Comparison of clinical characteristics and severity of COVID-19 with or without viral co-infection in hospitalized children.

BACKGROUND: Co-infection with other pathogens can alter the severity and clinical outcomes of viral infections. However, the information regarding viral co-infections in pediatric coronavirus disease 2019 (COVID-19) cases is still limited. METHODS: This is a nationwide, retrospective cohort study using the data from the COVID-19 Registry Japan. The pediatric (<18 years), laboratory confirmed, hospitalized COVID-19 patients in the Omicron variant of concern predominant period (January 2022 to January 2024) were included. Co-infection was investigated by multiplex PCR. We compared clinical char

Procalcitonin levels in bloodstream infections caused by different sources and species of bacteria.

OBJECTIVE: The aim of this study was to evaluate procalcitonin (PCT) diagnostic accuracy in discriminating gram-negative (GN) from gram-positive (GP) bloodstream infections and determining the relationship between PCT levels, infection sites, and pathogen types. METHODS: Clinical and laboratory data were collected from patients with blood culture (BC)-positive sepsis between January 2014 and December 2015. PCT levels at different infection sites were compared, as was the presence of GN and GP bloodstream infection. A receiver operating characteristic (ROC) curve was generated to assess diagno

Hepatitis E virus coinfection in patients with HIV infection.

OBJECTIVES: Hepatitis E virus (HEV) infection is an emerging infection in developed countries and is thought to be a porcine zoonosis. HEV can cause chronic infection and cirrhosis in the immunosuppressed, including patients with HIV infection. Little is known about HEV and HIV coinfection. The aim of the study was to document the incidence of chronic HEV coinfection in patients with HIV infection and to determine the anti-HEV seroprevalence and compare it with that of a control population. METHODS: A cohort/case-control study was carried out in two teaching hospitals in southwest England. A

Screening of early predictive serum biomarkers and construction of a combined predictive model for refractory Mycoplasma pneumoniae infection in children.

BACKGROUND: Mycoplasma pneumoniae pneumonia (MPP) is a prevalent respiratory infection. Refractory MPP (RMPP) presents more severe symptoms and requires more intensive treatment compared to general MPP (GMPP). This study aimed to identify distinguishing clinical, laboratory, and radiological characteristics between RMPP and GMPP and develop an early predictive model for RMPP risk stratification. METHODS: A total of 568 patients, including 130 RMPP cases and 438 GMPP cases, were enrolled. Clinical information, laboratory tests, and radiological features were compared. Univariate and multivaria

Diagnostic stewardship as the heart of clinical microbiology: a position paper of the French Society for Microbiology (SFM).

Caring about the clinical impact of laboratory results, beyond their technical performance, has been part of the clinical microbiologist's mission since the very origins of the discipline. The term diagnostic stewardship is more recent, emerging from a renewed need created by powerful new diagnostic technologies: as testing becomes faster, more sensitive and more widely used, results grow harder to interpret and easier to misuse. A result reported without interpretation is not neutral, and diagnostic misuse readily translates into inappropriate antibiotic use, making the laboratory an unrecogn

Comparison and Recommendations for Use of Dientamoeba fragilis Real-Time PCR Assays.

Dientamoeba fragilis is a gastrointestinal trichomonad parasite whose pathogenicity is yet to be determined. The difficulty involved in microscopically diagnosing D. fragilis in feces led to the development of real-time PCR methodologies for the detection of D. fragilis in stool samples. Prevalence studies in Europe show much higher levels of infection where a laboratory-developed real-time assay is the predominant assay for the detection of Dientamoeba fragilis than in regions that use the EasyScreen assay for detection of gastrointestinal pathogens. The aim of this study was to compare a com

The dengue and dengue hemorrhagic fever epidemic in Puerto Rico, 1994-1995.

