Adult-onset Buphthalmos in Down Syndrome.

PURPOSE: To report a peculiar case of adult-onset buphthalmos. METHODS: Review of the medical record of a 24-year-old patient with Down syndrome who developed buphthalmos in the left eye after corneal transplantation for keratoconus. RESULTS: In the next 2 years after surgery, the operated eye evolved with chronic anterior uveitis that led to progressive peripheral synechiae, oscillating intraocular pressure, cataract, graft failure, and buphthalmos. CONCLUSIONS: The finding of buphthalmos of adult-onset in Down syndrome raises important issues concerning the classification of this eye cond…

[Primary ophthalmological diagnosis of Treponema pallidum infection: A case series].

This report is on five patients (four men and one woman) between the age of 24 and 66 years old who presented with unclear visual impairment in our clinic between 2009 and 2016 for co-evaluation. The clinical picture included intermediate uveitis, chorioretinitis, panuveitis and bilateral spontaneous cystoid macular edema. None of the patients reported systemic or dermatological symptoms. In all five patients, serological testing revealed a Treponema pallidum infection as the reason for ocular inflammation. The ophthalmologist was therefore the first to discover a syphilitic infection. After i…

Unusual Clinical Presentations in Early-Onset Childhood Sarcoidosis: A Correlation or Coincidence?

Sarcoidosis is a multisystem granulomatous disease which frequently affects young adults. Because of its rarity, the exact incidence and prevalence of childhood sarcoidosis is not known. It mostly affects children of older age group i.e., 13-15 years. Early onset sarcoidosis (<5 years) is characterized by a triad of arthritis, uveitis and rash. Late onset sarcoidosis present with a multisystem disease similar to adults, with frequent pulmonary infiltrations and lymphadenopathy. Herein, we report a case of early-onset childhood sarcoidosis in a four-year-old female along with uncommon clinical …

Coexistence of familial Mediterranean fever and Behçet's disease: a case report.

Behçet's disease (BD) is a chronic, multi-systemic vasculitis, characterized by a triad of recurrent aphthous stomatitis, genital aphthae, and uveitis. It is common in the Eastern Mediterranean, Middle East, and Eastern Asian countries. Familial Mediterranean fever (FMF) is an autosomal recessive autoinflammatory disorder, which is common seen in the Turkish, Armenian, Arabic, and Sephardic Jewish populations. It is characterized by recurrent episodes of fever, peritonitis, pleuritis, arthritis, and erysipelas-like skin lesions. Behçet's disease and FMF have common clinical features and geogra…

[TREATMENT DILEMMAS IN BEHÇET'S SYNDROME].

Behçet's disease is an inflammatory systemic disorder, characterized by a relapsing and remitting course, it manifests with oral and genital ulcerations, skin lesions, uveitis, vasculitis, central nervous system and gastrointestinal involvement. The main histopathological finding is widespread vasculitis of the arteries and veins. Therapy is variable and depends largely on the severity of the disease and organ involvement. There is common practice to treat with anticoagulation in patients suffering from vessel thrombosis, but there are no control trials to support this tendency. Anticoagulatio…

Zika virus disease for neurologists.

PURPOSE OF REVIEW: Zika virus (ZIKV) was not initially thought to be an important cause of neurologic disease, but with the recent epidemic in the Western Hemisphere, there is a growing body of evidence for possible neurologic complications. This review discusses these in addition to what is known about modes of transmission, testing for the virus, as well as future directions. RECENT FINDINGS: ZIKV disease has been associated with microcephaly and congenital eye disease in infants, as well as with Guillain-Barré syndrome, uveitis, myelitis, and meningoencephalitis in adults. SUMMARY: Even t…

Classification of intraocular tuberculosis.

Tuberculosis in the TB endemic countries commonly manifests in the eye as tuberculous uveitis. There is a wide spectrum of the clinical manifestations of intraocular tuberculosis (IOTB). For want of any gold standard diagnostic tests or diagnostic criteria, the estimates of IOTB prevalence have varied greatly. None of the previously suggested guidelines for diagnosis of IOTB have been validated. We give definitions of clinical signs and diagnostic tests that have been used in the literature. Based on these, we propose a classification of IOTB comprising "confirmed IOTB," "probable IOTB," and "…

Update on the therapy of Behçet disease.

Behçet disease is a chronic inflammatory systemic disorder, characterized by a relapsing and remitting course. It manifests with oral and genital ulcerations, skin lesions, uveitis, and vascular, central nervous system and gastrointestinal involvement. The main histopathological finding is a widespread vasculitis of the arteries and veins of any size. The cause of this disease is presumed to be multifactorial involving infectious triggers, genetic predisposition, and dysregulation of the immune system. As the clinical expression of Behçet disease is heterogeneous, pharmacological therapy is va…

Musculoskeletal Findings in Behcet's Disease.

