The impact of session-introducing mindfulness and relaxation interventions in individual psychotherapy for children and adolescents: a randomized controlled trial (MARS-CA).

BACKGROUND: The investigation of mindfulness-based interventions (MBIs) in cognitive-behavioral therapy has greatly increased over the past years. However, most MBI research with youth focuses on structured, manualized group programs, conducted in school settings. Knowledge about the implementation and effects of MBIs in individual psychotherapy with children and adolescents is scarce. To fill this research gap, the "Mindfulness and Relaxation Study - Children and Adolescents" (MARS-CA) is designed. It aims to assess the effects of short session-introducing interventions with mindfulness eleme

Development of a cognitive bias modification intervention for anxiety disorders in primary care.

OBJECTIVES: There is a great need for low-intensity, scalable treatments in primary care, where most anxious patients first present for treatment. We describe Stage IA treatment development and a Stage IB feasibility trial of cognitive bias modification (CBM) for transdiagnostic anxiety in primary care. METHODS: The online intervention, Mental Habits, comprised eight sessions of a personalized CBM targeting attention and interpretation biases. Coaches assisted patients in using the website, monitored progress via a dashboard, and shared information with primary care providers. We evaluated Me

Taking care of our future doctors: a service evaluation of a medical student mental health service.

BACKGROUND: Studies suggest medical students experience high levels of mental distress during training but are less likely, than other students, to access care due to stigma and concerns regarding career progression. In response, The School of Clinical Medicine, University of Cambridge supported the development of the 'Clinical Student Mental Health Service' to provide specialist input for this vulnerable group. This study evaluates the efficiency and effectiveness of this service. METHODS: Using mixed-methods, cross-sectional analysis of validated psychiatric rating scales and qualitative fe

Novel Psychological Formulation and Treatment of "Tic Attacks" in Tourette Syndrome.

One important, but underreported, phenomenon in Tourette syndrome (TS) is the occurrence of "tic attacks." These episodes have been described at conferences as sudden bouts of tics and/or functional tic-like movements, lasting from 15 min to several hours. They have also been described by patients in online TS communities. To date, there are no reports of tic attacks in the literature. The aim of this article is to stimulate discussion and inform clinical practices by describing the clinical presentation of 12 children (mean age 11 years and 3 months; SD = 2 years and 4 months) with TS and tic

Effectiveness of dual-task functional power training for preventing falls in older people: study protocol for a cluster randomised controlled trial.

BACKGROUND: Falls are a major public health concern with at least one third of people aged 65 years and over falling at least once per year, and half of these will fall repeatedly, which can lead to injury, pain, loss of function and independence, reduced quality of life and even death. Although the causes of falls are varied and complex, the age-related loss in muscle power has emerged as a useful predictor of disability and falls in older people. In this population, the requirements to produce explosive and rapid movements often occurs whilst simultaneously performing other attention-demandi

Visual rating system for assessing magnetic resonance images: a tool in the diagnosis of mild cognitive impairment and Alzheimer disease.

OBJECTIVE: Subjects with mild cognitive impairment (MCI) and mild Alzheimer disease (AD) can be distinguished from elderly subjects with no cognitive impairment (NCI) by the degree of atrophy in the entorhinal cortex (ERC) and the hippocampus (HPC), quantified by volumetric magnetic resonance image (MRI) studies. Because volumetric MRI requires rigorous standards for image acquisition and analysis and is not suitable for routine clinical use, we have used calibrated visual rating to measure atrophy in the ERC, HPC, and perirhinal cortex (PRC) and evaluated its utility in the diagnosis of very

Efficacy of a Prescription-Based Mobile Digital Therapeutic as an Adjunct to Pharmacotherapy for the Acute Phase of Panic Disorder: Multicenter Randomized Controlled Trial.

BACKGROUND: Although pharmacotherapy is the primary treatment for patients with acute-phase panic disorder, its incomplete efficacy causes them to experience frequent panic attacks and severe anticipatory anxiety for a considerable period. A prescription-based mobile digital therapeutic (DTx) that integrates self-guided cognitive behavioral therapy (CBT), real-time symptom management, and lifestyle tracking can be used as an adjunct to pharmacotherapy for these patients, helping them achieve rapid symptom recovery. OBJECTIVE: This study aimed to evaluate the efficacy of this adjunctive DTx in

Not Everything Has a Single Diagnosis: A Case of Hematemesis, Pneumothorax, and Acute Pancreatitis Mimicking Boerhaave Syndrome.

