A total of 202 delivering mothers and their newborns were studied with the intention to follow the materno-fetal transmission of malaria. Malaria infection was encountered in 35 cases (17.3%) in which Plasmodium falciparum predominated in peripheral blood while P. malariae predominated on the placental surface. This comprehensive review summarizes current ergonomics recommendations and provides an overview of current potential ergonomic problems and proposed interventions, with special attention to minimally invasive surgeons. While the field of surgical ergonomics has gained momentum in the past couple years, there is still much research and development to be done. Attending diligently to the nutrition and hydration needs of patients with short bowel syndrome (SBS) is a key tenet of their care, both postoperatively and in the years that follow. For, without each, patients are left to themselves to navigate the nutrition consequences of SBS, including malnutrition, nutrient deficiencies, renal compromise, osteoporosis, fatigue, depression, and impaired quality of life. The intent of this review is to discuss the initial nutrition assessment, oral diet, hydration, and home nutrition support for the patient with SBS.

Zika virus is a re-emerging flavivirus transmitted primarily by arthropod vectors. The recent devastating outbreak of Zika virus in Brazil was preceded by the slow global encroachment of this virus over many decades. To date, significant research efforts are underway to understand the spread and the unique pathogenesis of this virus; with the intent to rapidly develop vaccines and therapeutics. Several model systems have emerged to study Zika. This review will focus on the use of nonhuman primates to model Zika infection.

The exclusive respiratory involvement by Kaposi sarcoma is rare, especially in the upper airway. When present, it could lead to severe airway compromise. We present the case of a human immunodeficiency virus infection-positive male patient who presents with dyspnea and stridor, and was found to have bilateral vocal cord paralysis and stenosis of the upper airway due to laryngeal and tracheal involvement by Kaposi sarcoma. The patient underwent bronchoscopy for both diagnostic and therapeutic intentions.

Although child abuse is an age-old problem, it was not configured as a medical problem in the United States until the 1960s. Prior to that time, physician involvement in child abuse cases was limited. In the 1960s doctors began to medicalize child abuse by reporting on constellations of symptoms and radiographic findings that resulted from intentional trauma. Many social, political, and professional forces combined to make physicians more interested in playing a leadership role in identifying and treating abused children.

We report the fifth case of neoplastic involvement of the infrahepatic portion of the inferior vena cava, observed in a young woman affected by a left adrenal cortical carcinoma. The intracaval extension of a neoplastic thrombus from an adrenal tumor is a rare complication that should not be considered a dismal prognostic factor and does not contraindicate tumor resection with curative intent. Caval invasion must be preoperatively investigated to plan the correct surgical access and avoid the risk of neoplastic embolization.

A 35-year-old female alcohol addicted ingested with suicidal intentions a single dose of 500 mg thioridazine. The course of intoxication was complicated by the development of polymorphous ventricular tachycardia of the torsade de pointes type, about six hours after ingestion. The dysrhythmia was immediately suppressed by stimulation from the right atrium. After 36 hours the stimulation could be abolished. Because of delirium tremens the patient with a normal ECG tracing was returned to the psychiatric clinic.

This chapter has dealt with some of the general issues that impact upon postoperative care. Within the context of this volume, a detailed analysis of each subject is not feasible. Some subjects were deliberately deferred to other contributors. My intent was to review some basic physiology and patho-physiology to emphasize the complex physical and metabolic alterations produced by abdominal pelvic surgery and to emphasize my personal belief that an uncomplicated postoperative recuperation begins with appropriate presurgical care.

This is a report on an intentional, direct, intravaginal and transdouglacal genital lesion during a fit of jealousy on the part of the alcoholized partner, caused postcoitally by means of a glass bottle of brandy of 0.35 l capacity. The bottle remained in the peritoneal cavity for 23 hours. After laparotomy had been performed, it was necessary to extirpate the uterus and its left adnexae. Wound healing proceeded without complications. No lesions or functional disorders of the pelvic organs were seen subsequently.
