Radionuclide Cisternography Revealed Neck Cyst Formed by Cerebrospinal Fluid Leakage After Cervical Spine Surgery.

A 71-year-old man presented with nausea, vomiting, headache, and neck swelling two months after cervical spine surgery. Initial brain computed tomography and magnetic resonance imaging revealed ventricular dilation, suggestive of hydrocephalus. However, radionuclide cisternography subsequently demonstrated cerebrospinal fluid (CSF) leakage into a prevertebral cyst, rather than normal pressure hydrocephalus, thereby confirming communication between the prevertebral cyst and the spinal canal. These findings suggest that the patient's postoperative symptoms were likely attributable to CSF leakage

Haemoperitoneum in the setting of therapeutic anticoagulation and manual assisted cough for a patient with an acute cervical spinal cord injury and pulmonary embolus: a case report.

INTRODUCTION: Respiratory complications are the leading cause of morbidity and mortality in people with spinal cord injuries (SCI). Respiratory muscle weakness and altered mechanics occur in patients with cervical SCI, resulting in impaired airway clearance and low lung volumes. Respiratory physiotherapy includes cough augmentation techniques, such as manual assisted cough and/or mechanical insufflation-exsufflation (MI-E) to address these problems. Additionally, this population is at risk of pulmonary embolism, often receiving therapeutic anticoagulation. CASE REPORT: This case study present

Pathologies of the cervical spine in skeletal syndromes and dysplasias.

Skeletal syndromes and dysplasias include more than 150 entities, most often of genetic origin. Some of them cause abnormalities in the cervical spine, with or without instability, distortion or compression of the spinal cord. These abnormalities must be detected and treated if necessary because they can have serious consequences such as quadriplegia. Up to 30% of patients with Down syndrome are affected by occipitocervical or atlantoaxial instability. Dynamic cervical spine radiographs are the most common screening tool. Mucopolysaccharidoses (MPS) are a group of inherited lysosomal storage d

Intradural anastomoses between the cervical spinal nerves: Anatomical study.

INTRODUCTION: Peripheral nerves can present anastomoses at different levels, with intraspinal anastomoses being relatively common, but little studied. OBJECTIVE: To study the presence and number of intradural anastomoses of the cervical spinal nerves in the Latin population. MATERIALS AND METHODS: 14 adult corpses of both sexes were dissected, fixed in formaldehyde solution. Sex of the corpse, existence of anastomosis, side, height of the anastomoses and sensory/motor origin were recorded. RESULTS: of the 14 corpses, 8 were male and 6 were female. Eleven cases presented anastomoses (79%), 8

Cervical dumbbell meningioma exhibiting distinct histopathological subtypes in intradural and extradural components: a case report.

BACKGROUND: Spinal dumbbell meningiomas are rare, accounting for only 4–5% of all spinal meningiomas. Histological heterogeneity within the same dumbbell lesion is even rarer, with all previously reported cases involving coexistence of a meningioma with either a schwannoma or a neurofibroma. To our knowledge, this is the first report of distinct meningioma subtypes occurring in separate anatomical compartments of a single dumbbell-shaped meningioma, a novel finding with important implications for surgical planning and outcomes. CASE PRESENTATION: A 57-year-old man presented with progressive my

Operative Timing in Cervical Spine Trauma.

STUDY DESIGN: Narrative review. OBJECTIVE: To review existing literature regarding surgical timing in cervical trauma with a focus on acute traumatic central cord syndrome. SUMMARY OF BACKGROUND DATA: Traumatic central cord syndrome is the most common incomplete spinal cord injury. Substantial basic science literature has proposed ischemic and secondary injury-driven mechanisms underpinning the urgency of operative intervention. However, only recently has a relative consensus emerged in the clinical literature regarding the safety, efficacy, and necessity of early operative intervention for

Spinal Meningeal Mass Lesion: A Rare Presentation of Primary Dural Follicular Lymphoma.

INTRODUCTION: The differential diagnosis of a spinal intradural extramedullary mass lesion is broad and includes meningioma, schwannoma, neurofibroma, leptomeningeal metastasis, and myxopapillary ependymoma. Though rare, lymphoma should be included in the differential diagnosis of a dural mass lesion. CASE REPORT: A 38-year-old man presented with back pain that progressed over 1 month with associated focal tenderness over his mid to lower thoracic spine. He developed intermittent numbness of the bilateral lower extremities, nuchal rigidity, difficulty sleeping, and night sweats. A magnetic re

A case of dural arteriovenous fistula at the craniocervical junction successfully treated by transvenous embolization.

