Intracranial Infectious Aneurysm in Orbital Cellulitis.

Infectious intracranial aneurysm and cavernous sinus thrombosis are rare complications of orbital cellulitis. We report the case of a 46-year-old male presenting with sinusitis and orbital cellulitis complicated by the development of an orbital mass. Following orbitotomy with debulking, the patient underwent bony orbital decompression for increasing proptosis postoperatively. While his exam stabilized, the patient developed complete ptosis and extraocular motor palsy in the contralateral eye after undergoing bilateral sinus debridement. Imaging was notable for the presence of a pseudoaneurysm

Antiviral agents convey added benefit over steroids alone in Bell's palsy; decompression should be considered in patients who are not recovering.

BACKGROUND: The management of Bell's palsy has been the subject of much debate, with corticosteroids being the preferred medication. However, evidence also supports the use of antiviral drugs for severe cases and even decompression surgery in patients who, despite medical treatment, are not recovering. METHOD: A literature review was conducted on the management of Bell's palsy. RESULTS: This paper describes the background, statistical evidence, study results and pathophysiological theories that support more aggressive treatment for patients with severe palsy and those who have inadequate rec

Double Crush Syndrome.

Double crush syndrome is a distinct compression at two or more locations along the course of a peripheral nerve that can coexist and synergistically increase symptom intensity. In addition, dissatisfaction after treatment at one site may be the result of persistent pathology at another site along a peripheral nerve. Double crush syndrome is a controversial diagnosis; some scientists and surgeons believe it is an illness construction that may do more harm than good because it emphasizes an objective pathophysiologic explanation for unexplained symptoms, disability, and dissatisfaction that may

An interesting case of intractable proctalgia cured with decompression spinal surgery.

We report a case of intractable proctalgia on a background of lower back and bilateral leg pain secondary to spinal canal stenosis and spondylolisthesis. A complete resolution was observed following spinal surgery. This case highlights consideration of a spinal aetiology when intractable proctalgia is associated with back and leg pain.

Twenty-year follow-up of flow reversal and revascularization for a giant serpentine basilar artery aneurysm.

BACKGROUND AND IMPORTANCE: Current microsurgical and endovascular therapies have offered little advancement for the treatment of complex vertebrobasilar aneurysms. The outcome of patients with these rare lesions has remained poor, despite sometimes heroic measures. CLINICAL PRESENTATION: The authors report a case of a 65-year-old man who 20 years earlier had presented with symptoms suggestive of brainstem compression. Imaging at the time revealed a giant, serpentine aneurysm of the basilar artery. The patient was treated with superficial temporal artery to superior cerebellar artery bypass an

Percutaneous endoscopic decompression for lumbar spinal stenosis.

Percutaneous endoscopic lumbar discectomy has become a representative minimally invasive spine surgery for lumbar disc herniation. Due to the remarkable evolution in the techniques available, the paradigm of spinal endoscopy is shifting from treatments of soft disc herniation to those of lumbar spinal stenosis. Lumbar spinal stenosis can be classified into three categories according to pathological zone as follows: central stenosis, lateral recess stenosis and foraminal stenosis. Moreover, percutaneous endoscopic decompression (PED) techniques may vary according to the type of lumbar stenosis,

Spinal subdural hematoma following lumbar decompressive surgery: a report of two cases.

Spinal subdural hematoma (SSDH) following spine surgery is an extremely rare condition, with only three cases being reported in the literature. Unintended durotomy has been associated with SSDH due to alterations of pressures in the dural compartments. The objective of the present report was to report two rare cases of acute SSDH developed after lumbar decompressive surgery. In one of the patients, the diagnosis of SSDH was followed by urgent hematoma evacuation via durotomy due to the patient's worsening neurological symptoms. In the second patient, the SSDH was treated conservatively due to

Extensive ischemic brainstem lesions and pneumocephalus after application of hydrogen peroxide (H2O2) during lumbar spinal surgery.

