This study evaluates our complications arising directly or indirectly from placement or management of percutaneous endoscopic gastrostomy (PEG) tubes and provides recommendations for avoidance of complications. Seventy-one patients received PEG tube placement by otolaryngologists between January 1991 and May 1997. Records were reviewed for diagnoses, combined procedures, and complications. Addressing potential causes of complications prompted modification of our technique of PEG tube placement and management. We present a case of juvenile myelomonocytic leukemia (JMML) with ocular infiltration. A 1-month-old boy presented with myeloid precursors in peripheral blood and a white blood cell count >10x10(9)/l. His peripheral blood monocyte count was >1x10(9)/l, bone marrow blasts were <20%, and no Ph chromosome was identified. The boy also presented with hepatosplenomegaly, pallor, fever, and skin rash. PURPOSE: A new modification for urinary diversion following cystectomy is presented.
MATERIALS AND METHODS: A total of 20 patients with bladder cancer who were treated with radical cystectomy underwent urinary diversion using a new technique. The sigmoid colon is dismembered approximately 30 cm. proximal to the peritoneal reflection and detubularized by anterior incision in its proximal two-thirds. Intractable detrusor overactivity can result in considerable morbidity and, in the case of neurogenic bladder dysfunction, can put the upper tracts at risk. Colovesical fistulas are a serious complication of diverticular disease. Management by one-stage resection and anastomosis has resulted in lower morbidity and shorter hospital stay. Nevertheless, hospital time remains long, approaching an average of 2-3 weeks. Here we describe our technique of laparoscopic approach to colovesical fistulas. Our initial experience suggests that this is a safe operation with minimal pain, absent ileus, and a short postoperative stay.

The prognostic significance of epithelioid granulomas in Crohn's disease (CD) remains controversial. We have determined the prognostic significance of epithelioid granulomas noted in endoscopic rectosigmoid biopsies obtained from untreated pediatric patients at initial presentation of CD. Transabdominal posterior rectopexy with resection of the redundant left colon (Frykman-Goldberg operation) was performed on 48 selected patients with complete rectal prolapse. Uterine suspension was also performed on most of the women. The 30-day mortality rate was 2.1%. Prolapse recurred in 4 (9%) of the 45 patients followed up for 1-10 (mean 4.3) years. Over a period of 3 years. 10 cases of enterocystoplasties have been done in congenital neurologic bladder in children. Seven patients have been operated using sigmoide (6), caecum (1) in their tubular form. In the last 3 cases sigmoide was used as a patch without any resection of the bladder. Post-operative urodynamic testings were better with sigmoide patch. The authors pointed out this technique.

Some aspects of surgical tactics in 57 patients with sigmoid ileus are discussed. In 42 patients palliative operations were performed, 7 patients (16.7%) died. In 15 cases radical operations were performed. Palliative operations are recommended only for aged patients, but in most cases sigmoidectomy seems to be warranted.

A poorly differentiated adenocarcinoma of the rectum developed in a 43-year-old man 37 years after ureterosigmoidostomy for epispadias with urinary incontinence.
