Dependence of premature ventricular complexes on heart rate-it's not that simple.

OBJECTIVE: Frequent premature ventricular complexes (PVCs) can lead to adverse health conditions such as cardiomyopathy. The linear correlation between PVC frequency and heart rate (as positive, negative, or neutral) on a 24-hour Holter recording has been proposed as a way to classify patients and guide treatment with beta-blockers. Our objective was to evaluate the robustness of this classification to measurement methodology, different 24-hour periods, and nonlinear dependencies of PVCs on heart rate. MATERIALS AND METHODS: We analyzed 82 multi-day Holter recordings (1-7 days) collected from

MADE: leveraging informative missingness and temporal evaluation for electronic health record-based deterioration prediction in general wards.

OBJECTIVE: Electronic health record (EHR) time series are irregularly sampled and frequently incomplete, with missingness potentially carrying clinically informative signals. We developed Masked-Attention-based timestamp-wise Data Embedding (MADE), a missingness-aware model for continuous prediction of deterioration in general wards, and evaluated it across heterogeneous hospitals. MATERIALS AND METHODS: Observed variables were tokenized into feature-specific embeddings and processed by a masked-attention Transformer, followed by bi-LSTM temporal aggregation for risk prediction. MADE was eval

Malfunctions in distributed clinical decision support: 3 cases from a multi‑component clinical decision support system.

INTRODUCTION: Clinical decision support (CDS) is increasingly delivered through distributed architectures that connect electronic health record (EHR) workflows to external services and community partners. While these designs enable rapid deployment and interoperability, they also introduce new failure modes that can silently disrupt time‑sensitive care processes. Practical, real‑world descriptions of these distributed CDS malfunctions remain limited. CASE REPORT: We describe 3 malfunctions in a distributed CDS system used to support smoking cessation referrals. The issues reflected common dis

Reporting follow-up budgets for diagnostic and predictive AI.

OBJECTIVE: To argue that diagnostic and predictive AI should be evaluated by both classification performance and the downstream work their outputs create. DISCUSSION: Prior work has framed AI as an intervention and has quantified alert burden or decision utility under resource constraints. This perspective extends that logic beyond surveillance alerts by proposing follow-up budget reporting for diagnostic and predictive AI. Across lung nodule detection, sepsis alerting, and autonomous diabetic retinopathy screening, similar classification performance can imply very different demands on imagin

Curation and Extraction of Drug-Related Entities from Reddit Platform.

Physicians learn primarily about illicit drugs from clinical overdose cases, limiting their understanding of real-world usage. Meanwhile, drug users share first-hand experiences online, offering insights into dosage and effects of drugs. To bridge this gap, we introduce ReDose (REddit Drug DOSe and Effect), a dataset of 6,435 Reddit posts on substance use. A board-certified toxicologist primarily annotated both the training and test sets, while two medical science students contributed to the test set, labeling DRUG, DOSE, and EFFECT entities. We benchmarked 6,267 annotations using BERT-based,
