Attenuating effects of vitamin C on lead (Pb)-induced toxicological and endocrine responses in male Wistar rats.

The aim of this study was to investigate the attenuating effect of vitamin C (VC) on lead (Pb)-induced toxicity in male Wistar rats. Twenty-four mature male rats were divided into 4 groups consisting of 6 animals using a completely randomized design. Rats were treated orally with either 60 mg/kg body weight (BW) Pb acetate or 100 mg/kg BW VC singly or combination. Controls group received water (vehicle control). After 65 days exposure, testes and epididymis of the animals were measured for testicular weight, epididymal weight and sperm estimations, while blood and liver samples were collected …

Temporary Transvenous Pacemaker Lead-Induced Cardiac Perforation Incidentally Detected by Right Ventriculography Prior to Leadless Pacemaker Implantation.

Temporary transvenous pacemakers (TTPs) are lifesaving device in patients with hemodynamically unstable bradycardia, but are associated with serious complications, including right ventricular (RV) perforation. We present a case of a 91-year-old woman with complete atrioventricular block and bradycardia who received a TTP. After that, she underwent leadless pacemaker implantation. Preprocedural right ventriculography (RVG) revealed an incidental RV wall perforation by the TTP lead. After preparing for pericardiocentesis, the lead was safely extracted without complications. This case highlights …

Deep Brain Stimulation Peri Lead Cyst Resolution with Nonoperative Management: Case Report and Review of the Literature.

Deep brain stimulation (DBS) is an effective treatment for medically refractory essential tremor (ET) and Parkinson's disease. We present the case of a patient treated with thalamic DBS for ET who developed a large right peri-lead cyst with associated edema three months postoperatively, causing symptoms of dysarthria and left-sided weakness. The patient was treated nonoperatively with a course of steroids and serial observation. Over three months, the cyst regressed in size, with resolution of the associated symptoms. The patient was ultimately able to continue bilateral DBS for ET, which prov…

Less haste, more speed: Does delayed blood culture transport time lead to adverse incubation times or yield?

BACKGROUND: Blood culture remains a vital diagnostic tool in the acutely unwell patient. The UK Standards for Microbiology Investigations (SMI) stipulate pre-analytical requirements that are aimed at increasing yield and reducing turnaround time. The requirement to load blood cultures on machines within 4 h has been shown to reduce turnaround time but limited evidence exists as to whether it improves yield. METHODS: We extracted blood culture results, including organism growth, time to detection, location and time of sample collection from 4 hospitals in Southwest England. We then used mixed

Two missense mutations in Dystonin lead to epidermolysis bullosa simplex complicated with lepromatous leprosy: a case report.

BACKGROUND: Epidermolysis bullosa (EB) is a severe, single-gene hereditary skin condition characterized by blisters, bullae, and erosion following minor trauma. Epidermolysis bullosa simplex (EBS), one of the four major types of EB, is typically caused by mutations in the KRT5 or KRT14 genes. However, mutations in the Dystonin (DST) gene, which is also involved in maintaining skin integrity, can lead to EBS. This study aims to report a unique case of EBS caused by two novel heterozygous mutations in the DST gene, complicated by lepromatous leprosy (LL), a chronic infectious disease caused by M…

Origin of Dopant-Carrier Exchange Coupling and Excitonic Zeeman Splitting in Mn2+-Doped Lead Halide Perovskite Nanocrystals.

Low-dimensional metal halide perovskites have unique optical and electrical properties that render them attractive for the design of diluted magnetic semiconductors. However, the nature of dopant-exciton exchange interactions that result in spin-polarization of host-lattice charge carriers as a basis for spintronics remains unexplored. Here, we investigate Mn2+-doped CsPbCl3 nanocrystals using magnetic circular dichroism spectroscopy and show that Mn2+ dopants induce excitonic Zeeman splitting which is strongly dependent on the nature of the band-edge structure. We demonstrate that the largest

Novel plasma cytokines identified and validated in children during lead exposure according to the new updated BLRV.

