PURPOSE: The objectives of this study were to identify the relationship between fracture characteristics and medial collateral ligament (MCL) injuries in terrible triad injuries of the elbow.
METHODS: Between 2010 and 2018, 60 patients who underwent surgery for terrible triad elbow injuries were retrospectively reviewed. Using magnetic resonance imaging (MRI) assessments, patients were stratified into those who had intact or low-grade partial MCL tears (low-grade MRI-MCL group) and those who had high-grade partial or full-thickness MCL tears (high-grade MRI-MCL group). We also analyzed patien

UNLABELLED: Axial carpal injuries are rare entities where a traumatic force transmits through the intermetacarpal space, dissociating the carpometacarpal joint and disrupting the distal carpal row in an axial radial or axial ulnar pattern via true axial force or compressive crush mechanism. Differences in outcomes remain unclear with regard to the specific type of axial carpal injury pattern. A retrospective case series identified 37 wrists (in 37 patients) over 25 years who presented to a Level 1 trauma centre, with 20 wrists experiencing an axial radial injury and 17 wrists experiencing an a

Although elbow injuries and fractures of the forearm are common in children, the combination of these injuries is rare. We present a case of a 5-year-old patient with a concomitant ipsilateral supracondylar humerus fracture and Monteggia lesion. After physical and radiographic examination of the injured extremity in the emergency department, closed reduction and percutaneous pinning were performed under image intensifier under general anaesthesia. A long-arm cast was applied for postoperative immobilisation. Excellent radiological and functional outcomes were obtained at the end of 1-year foll

A 4.5-kg, adult bald eagle (Haliaeetus leucocephalus) was presented after being found unable to fly. Crepitus was palpated in the left shoulder; however, the wing position was normal. Radiographs revealed caudoventral luxation of the left coracoid, affecting its distal articulation with the clavicle and its proximal articulation with the sternum. The luxation affecting the coracoid-sternum articulation was repaired by using a 4-hole, 1.5-mm T-plate and a 6-hole, 2.0-mm dynamic compression plate (DCP) placed side by side. The luxation affecting the coracoid and the clavicle was repaired by 2 ce

INTRODUCTION: To report the first case of traumatic C1-2 lateral subluxation with three-part fractures of the C2 axis (dens, lateral mass, and hangman fractures) that was successfully treated with only anterior dens screw fixation.
METHODS: A 56-year-old man was admitted to the hospital complaining of neck and left arm pains caused by a pedestrian traffic accident. Radiologic examination revealed traumatic C1-2 lateral subluxation, type 3 dens fracture (Anderson and D'Alonzo classification), fracture of both C2 lateral masses, and type 1 hangman fracture (Levine and Edwards classification).

Management of posterior malleolus fractures continues to be controversial, with respect to both need for fixation and fixation methods. Fixation methods include an open posterior approach to the ankle as well as percutaneous reduction and fixation with or without arthroscopy for visualization of the articular surface. Plain radiographs are unreliable in identifying fracture pattern and intraoperative reduction, making arthroscopy a valuable adjunct to posterior malleolus fracture management. In this article, we report a case of tibialis posterior tendon entrapment within a posterior malleolus

The objective of this study was to compare the long-term histological and behavioral outcomes after spinal cord injury (SCI) induced by one of three distinct biomechanical mechanisms: dislocation, contusion, and distraction. Thirty male Sprague-Dawley rats were randomized to incur a traumatic cervical SCI by one of these three clinically relevant mechanisms. The injured cervical spines were surgically stabilized, and motor function was assessed for the following 8 weeks. The spinal cords were then harvested for histologic analysis. Quantification of white matter sparing using Luxol fast blue s

In the realm of orthopaedics, the terrible triad of the elbow is infamous, not simply because the prognosis is poor for most patients, but also, maybe to a greater extent, because the unique name of this malady attracts considerable attention and interest in both doctors and patients. The adjective terrible is bestowed on an elbow triad that comprises three coexisting complicated traumas; namely, radial head and ulnar coronoid process fractures and posterior dislocation of the elbow joint. In this review, the classification, treatment principles and prognosis for different forms of management

Although the incidence of hip fractures is decreasing, the overall prevalence continues to increase because of an aging population. People older than 65 suffer fractures at a rate of 0.6% per year--2% per year for persons older than 85. One in 5 patients suffering a hip fracture will die within a year. Additionally, the emergency physician must consider entities such as avascular necrosis, compartment syndrome, and muscular disruption. This article reviews patterns and complications of acute hip and thigh injuries and clinically relevant diagnostic, anesthetic, and treatment options that facil

Malunion after a malleolar fracture can include a displaced posterior malleolus with associated posterolateral subluxation of the talus. Corrective osteotomy including the posterior malleolus was performed in four patients. Joint congruity was obtained in every case. The patients were followed for 46 to 80 months postoperatively. They all experienced an improvement in pain and three of four patients were unlimited in their walking capacity. Mild to moderate residual symptoms were frequent. The symptoms were attributed to the damage of the cartilage and soft-tissues both from the initial injury

Secondary ulnar nerve palsy, an unusual condition in which the onset of ulnar nerve dysfunction occurs 1 to 3 months after elbow trauma, can be the cause of sudden deterioration of elbow function. Initially recognized in 1899, this condition has not been reported often. We describe 2 patients who had no subjective or objective evidence of ulnar nerve dysfunction after elbow trauma but had a sudden loss of motion, pain, and clinical and electrophysiologic evidence of ulnar nerve compression at the elbow 4 to 5 weeks after trauma. Marked improvement occurred after ulnar nerve subcutaneous transp

A unilateral facet dislocation is an injury classically localized to the cervical spine. A series of 3 unilateral thoracic facet dislocations are reported in 2 adults and 1 child. Each had severe concomitant injuries to the thorax, which caused respiratory compromise. On lateral radiographs, all had anterior vertebral body translation of less than 20% of the vertebral body width with minor degrees of kyphosis. Axial computed tomographic images showed a unilateral empty facet sign. All patients were neurologically intact and none had any mechanical pain at last followup. The mechanism of injury

More understanding of the biomechanics of injured segments, standardized dorsal and ventral accesses to all spinal column levels, and developments in fixation systems specifically conceived for the stabilization of vertebral column lesions have all greatly expanded the operative indications for spinal column injuries. Modern fixation systems should have the following features: short-term application, angular stability, universal suitability for all types of fractures, ability to bear axial loads, thus dispensing with the need for additional external stabilization.

These injuries are rare. Additional soft tissue lesions and multiple traumatic disorders of the foot are common, due to crush and traffic injuries. Surgery is needed for the reconstruction of the arch of the foot, its joints and soft tissue. The traumatic lesion of the soft tissue requires meticulous debridement, an exact reduction and osteosynthesis and primary wound closely by means of a synthetic material like Epigard. Providing reduction is adequate, it should be stabilised with percutaneous Kirschner wires.

In proximal humeral fractures with first degree displacement and in fractures with second degree displacement, which primarily prove to be stable, reduction is not necessary. Closed reduction is indicated in unstable fractures with second degree displacement and in all fractures with third degree displacement. If closed reduction fails to improve the position of the fracture, open reduction and Kirschnerwire transfixation should be performed. Retention for two to three weeks is sufficient.