BACKGROUND CONTEXT: The hemostatic properties of hydrogen peroxide (H2O2) are often used in neurosurgical practice. CASE REPORT: We present the case of an 81-year-old woman who underwent lumbar spinal surgery (microsurgical decompression) in an external hospital. H2O2 was used during the procedure. The patient was transferred to our hospital. She remained unconscious postoperatively, with progressive loss of brainstem reflexes. Computed tomography showed intra- and extradurally trapped air ascending from the operated lumbar segment up to frontal lobe. Magnetic resonance imaging demonstrated s

Holospinal epidural abscess of the spinal axis: two illustrative cases with review of treatment strategies and surgical techniques.

Despite the increasing prevalence of spinal infections, the subcategory of holospinal epidural abscesses (HEAs) is extremely infrequent and requires unique management. Panspinal imaging (preferably MRI), modern aggressive antibiotic therapy, and prompt surgical intervention remain the standard of care for all spinal axis infections including HEAs; however, the surgical decision making on timing and extent of the procedure still remain ill defined for HEAs. Decompression including skip laminectomies or laminoplasties is described, with varied clinical outcomes. In this review the authors presen

Acute spinal subdural hematoma with hemiplegia after acupuncture: a case report and review of the literature.

BACKGROUND CONTEXT: Subdural spinal hematoma (SDH) is a very rare entity; however, it can lead to serious complications resulting from injuries to the spinal cord and roots. Although acupuncture has been a popular method for the management of pain control, we encountered the first case of SDH after acupuncture. PURPOSE: The purpose of this case report was to present the first case of subdural hematoma after acupuncture and the reasons for the risks of blind cervical acupuncture. STUDY DESIGN: A case report and review of the previous literature are presented. METHODS: A 69-year-old man compl

Civilian gunshot injuries of the spinal cord: a systematic review of the current literature.

BACKGROUND: The principles that guide management of spinal cord injury (SCI) derive from injury resulting from blunt trauma, not gunshot wounds. Civilian gunshot-induced spinal cord injury (CGSWSCI) is a common, potentially serious cause of neurological deficit; there is disagreement about whether the same approaches used for SCI caused by blunt-force trauma should apply to gunshot-induced SCI. QUESTIONS/PURPOSES: We reviewed the literature to answer the following questions regarding presentation and outcome of gunshot wound-induced SCI: (1) Are there differences in recovery prognosis between

Lead toxicity and management of gunshot wounds in the lumbar spine.

INTRODUCTION: Gunshot wounds are one of the commonest causes of spinal injury. Management of these patients differs from other blunt trauma injuries to the spine. We present a case of a gunshot wound to the lumbar spine that occurred in 1985 which was treated non-operatively. METHODS: In the last 10 years, the patient was admitted multiple times for confusion and lead toxicity with blood levels over 100 μg/dl. Inpatient chelation therapy was implemented. After multiple recommendations for surgery, the patient agreed to have as much of the bullet removed as possible. The patient successfully u

Surgical treatment of Klippel-Feil syndrome with basilar invagination.

INTRODUCTION: Klippel-Feil syndrome (KFS) is a congenital cervical vertebral union caused by a failure of segmentation during abnormal development and frequently accompanies conditions such as basicranial malformation, atlas assimilation, or dens malformation. Especially in basilar invagination (BI), which is a dislocation of the dens in an upper direction, compression of the spinomedullary junction from the ventral side results in paralysis, and treatment is required. CLINICAL PRESENTATION: We present the case of a 38-year-old male patient with KFS and severe BI. Plane radiographs and comput

Surgical treatment of ossification of the posterior longitudinal ligament and its outcomes: posterior surgery by laminoplasty.

STUDY DESIGN: Review article. OBJECTIVE: To discuss the history, indication, surgical outcomes, prognostic factors, and complications of laminoplasty for ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. SUMMARY OF BACKGROUND DATA: OPLL usually requires surgery, which often involves indirect decompression by the posterior approach. Of the posterior procedures for OPLL, laminoplasty is the favored treatment. Here, we review the previously published literature on laminoplasty for OPLL. METHODS: PubMed and previously published English and Japanese book chapters