Patients presenting with multiple unrelated complaints are a common occurrence in the emergency department and one that poses significant difficulty. While we as physicians are trained to look for a unifying diagnosis, we must never forget "Hickam's dictum," stating that a patient may have multiple simultaneous diagnoses. A 48-year-old man presented to the emergency department with a history of hematemesis, acute shortness of breath, epigastric pain, and right-sided chest pain, after a bout of alcohol binge. On examination, the patient had reduced air entry on the right side, raising suspicion

Evaluating StyleGAN2-ADA and f-AnoGAN for GAN based unsupervised anomaly detection in gastrointestinal endoscopy.

Manual interpretation of gastrointestinal (GI) endoscopy images is a subjective and time-intensive process, often leading to variability between clinicians and the risk of missed abnormalities. To address these challenges, we investigate the use of StyleGAN2-ADA as a generative model for unsupervised anomaly detection in gastrointestinal endoscopy images. StyleGAN2-ADA combines adaptive discriminator augmentation with style-based generative modelling, enabling the model to capture both global anatomical structures and fine-grained mucosal textures. In this work, the performance of StyleGAN2-AD

Standardizing EEG preprocessing for cross-site integration - the CLEAN pipeline.

Electroencephalography (EEG) is a powerful tool for investigating neural processes underlying cognition and neuropsychiatric disorders. Yet, variability in EEG preprocessing strategies restricts reproducibility and data integration across study sites and laboratories, particularly in larger research consortia. This paper introduces the CLEAN-EEG preprocessing pipeline, designed to standardize data processing and documentation across multiple sites. The CLEAN pipeline is implemented in MATLAB using EEGLAB. It comprises three modular, script-based stages: main preprocessing (including down-sampl

Effects of Notation Type and Score Difficulty on Eye Movements During Erhu Sight-Reading.

A parallel notational system comprising number notation and staff notation has long been employed in the pedagogy of traditional Chinese instrumental music. However, it remains unclear how these two representationally distinct notational systems shape performers' visual-processing strategies. Therefore, using eye-tracking methods, the present study examined the interactive effects of notation type and score difficulty on visual-cognitive processing in a sight-reading task performed on the erhu, a Chinese two-stringed bowed musical instrument. The results showed that increased difficulty genera

Interpretable 2D Deep Learning for Alzheimer's Detection from sMRI: A Lightweight Residual CNN Approach with Comprehensive Preprocessing and Stratified Data Partitioning.

Neuroimaging is a promising modality for early AD detection, facilitating timely clinical intervention. This study proposes an enhanced deep learning framework that extracts critical AD biomarkers from structural MRI (sMRI) data acquired from the ADNI. Our novel CNN architecture integrates conventional convolutional layers with residual and skip connections for efficient feature extraction, achieving substantially lower computational cost than standard deep architectures such as VGG-16 (138 M), while remaining more parameter-intensive than highly compact architectures such as MobileNet and Eff

Robust encoding of stimulus-response mapping by neurons in visual cortex.

Neural activity in sensory cortex is modulated by behavioral and cognitive factors, and this modulation is thought to contribute to the selection of specific sensory information needed to achieve behavioral goals. In contrast, more abstract behavioral variables that are independent of stimulus selection, such as stimulus-response mapping, are thought to be encoded by neurons outside of sensory cortex. We show that information about such mapping is robustly encoded in the responses of neurons in primate visual cortex. Monkeys were trained to alternate between two tasks that differed in the rule

AI-assisted computational screening and docking simulation prioritize marine natural products for small-molecule PCSK9 inhibition.

SARS-CoV-2 infection has been associated with long-term cardiovascular complications including myocarditis and heart failure, as well as central nervous system sequelae such as cognitive dysfunction and neuropathy. Proprotein convertase subtilisin/Kexin type 9 (PCSK9), a hepatic protease involved in cholesterol regulation, has shown associations with a spectrum of diseases potentially relevant to these Covid-19 complications, such as atherosclerosis. To identify novel human PCSK9 inhibitors, a custom virtual screening pipeline was developed employing (1) a convolutional neural network-based de
