Lupus erythematosus (LE)-specific bullous lesions are often difficult to distinguish from other bullous diseases presenting in patients with systemic lupus erythematosus. Herein, we describe a 49-year-old woman with systemic lupus erythematosus with recurrent tense bullae on the forearms. Clinical, histopathologic, and serologic findings led to the diagnosis of LE-specific bullous lesions. We also summarize the diagnostic clues for distinguishing LE-specific bullous lesions, bullous systemic lupus erythematosus, and erythema multiforme-like lesions in LE (Rowell syndrome).

Hand amputation is known to be one of the most debilitating injuries. Its impact on the patient results in multiple diverse outcomes and affects daily life activities and career. With that in mind, we understand how devastating upper limb amputations can be. In this case, we report a middle-aged male who suffered an amputation at the level of the distal forearm and underwent replantation at King Saud Medical City in Riyadh, Saudi Arabia. Dealing with an amputated limb requires knowledge and awareness, starting with the patient and moving on to all healthcare providers involved.

Nonunion following a long bone fracture causes considerable morbidity when it occurs. Focal myositis (FM) is a rare inflammatory myopathy characterized by a painful swelling of a skeletal muscle. The lower limb is the most common site for FM, but it has also been reported in the abdomen, forearm, neck, and tongue musculature. Some conservative treatments such as non-steroidal anti-inflammatory drugs, physiotherapy, and steroids can relieve muscle pain and its minor complications. This report describes a patient who had shoulder pain and was diagnosed with FM in the left trapezius muscle with an excellent response to high-intensity laser therapy.

A long arm cast with the elbow extended is a useful but often forgotten technique to manage some pediatric fractures. Specifically, proximal forearm fractures of both the radius and ulna, olecranon fractures, flexion-type supracondylar fractures and type II Monteggia fractures are well managed in this manner. Electrical injuries make up a relatively small portion of burn injuries. Safety measures in place on domestic electricity supply have reduced the occurrence of high voltage electrical injuries. We present the case of a young man who sustained a high voltage electrical injury on all four limbs. Early fasciotomy was performed on both his hands and forearms. Despite early compartment release, the left upper limb deteriorated and required amputation. In this article we discuss the indications, outcomes and complications of early fasciotomy.

We report a case of secondary hyperparathyroidism after peritoneal dialysis. After given the total parathyroidectomy and forearm autoplantation (PTX+AT), the patient presented with an acute low calcium crisis (tongue myoparalysis, tetany and ECG abnormalities). As the conventional calcium treatment was invalid, the calcium solution was then changed, and replaced by the standard calcium dialysate with monitoring calcium ions. Selection of muscles for botulinum toxin injection for limb dystonia is particularly challenging. Limb dystonias vary more widely in the pattern of dystonic movement and involved muscles than cervical dystonia or blepharospasm. Marine animal injuries are rare causes of emergency room visits and vascular injuries worldwide. Penetrating injuries from marine animals risk damage to vasculature, and physicians must be alert to such possibilities. We report a 7-year-old boy with penetrating trauma and retained foreign body in the forearm from a catfish injury. Initial imaging suggested transection of the radial artery, but on exposure in a controlled setting the foreign body was found to compress the artery without any vascular injury. No vascular repair was needed after foreign body removal.

The aim of this study was to assess the resistance of microsurgically repaired proper palmar digital nerves (PPDN) to mobilization. Thirty-nine PPDN from fresh forearms cadavers were transected and then sutured using 10/0 nylon. After skin closure, each finger was forcefully flexed and extended 10 times. Out of the 39 repaired nerves, two were elongated and four were ruptured; this amounts to a 15.38% complication rate. We recommend immobilization of nerve repairs, although this contradicts most recent studies.

Pseudoaneurysm at the anastomosis of the free flap following ablative head and neck surgery is uncommon. Fracture of the radial neck are uncommon injuries. In children, they may present as radial neck fractures, a components of forearm fracture dislocations, or as isolated fracture dislocations. Here, we present an unusual and previously undescribed variant of radial neck fracture with dislocation of the radial head to the medial side and ulnar nerve injury. The fracture dislocation was openly reduced and fixed with a small fragment plate. The fracture healed with some loss of rotational movements. At short followup of 6 months patient had useful elbow function but ulnar nerve did not recover.

These days we have fast paced traffic on our roads to help us keep up with our fast paced life. But every boon has a down side and our high velocity traffic is no exception. Here is a case report of a blunt abdominal injury following a road traffic accident. Externally the deceased had only a few grazed abrasions on the forehead and right forearm. But on internal examination of abdomen, it was noticed that the left hemi-diaphragm was torn and the stomach and intestines were found displaced into the left thoracic cavity.

This article highlights 2 important complications of fracture: acute compartment syndrome and fat embolism syndrome (FES). FES is most commonly associated with long-bone and pelvic fracture, whereas acute compartment syndrome is often associated with tibia or forearm fracture. The onset of both of these complications may be difficult to assess in the nonverbal patient or in the patient with multiple trauma. Cryptococcus laurentii is an extremely rare human pathogen. We report a case of primary cutaneous cryptococcosis caused by Cryptococcus laurentii in an immunocompetent patient, an 8-year-old child with a solitary lesion on the forearm. It was impossible to determine the source of infection and no predisposing factors were found. Oral treatment with fluconazole was totally successful.