Arteriovenous fistula (AVF) occurring within the craniocervical junction (CCJ) is an uncommon vascular anomaly. A 56-year-old man presented with cervical pain and gait disturbances. Computed tomography revealed a hematoma in the epidural space at the cervical spinal level. Subsequent cerebral angiography revealed an epidural shunt at the CCJ, leading to a diagnosis of epidural AVF (EDAVF). Transvenous embolization, performed under general anesthesia, successfully resolved the shunt. Transarterial embolization of CCJ-EDAVFs carries a substantial risk of ischemic complications. Conversely, trans

Table-mounted Versus Self-retaining Retraction: An Assessment of Postoperative Dysphagia Following Anterior Cervical Spine Surgery.

STUDY DESIGN: Retrospective study. OBJECTIVE: To determine whether there are significant differences in postoperative dysphagia when using table-mounted versus self-retaining retractor tools. SUMMARY OF BACKGROUND DATA: Retraction of prevertebral structures during anterior cervical spine surgery (ACSS) is commonly associated with postoperative dysphagia or dysphonia. Retractors commonly used include nonfixed self-retaining retraction devices or fixed table-mounted retractor arms. However, there is a paucity of literature regarding differences in dysphagia between retractor types. METHODS: P

Action protocol of medical staff for airway obstruction after anterior cervical spine surgery: A systematic review of case reports.

BACKGROUND: Postoperative airway obstruction after anterior cervical spine surgery (ACSS) can be a fatal complication. Occasionally, it rapidly progresses to complete obstruction. There are no established standardized protocols on how medical staff should assess for signs and symptoms, seek help, or facilitate airway management after ACSS to prevent unfavorable events. This study aimed to primarily describe a systematic approach by assessing the signs and treatment outcomes of airway compromise in patients who underwent ACSS. Further, it recommended an action protocol after extubation for medi

Classical Ehlers-Danlos syndrome with cranio-cervical instability in an infant due to a novel COL5A1 gene mutation.

Ehlers-Danlos syndromes (EDSs) are a group of connective tissue disorders with diverse clinical and genetic profiles. Classical Ehlers-Danlos syndrome (cEDS), the second most common type, is characterised by skin hyperextensibility, atrophic scars and joint hypermobility, primarily due to mutations in COL5A1 and COL5A2 genes. We report a case of an infant with severe cEDS presenting with motor developmental delay, joint hyperextensibility and cranio-cervical instability. Genetic analysis identified a novel heterozygous missense mutation in COL5A1 (c.386G>T) and a non-segregating variant in COL

Step-by-Step Description of Percutaneous Full-Endoscopic C2 Ganglionectomy: An Anatomic Feasibility Study in Human Cadavers.

BACKGROUND AND OBJECTIVES: The percutaneous full-endoscopic C2 ganglionectomy (PEC2G), an innovative procedure developed for the surgical treatment of intractable occipital neuralgia, was firstly reported by us in 2021. However, a universally accepted and well-articulated protocol modality remains elusive. The primary objective of this anatomic investigation was to meticulously elucidate the standard procedural steps of PEC2G and assess the anatomic features supporting the safe implementation of PEC2G. METHODS: Eighteen fresh adult cadavers were incorporated into this study. From this sample,

A Novel Ultrasonographic Method to Quickly and Accurately Access the C2 Dorsal Root Ganglion.

BACKGROUND: Occipital neuralgia manifests as pain in the cutaneous distribution of occipital nerves, with the greater occipital nerve stemming from the C2 spinal nerve and the lesser occipital nerve originating from the C2 and C3 spinal nerves. While pulsed radiofrequency ablation of the C2 dorsal root ganglion (DRG) is an effective treatment for refractory occipital neuralgia, accessing the C2 DRG remains a clinical challenge even under fluoroscopic guidance. OBJECTIVE: We aimed to develop an ultrasonographic method for quickly and accurately accessing the C2 DRG. STUDY DESIGN: This is a pr

Automated Segmentation and Diagnostic Measurement for the Evaluation of Cervical Spine Injuries Using X-Rays.

Accurate assessment of cervical spine X-ray images through diagnostic metrics plays a crucial role in determining appropriate treatment strategies for cervical injuries and evaluating surgical outcomes. Such assessment can be facilitated through the use of automatic methods such as machine learning and computer vision algorithms. A total of 852 cervical X-rays obtained from Gachon Medical Center were used for multiclass segmentation of the craniofacial bones (hard palate, basion, opisthion) and cervical spine (C1-C7), incorporating architectures such as EfficientNetB4, DenseNet201, and Incepti

History of Cervical Spine Localization: Surface Landmarks.

Interest in the relational anatomy of the anterior neck among spine surgeons increased after the popularization of anterior cervical discectomy and fusion in the 1950s. Five surface landmarks and their relationship to specific spinal levels have been routinely described, comprising the angle of the mandible for the C2-C3 interspace, the hyoid bone for the C3 body, the thyroid cartilage for the C4-C5 disc, and the cricoid cartilage and carotid tubercle for the C6 body. We aimed to investigate the origins of these anatomic relationships and their application in modern practice.
