The fingertip is mankind's tactile interface with the physical world, from reading braille, to using touchscreens, to wielding power tools. Its special tissue architecture demands astute evaluation and meticulous surgical or nonsurgical care after injury to return patients to their preinjury level of function. Attentive deliberation of physiologic, vocational, and psychosocial factors could improve the odds of achieving satisfactory results. In this article, we explore these aspects of fingertip injury to provoke readers to examine their practices and philosophies.

Lpg0189 is a type II secretion system-dependent extracellular protein with unknown function from Legionella pneumophila. Herein, we determined the crystal structure of Lpg0189 at 1.98 Å resolution by using single-wavelength anomalous diffraction (SAD). Lpg0189 folds into a novel chair-shaped architecture, with two sheets roughly perpendicular to each other. Bioinformatics analysis suggests Lpg0189 and its homologues are unique to Legionellales and evolved divergently. The interlinking structural and bioinformatics studies provide a better understanding of this hypothetical protein.

Vocal fold paralysis in adduction can result in significant breathing difficulties. Techniques such as vocal fold lateralization and/or arytenoidopexy help to improve respiratory function in this setting. These techniques require open approach or specific instruments. The authors describe an original vocal fold lateralization technique performed exclusively via an endoscopic approach. This technique helps to enlarge the glottic aperture, while preserving laryngeal architecture, and permanently improves respiratory function in patients with vocal fold paralysis in adduction.

Papillary cystadenocarcinoma is a very rare slow growing malignant neoplasm characterized by cysts and intraluminal papillary projections. It is defined by WHO as a separate entity. On FNA, cytological features can mimic with various papillary and cystic neoplasms of salivary gland. It is also difficult to distinguish from benign salivary gland neoplasms as it has bland nuclear features. Thus we present cytomorphological features and discuss its differential diagnosis with neoplasms having prominent papillary architecture with review of literature.

A case report is presented which demonstrates the effectiveness of comprehensive orthodontic treatment combined with orthognathic surgery in the correction of malocclusion and reduction in the sequelae of Obstructive Sleep Apnea (OSA). The patient's severe OSA was improved to very mild as evaluated by full overnight polysomnogram. The orthodontic treatment included the expansion of both dental arches and mandibular advancement surgery. There was significant improvement in the patient's sleep continuity and architecture with the elimination of obstructive apneas.

Using a mouse model of Alport disease, Dufek et al. report that endothelial cell-derived endothelin-1 activates mesangial cells, which deposit abnormal laminin isoforms in the Alport glomerular basement membrane. This study extends findings obtained previously by this laboratory implicating mesangial cells in the early pathogenesis of Alport disease. Together with abnormalities in matrix receptor expression, cytoskeletal architecture, and proteinase secretion in podocytes, a clearer picture is emerging on the inception of proteinuria in Alport disease.

This paper presents mu grid, a light weight middleware for grid applications, and focuses mainly on security issues--more specifically on the access control to resources--that are critical for the gridification of many medical applications. For this purpose, we use Sygn as a distributed, certificate based, and flexible access control mechanism, which has been fully integrated in mu grid. We discuss the advantages of the solution compared to classical grid approaches and the limitations of the final architecture.

Polycystic disease of the adult is the most common presentation of renal cystic disease and is transmitted by dominant autosomic heredity. Development of renal cell carcinoma has been reported within this cystic disease, but the frequency of such coincidence is not higher than in the remaining population. Most of these tumours are found incidentally and may involve a diagnostic problem due to the marked distortion of the kidney's architecture produced by the polycystic disease of the adult.

Orthotopic cardiac transplantation was performed successfully in a patient with acquired atresia of the left pulmonary artery 19 years after repair of tetralogy of Fallot. Only the right lung could be incorporated into the cardiopulmonary circulation at transplantation, resulting in transient right ventricular dysfunction, which resolved with vasodilator therapy. Perfusion of a single pulmonary vascular bed does not preclude successful heart transplantation, provided there is a low pulmonary vascular resistance and pulmonary artery architecture free of stenoses.

We report a case of fulminant hepatic necrosis occurring in the setting of acetaminophen overdose and chronic alcohol use. The pathology of severe acute toxicity is documented with a surgical biopsy. Follow-up liver biopsy 4 yr later documents hepatic regeneration without cirrhosis. Maintenance of the hepatic reticulin framework in the face of acute hepatic necrosis was well documented, and may be a useful prognostic feature indicating a potential for the maintenance of normal hepatic architecture following regeneration.

Electron microscopic studies on sinusoidal dilatation of the liver were reported in 14 patients on oral contraceptives. The architecture of dilated sinusoids was preserved together with a striking perisinusoidal fibrosis. Proliferation and enhanced activity of sinusoidal endothelial cells and of perisinusoidal cells could be observed. The lumen of the dilated sinusoids was empty with no extravasation of the erythrocytes. The changes were different from peliosis hepatis described in association with the taking of androgenic-anabolic steroids.

Auto-immune disease may result from the interaction of the genetic load of the individual, modification of self-tissue antigens by environmental agents such as virus or drugs and abnormalities of the immunological system itself such as the loss of controlling or suppressor T cells with age. In the majority of people the outcome is tolerance, maintenance of normal tissue architecture and function. In the unfortunate few the outcome is auto-immune disease, that is, failure to recognize "self".