Behcet's disease is a multisystem disease characterized by recurrent oral and genital ulcers, relapsing uveitis, mucocutaneous, articular, gastrointestinal, neurologic, and vascular manifestations. Rheumatologic manifestations may also occur in Behcet's disease, and arthritis and arthralgia are the most common musculoskeletal findings followed by enthesopathy, avascular necrosis, myalgia, and myositis. Although the main pathology of Behcet's disease has been known to be the underlying vasculitis, the etiology and exact pathogenesis of the disease are still unclear. Musculoskeletal findings of …

Behçet's disease: external ear involvement.

Behçet's disease is an autoimmune vasculitis characterized by recurrent episodes of oral and genital ulcerations, skin lesions and uveitis. It also affects neurological, vascular, articular and gastrointestinal systems. Although etiology and pathogenesis are under investigation, studies have shown that HLA-B51 influences genetic susceptibility. The development of the disease is associated to neutrophil activation. Treatment depends on the severity of the disease and immunosuppressive drugs are often used. Blindness and dementia are long-term concerns. We report an unprecedented case of a child…

[Chronic polyradiculoneuritis disclosing sarcoidosis].

Peripheral Nervous System (PNS) involvement occurs in 10 to 40 percent of patients presenting with sarcoidosis. Axonal polyneuropathies and mononeuropathies are the most frequently reported whereas acute or chronic polyradiculoneuritis are rarely described. We report the case of a 54 year-old-woman admitted for a progressive sensorimotor deficit of the lower limbs. Bilateral ocular redness corresponding to an uveitis was also observed. Neurological examination and electrophysiological data were consistent with a chronic polyradiculoneuritis. The neurological status of the patient improved afte…

Osteonecrosis and bone infarction in association with Behcet's disease: report of two cases.

We describe two cases with Behcet's disease (BD) developing osteonecrosis or bone infarction. One patient developed the extensive bone infarction of the left knee without the use of corticosteroids. The other patient had osteonecrosis at the right femoral head. He had had a past history of significant corticosteroid administration to treat several complications of BD such as central nervous system involvement, uveitis, gastrointestinal involvement, and pulmonary involvement. Anticardiolipin (aCL) antibodies were positive in these two patients. One was IgG type, and the other was IgM type. Howe…

[Tears' immunology in acute eye diseases].

The aim of the study is to research the immunoglobulins' concentration into the tears liquid and into the blood serum at the patients with acute affections of the anterior ocular pole. The study was accomplished on two groups of patients: one group with herpetic Keratitis, the other with anterior uveitis, the second having a different etiology--that the viral one. Another group of patients with senile cataract was used like witness-group. The immunoglobulins concentration were detected into the serum and into the tears by the Mancini method of the radial immunodiffusion. The results indicate a…

Erythema nodosum: a presenting sign of early onset sarcoidosis.

"Early onset sarcoidosis" is a chronic granulomatous disease occurring in children younger than 5 years of age, and characterized by a classic symptom triad consisting of skin, eye and joint lesions, with on rare occasion pulmonary involvement. The disorder often goes unrecognized because of its rarity and, since polyarthritis and uveitis are the predominant symptoms, most of these children are misdiagnosed as having juvenile chronic arthritis (JCA). A child with erythema nodosum at 7 months of age, later diagnosed as JCA and definitively recognized as "early onset sarcoidosis" is reported. Th…

Ocular Halicephalobus (syn. Micronema) deletrix in a horse.

Ocular contents from a horse with a 4-week history of severe unilateral uveitis were submitted for histopathologic examination. A severe unilateral granulomatous chorioretinitis with intralesional Halicephalobus deletrix was diagnosed. The horse developed progressive neurologic signs several days following the surgery to remove ocular contents and implant a prosthesis and was subsequently euthanatized. A severe multifocal granulomatous encephalitis with intralesional H. deletrix, localized primarily to the optic chiasm, thalamus, and brain stem, was diagnosed from tissues acquired at necropsy.…

[Implantations of posterior chamber intraocular lenses in children with monocular cataracts].

Extracapsular cataract extractions with posterior chamber intraocular lens implantations were performed on 43 eyes with monocular cataracts in children including 35 cases of traumatic and 8 cases of congenital cataracts. 65.7% of eyes with traumatic cataracts and 25% of eyes with congenital cataracts achieved postoperative corrected visual acuities of > or = 0.5. It has been found that children have good tolerance to posterior chamber intraocular lens and no serious ocular complications, such as persistent uveitis, bullous keratopathy, or secondary glaucoma were seen. The main factors influenc…
