Cluster headache sine headache: two new cases in one family.

We report two cases of cluster headache sine headache occurring in a family of cluster headache sufferers. A 54-year-old female, suffered for 15 years from short-lasting, right-sided attacks of eye reddening, lacrimation, rhinorrea, palpebral oedema and miosis, seven to eight times per day, in periods of 20-30 days. She did not experience headache, either with or without these phenomena. Her son suffers from periorbital, short-lasting, right-sided painful attacks accompanied by eye reddening, lacrimation, rhinorrea and nasal stiffnesss, occurring in periods of 30-35 days. Soon after the end of

[Cardiac arrhythmias in Guillain-Barre syndrome. An overview of the diagnosis of a rare but potentially life-threatening complication].

Autonomic neuropathy is an important complication of Guillain-Barré syndrome (GBS). In particular, over-reactivity of vagal function may lead to serious cardiac disturbances ranging from bradycardia to asystole. Early recognition of these events is essential for appropriate intervention such as the insertion of a cardiac pacemaker. Recent studies indicate that serious bradyarrhythmias could be observed in severely and even less severely affected patients who were still able to walk more than 5 meters. Conventional measures such as the presence of tachycardia, heart rate and blood pressure fluc

[Pathophysiology of blood pressure variability in patients with chronic renal failure under maintenance hemodialysis].

The rise of blood pressure is negatively related with the glomerullar filtration rate(GFR) in patients with terminal renal failure. Hypertension may be a mechanism to maintain renal blood flow and GFR constant by the increased driving force of blood to the kidney. Elevated levels of a so-called third factor, now designated as endogenous digitalis, are found in those patients. The most likely candidate of the endogenous digitalis is ouabain, which causes hypertension with chronic administration. On the other hand, extreme hypotension often occurs during maintenance hemodialysis, and since hemod

Assessment of coronary morphology and flow in a patient with Guillain-Barré syndrome and ST-segment elevation.

Patients with Guillain-Barré syndrome often have cardiac disturbances as a manifestation of autonomic dysfunction. Such abnormalities consist of arrhythmias and disturbances of heart rate and blood pressure. We report a case of a patient with Guillain-Barré syndrome who developed ST-segment elevation in the inferolateral leads, suggestive of an acute coronary syndrome. Cardiac catheterization revealed angiographically normal coronary arteries. Intracoronary ultrasound was also normal. Intracoronary Doppler flow measurements revealed an elevated baseline coronary flow velocity of up to 41 cm/s

Autonomic imbalance, hypertension, and cardiovascular risk.

Essential hypertension conveys an increased risk of cardiovascular morbidity and mortality. The common finding of an autonomic imbalance in these patients contributes not only to the etiology of hypertension itself, but also to the cardiac risk and resulting adverse sequelae. A high sympathetic tone in particular is responsible for many of the metabolic, hemodynamic, trophic, and rheologic abnormalities that cluster in patients with high blood pressure. Methods to clarify the respective importance of prereceptor versus receptor abnormalities for the etiology of insulin resistance are warranted

[Episodes of unilateral sympathetic hyperfunction, changes in consciousness status and respiratory dysfunction in a girl with type II Chiari malformation].

INTRODUCTION: The Chiari type II malformation, often associated with a myelomeningocele, is characterized by a small posterior fossa and downward displacement of the cerebellum, IV ventricle and brainstem, with elongation and dysgenesis of other structures. It is usually linked to hydrocephaly, anomalies of the corpus callosum and hydrosyringomyelia. In symptomatic cases, the age and form of presentation are very variable and partly dependent on associated anomalies. Respiratory symptoms, the main cause of death in children with myelomeningocele, are frequent. There may be dysfunction of other

Intestinal pseudo-obstruction and acute pandysautonomia associated with Epstein-Barr virus infection.

We report the association of neurological and intestinal disorders with the reactivation of Epstein-Barr virus (EBV) in a child. This previously healthy 13-yr-old boy presented with pharyngitis and acute abdominal ileus. Laparotomy excluded a mechanical obstruction. Postoperatively, he suffered from prolonged intestinal obstruction, pandysautonomia, and encephalomyelitis. Histological examination of the appendix and a rectal biopsy taken 3 months after the onset showed an absence of ganglion cells (appendix) and hypoganglionosis (rectum), with a mononucleate inflammatory infiltrate in close co

Short-lasting primary headaches: focus on trigeminal automatic cephalgias and indomethacin-sensitive headaches.

Short-lasting primary headache syndromes provides both a diagnostic challenge and considerable therapeutic reward. Primary short-lasting headaches broadly divide themselves into those associated with auto nomic symptoms, trigeminalautonomic cephalgias and those with little autonomic syndromes. The trigeminal-autonomic cephalgias include cluster headache and paroxysmal hemicranias, in which head pain and cranial autonomic symptoms are prominent. Consideration of short- lasting headaches, particularly in the context of the differential diagnosis between cluster headache and paroxysmal hemicrania

Gastrointestinal autonomic nerve tumors in the pediatric population: a report of four cases and a review of the literature.

