Combining responsive neurostimulation with resection or ablation for treatment of epilepsy: a multicenter experience.

OBJECTIVE: Despite advances in surgical and neuromodulatory treatments, a substantial proportion of patients with drug-resistant epilepsy remain ineligible for standalone resection or ablation. Combining resection or laser interstitial thermal therapy (LITT) with responsive neurostimulation (RNS) has emerged as a promising approach for patients with multifocal seizure onset or eloquent cortex involvement. However, the clinical impact of combination surgery remains poorly characterized. The aim of this study was to characterize the clinical rationale and surgical decision-making used across mul…

Changing Epidemiological Patterns of Epilepsy in China, 1990-2023, with Projections to 2050.

INTRODUCTION: Epilepsy remains a major source of chronic neurological morbidity and long-term care need. In China, its contemporary burden is likely being reshaped by population ageing and by shifts in the balance between idiopathic and secondary epilepsy, yet updated national evidence integrating subtype-specific burden, demographic decomposition, and future projections remains limited. METHODS: We performed a secondary analysis of the Global Burden of Disease Study 2023 to assess epilepsy burden in China from 1990 to 2023, using global estimates for contextual comparison. We examined preval…

Prevalence of epilepsy and the burden of underlying causes: Canadian validation of Danish estimates.

OBJECTIVE: Recent work has provided population-based estimates of the prevalence of cause-specific epilepsy across the lifespan in Denmark. We sought to externally validate these Danish "secondary epilepsy" findings to ascertain consistency across geographic regions. METHODS: We conducted a population-based cross-sectional study using linked Alberta Health Care Insurance Plan administrative datasets. Epilepsy was identified using a validated algorithm. Point prevalence was calculated on December 31, 2018 among residents alive at that date. Secondary epilepsy was assigned when a recognized ant…

Goosebumps and chills: bilateral piloerection as a manifestation of right mesial temporal epilepsy.

Ictal piloerection is a rare autonomic seizure manifestation that can obscure localization and mimic non-epileptic conditions. We report a 21-year-old man with drug-resistant focal epilepsy whose seizures consisted of isolated bilateral piloerection and chills followed by coughing. MRI demonstrated right mesial temporal sclerosis, and FDG-PET showed concordant hypometabolism. Scalp EEG suggested a right temporal onset. Stereo-electroencephalography recorded eleven habitual seizures with electrographic onset in the right amygdala and hippocampus preceding the clinical symptoms. Notably, cortica…

Analysis of the Immunoinflammatory Microenvironment Characteristics of Epilepsy Based on Single-Cell Transcriptomics.

Epilepsy is a common neurological disease that affects a wide range of people and seriously impairs patients' quality of life and social adaptability. Neuroinflammation plays an important role in the pathogenesis of epilepsy. Glial cells, particularly astrocytes and their differentiated forms, as well as their interactions with other cell types, are critical in the process of neuroinflammation. A better understanding of the inflammatory mechanisms of epilepsy might facilitate the development of new therapeutic strategies. Single-cell RNA sequencing has provided new insights into the inflammato…

Characteristics of subclinical seizures in an adult epilepsy population-a retrospective observational study.

PURPOSE: Subclinical seizures (SCSs) are commonly detected during video-EEG monitoring but frequently overlooked in clinical practice. Their prevalence, localization characteristics, and relationship to clinical seizures (CSs) and interictal epileptiform discharges (IEDs) remain incompletely defined. This study aimed to characterize SCS prevalence, localization, frequency, and concordance with CS and IED localization in a presurgical epilepsy cohort, and to examine associations between SCS presence and neuropsychological performance. METHODS: A retrospective observational study was conducted …

Resilience as a mediating mechanism between psychological distress and quality of life in epilepsy.

INTRODUCTION: Psychological distress is a major determinant of impaired health-related quality of life (HRQoL) in people with epilepsy. Although resilience has been associated with better psychosocial adjustment and well-being, its contribution to this relationship remains unclear. We investigated whether resilience mediates the association between psychological distress and HRQoL while adjusting for seizure frequency. METHODS: In this cross-sectional study, 89 adults with focal and generalized epilepsy completed measures of HRQoL (QOLIE-31), psychological symptoms (DASS-42), and resilience (…

Management of Epilepsy in the Brain Tumour Patient: Current Concepts and Practical Approaches.

Brain Tumour-Related Epilepsy (BTRE) is among the most common, disabling and feared complications of primary and metastatic brain tumours. Seizures cause significant suffering, psychosocial burden, reduced quality of life and lead inevitably to the loss of a driving license. The mechanisms underlying BTRE are multifactorial, including tumour-driven alterations in neuronal excitability and pro-epileptogenic metabolites. Management is more challenging than idiopathic epilepsy due to high rates of drug resistance, the natural history of brain tumours and potential interactions between anti-seizur…

Unraveling the role of FGF21 in epilepsy disease: mechanistic insights and therapeutic Potential.

Epilepsy, progressive neurological disorder marked repeated seizures, producing aberrant neuronal excitability, followed by oxidative stress, mitochondrial malfunction, and neuroinflammation. Fibroblast growth factor 21 (FGF21), an endocrine member of the FGF family mainly secreted by liver, adipose tissue, and muscle, has turned out to be a potent neuromodulator in addition to its established as lipid and glucose homeostasis. According to studies, FGF21 can cross the Blood-brain barrier (BBB) and provide neuroprotection in experimental models of epilepsy via attenuation of neuronal hyperexcit…

Music-based intervention as an adjunctive therapy in people with epilepsy: A scoping review.

Epilepsy is a major neurological disorder that affects individuals across all age groups and can significantly impair their quality of life. Although pharmacological treatments are available, there remains a need for complementary non-pharmacological lifestyle interventions. Music-based intervention (MBI) is a possible option, as it is non-invasive, cost-effective, and easily implemented. This scoping review aims to identify and describe MBIs used in the management of epilepsy. A systematic search was conducted from April 9 to June 10, 2025, using two electronic databases: Ovid Medline and Sco…