From June 1, 1994 to May 31, 1995 a total of 24,700 cases of dengue (7.01/1,000 population) were reported to the laboratory-based surveillance system in Puerto Rico (1991-1994, annual average: 2.55/1,000). Dengue virus 2 predominated. The earliest indicator of epidemic activity was the virus isolation rate in May 1994 (14.0% versus 5.7% average). The male-to-female ratio among cases was 1:1.1; 65.4% were younger than 30 years (the 10 to 19 year age group had the highest incidence, 11.8/1,000). At least 5,687 cases (23.0%) showed a hemorrhagic manifestation; 4,662 (18.9%) were hospitalized, and

Damage to the host cadaver, simulating the effects of scavenging, differentially affects fitness of entomopathogenic nematode species.

Insect cadavers infected by entomopathogenic nematodes (EPN) are defended against scavengers by chemical mechanisms and other means. Despite these defences, the cadaver may be bitten before being rejected. In this study, we investigated the effect of damage to the cadaver cuticle on the fitness of nematodes (Heterorhabditis downesi Stock, Griffin & Burnell or Steinernema feltiae Filipjev) developing inside. We first quantified the severity of scavenger damage to EPN-infected Galleria mellonella Linnaeus cadavers in the field, and separately, with crickets (Gryllus bimaculatus De Geer) in the l

Oral fluid: Non-invasive alternative for parvovirus B19 diagnosis?

BACKGROUND: Infections with parvovirus B19 (B19V) have been associated with a wide range of disease manifestations of which erythema infectiosum (fifth disease) in children is most common. Clinical signs following infection of children with B19V can be similar to measles and rubella. Laboratory detection of B19V infections is based on detection of B19V-specific IgM antibodies by enzyme immunoassay (IgM-EIA) and/or B19V DNA by quantitative PCR (qPCR) on blood samples. The need for invasive sampling can be a barrier for public health diagnostics. OBJECTIVES: To evaluate the use of a dual target

Management of children with prolonged fever of unknown origin and difficulties in the management of fever of unknown origin in children in developing countries.

This is Part II of a 2-part paper on fever of unknown origin (FUO) in children. It examines the aetiology and management of prolonged FUO in children and the difficulties in the management of FUO in children in developing countries. Part I of this paper discussed acute FUO in children and was published in the March 2001 issue of Paediatric Drugs. Prolonged FUO is documented fever of more than 7 to 10 days which has no apparent source and no apparent diagnosis after 1 week of clinical investigations. About 34% of cases of prolonged FUO are caused by infections, with bacterial meningitis and uri

Influenza A: Associated Severe Cholestatic Liver Injury in a Patient With Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD).

Influenza A infection typically causes mild hepatocellular liver enzyme elevations, while clinically significant cholestatic liver injury is rare. We report a case of severe intrahepatic cholestasis associated with Influenza A infection in a patient with pre-existing metabolic dysfunction-associated steatotic liver disease (MASLD). A 65-year-old female with MASLD and chronic prosthetic hip infection presented with confusion, hypoglycemia, and sepsis. One month prior, liver function tests were normal. On admission, laboratory evaluation demonstrated a cholestatic pattern with total bilirubin 3.

A Silent Virus, a Dangerous Clot: A Case Report of Cytomegalovirus (CMV)-Associated Splanchnic Vein Thrombosis in an Immunocompetent Adult.

Cytomegalovirus (CMV) infection typically presents as an asymptomatic or mild illness in immunocompetent hosts. However, thrombotic complications, though rare, have been increasingly recognized. We report a case of acute superior mesenteric vein (SMV) and portal vein thrombosis with splenic infarction in a previously healthy young adult with primary CMV infection. A 28-year-old male with no significant past medical history presented with acute pain in the left upper quadrant (LUQ) of the abdomen, fever, and diarrhea. Acute confusional states and functional recovery after West Nile virus infection in hospitalized older adults.

The functional consequences of symptomatic West Nile virus (WNV) infection in hospitalized older adults remain poorly characterized. We aimed to describe the clinical presentation, acute confusional states, and six-month functional recovery trajectories of older adults hospitalized with symptomatic WNV infection. We conducted a retrospective case series of consecutive patients aged ≥65 years with laboratory-confirmed WNV infection admitted to a secondary-care hospital in Sardinia, Italy. Functional status was assessed using the Barthel Index at admission, discharge, and six-month follow-up. Se
