A Medline search was performed with the terms "Bonney test" and "urinary incontinence" to include 10 relevant articles in this review.
RESULTS: Eight articles study the mechanism of the maneuver or its derivatives, evoking a direct pressure on the urethra. Two studies are interested in the real predictivity of the maneuvers on the success of surgical tre…

Urinary incontinence can be due to neuromuscular or structural problems in either the bladder or the urethra. Urodynamics is often used to analyze the patientspecific cause of urinary incontinence. In urodynamics, a challenging part of the studies involves measurement of the urethral (contact) pressure profile. Here we present an instrumented urethral catheter that is equipped with a novel super-capacitive pressure transducer that is highly sensitive to the applied pressure. A solid ionic electrolyte is used to create a high capacitance device. Through an innovative design the solid electrolyt…

A 13-year-old girl with multiple genitourinary malformations, incomplete bladder exstrophy, urethral duplication with single bladder, septate vagina, and total urinary incontinence is presented. Prior to admission she had undergone surgery for teratoma and calculi of the bladder and partial fecal incontinence. A continent urinary diversion was done by bladder augmentation using a cecal-colonic segment and by surgical closure of the bladder neck. The distal end of the appendix was brought to the skin as an inconspicuous, easily catheterizable, watertight stoma, our modification of the Mitrofano…

In a series of 35 cases, I used a relatively uncommon but simple operation to correct urinary stress incontinence by the abdominal route. The operation is known as vesicourethrolysis and consists of dissection of the tissues in the space of Retzius. This establishes an anatomical urethrovesical angle without suturing and relieves urinary stress incontinence. The operation was first described in 1951 by Mulvaney, and although it has been done by others since, apparently with good results, the operation has never gained great popularity. Comparison of this operation with the Marshall-Marchetti-K