BACKGROUND: Gastrointestinal autonomic nerve tumors (GANTs) are a subpopulation of gastrointestinal stromal tumors (GISTs) that are characterized by ultrastructural features resembling enteric autonomic nerve cells, without epithelial, Schwannian, or smooth muscle differentiation. Delineation of the clinicopathologic features of GANT in the pediatric population is lacking. METHODS: The clinicopathologic and outcome data for four pediatric patients with GANT are presented. The data from these patients and four previously reported pediatric patients are summarized and compared with data for GAN

Managing autonomic dysreflexia through the use of clinical practice guidelines.

One of the goals listed in the American Association of Spinal Cord Injury Nurses (AASCIN) strategic plan 1993-1998, is to provide strategic vision to advance professional practice in spinal cord injury (SCI) nursing. In this quest, along with the development of Standards of Spinal Cord Injury Nursing Practice, the AASCIN Clinical Practice Committee is developing Clinical Practice Guidelines to describe a suggested course of action to address specific clinical conditions or needs of individuals with spinal cord injury. The first Clinical Practice Guideline to be completed is Autonomic Dysreflex

[Autonomic regulation of cardiac activity and the status of central hemodynamics in patients with idiopathic mitral valve prolapse].

The vegetative regulation of heart rhythm was estimated from mathematical analysis of heart rhythm in 79 patients with prolapse of mitral valve (PMV) and in 15 patients with abnormal position of chordae (ADC). The sympathetic tone elevation was more typical for patients with HMV. Meanwhile the decrease of sympathetic tone was observed in patients with PMV and ADC with vegetative crises in anamnesis. The hyperkinetic type of blood circulation was revealed both in patients with PMV and ADC.

[Familial amyloidotic polyneuropathy (FAP) type I and the therapies].

Since autonomic dysfunction in familial amyloidotic polyneuropathy (FAP) Type I is recognized in the early stage of FAP patients and restricts their daily life, we developed various therapies for various autonomic manifestations: Loading glucose was often effective for faintness caused by hypoglycemia and erythropoietin was also good therapy for orthostatic hypotension as well as anemia found in the end stage of FAP patients. Stoma and nasal drop of L-threo-DOPS were useful to control diarrhea and orthostatic hypotension. Duplex ultrasonography of the 4 vessels revealed that reverse flow was a

Assessment of autonomic function in traumatic quadriplegic and paraplegic patients by spectral analysis of heart rate variability.

We analyzed by means of autoregressive spectral analysis the spontaneous beat-to-beat heart rate variability (HRV) of quadriplegic and paraplegic male subjects at rest in the supine position. In agreement with our previous study, in nine of 15 quadriplegic patients only the high-frequency (HF: center frequency = respiratory frequency) component (a marker of vagal modulation of heart rate) was observed. In contrast, in six of the quadriplegic patients both the HF component and the low-frequency (LF: center frequency at approx. 0.1 Hz, 0.03-0.15 Hz in this study) component (a marker of sympathet

Amplitude reduction of H-reflex during mental movement simulation in elite athletes.

In order to investigate the changes of motoneuron excitability during mental simulation of a voluntary movement (motor imagery; MI), the soleus H-reflex and several activities of autonomic effectors were recorded simultaneously when MI of speed skate sprint was performed. The subjects were seven elite speed skate athletes ranging in age from 18 to 24 years old. They were all skilled in MI, and could imagine full vivid skating movement internally as if they were really exercising. The subjects were awake, relaxed and blindfolded. At first, rest (5 min), positive relaxation (4 min) and concentra

[Pure autonomic failure].

We present the case of a 64-year-old woman who, in the past 5 years, complained of constipation/diarrhea, hyposudoresis, xerostomia and xerophthalmia, dysuria and orthostatic hypotension. Cardiovascular reflexes analysis revealed sympathetic and parasympathetic failure. Norepinephrine was markedly reduced, both lying and after tilt. Norepinephrine infusion determined a significant rise in blood pressure, allowing the diagnosis of denervation hypersensitivity. The diagnosis of pure autonomic failure was made. Therapy with 9 alpha fludrocortisone and metoclopramide was initiated with marked and

Motor evoked potentials from the bladder on magnetic stimulation of the cauda equina: a new technique for investigation of autonomic bladder innervation.

Although neurophysiological techniques have become widely accepted in the diagnostic evaluation of urogenital tract dysfunction, arguments have been forwarded to question their validity. Current techniques merely deal with somatosensory nerves, whereas micturition and sexual functions depend on autonomic motor nerve function as well. This study presents a new technique of motor evoked potentials obtained after magnetic stimulation of the cauda equina. Bladder and pelvic floor motor evoked potentials could be measured with concentric needle electrodes. Mean latency was 35.6 msec. for bladder mo
