Outcomes of real-ear measurement-based fitting in unilateral hearing aid users: Influence of contralateral hearing.

OBJECTIVES: Real-ear measurement (REM) is commonly used to verify hearing aid fittings; however, its added benefit over non-REM fitting in unilateral users and the influence of contralateral ear hearing remain unclear. In this retrospective study, we compared short-term audiometric and patient-reported outcomes between an REM-based fitting pathway and a manufacturer-first-fit pathway followed by routine clinical fine-tuning in unilateral hearing-aid users, and examined associations with contralateral-ear hearing. METHODS: We retrospectively analyzed data from 105 unilateral hearing aid users …

A viscosity-tunable microgel-in-sol phase for localized inner ear delivery of small hydrophilic drug.

Cisplatin-induced ototoxicity leads to irreversible hearing loss in approximately 50% of treated patients, largely due to the cochlea's exceptionally poor cisplatin clearance compared with other organs. Although sodium thiosulfate (STS), a hydrophilic small molecule, is an established otoprotectant, achieving therapeutically sufficient cochlear concentrations remains challenging because systemic administration is limited by the blood-labyrinth barrier. Here, we report a viscosity-tunable microgel-in-sol system based on hyaluronic acid (HA) that enables sustained, localized delivery of STS to t…

Detection and Continuous Tracking of Breeding Pigs with Ear Tag Loss: A Dual-View Synergistic Method.

The lossof ear tags in breeding pigs can lead to the loss or confusion of individual identity information. Timely and accurate detection, along with continuous tracking of breeding pigs that have lost their ear tags, is crucial for improving the precision of farm management. However, considering the real-time requirements for the detection of ear tag-lost breeding pigs, coupled with tracking challenges such as similar appearances, clustered occlusion, and rapid movements of breeding pigs, this paper proposed a dual-view synergistic method for detecting ear tag-lost breeding pigs and tracking i…

Hybrid Auricular Framework of Autologous Rib and Ear Cartilage for Microtia Reconstruction through a Preauricular Incision.

BACKGROUND: Fabrication of the auricular framework is a key aspect of ear reconstruction. The use of autologous rib cartilage, which is considered an ideal grafting material, can minimize harvesting damage and optimize its use. This study introduces a method that combines both rib and ear cartilages to reconstruct the auricular framework using an anterior auricular approach, which can help achieve optimal results. METHODS: Between 2020 and 2024, 153 patients with microtia underwent ear reconstruction using a hybrid framework of autologous rib and ear cartilage. Reconstruction was performed th…

Molecular identity of the mechanotransduction machinery in inner ear hair cells and mechanotransduction-linked hearing loss.

Hearing is among the most sensitive sensory modalities in vertebrates, and its dysfunction can lead to hearing loss. Auditory perception within the inner ear relies on mechanosensitive hair cells and a sophisticated network of molecules that comprise the machinery responsible for mechanotransduction. The mechanosensory hair cells exhibit distinct structural features that enable them to transform sound vibrations into electrical signals, which are then relayed to the spiral ganglion neurons (SGNs) that are connecting between the cochlear hair cells and the cochlear nuclei with high accuracy and…

Intraoperative ABR measurements via the active middle ear implant vibrant soundbridge using different types of couplers.

PURPOSE: The Vibrant Soundbridge active middle ear implant is indicated for the treatment of different types of hearing loss. Recently, a novel system for determining implant performance based on auditory brainstem response (ABR) was developed. A reference value for the expected ABR hearing thresholds is determined on the base of a substantial number of measurements in this work. METHOD: ABRs were recorded in patients following VSB surgery. A standard ABR system was employed, utilizing an implemented optimized chirp stimulus, which was transmitted directly to the VSB via the AcoustiAP transmi…

Independent effects of sex and ear-canal size on auditory brainstem response amplitudes in young adults.

OBJECTIVE: Adult auditory brainstem response (ABR) amplitudes are employed by both researchers and clinicians, but exhibit substantial between-subject variability, reducing their sensitivity as measures of underlying auditory health. One source of variability is sex, for which a number of mechanisms have been proposed. The present analysis aimed to determine whether ear-canal size influences ABR wave I amplitude, independent of sex. DESIGN: ABR wave I amplitudes and estimates of ear-canal diameter and volume were obtained from 220 otologically healthy teenagers. Multiple linear regression mod…

Proton-FLASH: effects of ultra-high dose rate irradiation on an in-vivo mouse ear model.

FLASH-radiotherapy may provide significant sparing of healthy tissue through ultra-high dose rates in protons, electrons, and x-rays while maintaining the tumor control. Key factors for the FLASH effect might be oxygen depletion, the immune system, and the irradiated blood volume, but none could be fully confirmed yet. Therefore, further investigations are necessary. We investigated the protective (tissue sparing) effect of FLASH in proton treatment using an in-vivo mouse ear model. The right ears of Balb/c mice were irradiated with 20 MeV protons at the ion microprobe SNAKE in Garching near M…

Novel inductively coupled ear-bars (ICEs) to enhance restored fMRI signal from susceptibility compensation in rats.

Functional magnetic resonance imaging faces inherent challenges when applied to deep-brain areas in rodents, e.g. entorhinal cortex, due to the signal loss near the ear cavities induced by susceptibility artifacts and reduced sensitivity induced by the long distance from the surface array coil. Given the pivotal roles of deep brain regions in various diseases, optimized imaging techniques are needed. To mitigate susceptibility-induced signal losses, we introduced baby cream into the middle ear. To enhance the detection sensitivity of deep brain regions, we implemented inductively coupled ear-b…

Nanomechanics of wild-type and mutant dimers of the inner-ear tip-link protein protocadherin 15.

Mechanical force controls the opening and closing of mechanosensitive ion channels atop the hair bundles of the inner ear. The filamentous tip link connecting transduction channels to the tallest neighboring stereocilium modulates the force transmitted to the channels and thus changes their probability of opening. Each tip link comprises four molecules: a dimer of protocadherin 15 (PCDH15) and a dimer of cadherin 23, all of which are stabilized by Ca2+ binding. Using a high-speed optical trap to examine dimeric PCDH15, we find that the protein's mechanical properties are sensitive to Ca2+ and …

Scuba Diving-Induced Inner-Ear Pathology: Imaging Findings of Superior Semicircular Canal and Tegmen Tympani Dehiscence.

BACKGROUND Superior semicircular canal dehiscence is an inner-ear pathology which presents with vertigo, disequilibrium, and hearing loss. Although the exact etiology of superior semicircular canal dehiscence is unknown, it is thought that an increase in middle-ear pressure disrupts a thin overlying temporal bone. Superior semicircular canal dehiscence is frequently seen in association with dehiscence of the tegmen tympani, which overlies the middle ear. Here, we present a case report of a 52-year-old Puerto Rican man with vertigo, dizziness, vomiting, and mild hearing loss associated with sup…