Lead is a pervasive environmental contaminant with significant health risks, particularly to children. It is known for its neurotoxic and immunotoxic effects, causing developmental, cognitive, and behavioral impairments. Despite extensive research, the mechanisms of lead toxicity remain unclear. Cytokines, which are critical in immune response and inflammation, have emerged as potential biomarkers for lead toxicity. The recent Centers for Disease Control and Prevention (CDC) update to the blood lead reference value (BLRV) to 3.5 µg/dL emphasizes the need to explore novel biomarkers and mechani…

Pharmacokinetics and safety of LEAD-452, an EGFR-specific 4-1BB-agonistic trimerbody in non-human primates.

LEAD-452 is a humanized bispecific EGFR-targeted 4-1BB-agonistic trimerbody with a unique trimeric configuration compared to other 4-1BB-specific antibodies that are currently in development. Indeed, enhanced tumor-specific costimulation and very remarkable safety and efficacy profiles have been observed in mouse models. Here, we conducted for the first time a preclinical pharmacokinetic and toxicity study in non-human primates (NHP) (Macaca fascicularis). LEAD-452 exhibits comparable binding affinity for human and macaque targets, indicating its pharmacological significance for safety testing…

Impact of infective versus sterile transvenous lead removal on 30-day outcomes in cardiac implantable electronic devices.

BACKGROUND: Transvenous lead removal (TLR) is associated with increased mortality and morbidity. This study sought to evaluate the impact of TLR on in-hospital mortality and outcomes in patients with and without CIED infection. METHODS: From January 1, 2017, to December 31, 2020, we utilized the nationally representative, all-payer, Nationwide Readmissions Database to assess patients who underwent TLR. We categorized TLR as indicated for infection, if the patient had a diagnosis of bacteremia, sepsis, or endocarditis during the initial admission. Conversely, if none of these conditions were p…

Amyloid aggregates induced by the p53-R280T mutation lead to loss of p53 function in nasopharyngeal carcinoma.

Nasopharyngeal carcinoma (NPC) is a malignant tumor that is highly prevalent in Southeast Asia, especially in South China. The pathogenesis of NPC is complex, and genetic alterations of tumor suppressors and proto-oncogenes play important roles in NPC carcinogenesis. p53 is unexpectedly highly expressed in NPC and possesses an uncommon mutation of R280T, which is different from a high frequency of hotspot mutations or low expression in other tumors. However, the mechanism of p53 loss of function and its correlation with R280T in NPC are still unclear. In this study, p53 amyloid aggregates were

Four-Stage Rocket technique: A novel strategy for lead extractions using laser sheaths from the femoral vein.

INTRODUCTION: Transvenous lead extractions (TLEs) for cardiac implantable electronic device complications often encounter difficulties with strong adhesions to the myocardium or vessels. In this report, we introduce a novel "Four-Stage Rocket" technique for effective TLE in cases where conventional methods fail. METHODS AND RESULTS: Two challenging cases where conventional TLE methods failed were treated using a combination of four devices: Needle's Eye Snare, Agilis NxT Steerable Introducer, GlideLight Laser sheath, and GORE® DrySeal Flex Introducer sheath, employed via the inferior vena cav…

How Chicago's Past Resulted in Disproportionate Lead Poisoning of Minority Children of the Present: A Narrative Review.

Chicago's lead problem has been shown to disproportionately affect populations of color and lower socioeconomic status (SES). The disproportionate effects on low-income areas and communities of color can be traced back to several key decisions in Chicago's history. A search of the National Library of Medicine's MEDLINE/PubMed as well as Google, and Google Scholar was performed to find all articles relating to lead poisoning in Chicago, lead utilization, Chicago's municipal and political history, and lead physiology between May 2020 and May 2023. Additionally, several studies and textbooks were…

Dynamic risk stratification of worsening heart failure using a deep learning-enabled implanted ambulatory single-lead electrocardiogram.

AIMS: Implantable loop recorders (ILRs) provide continuous single-lead ambulatory electrocardiogram (aECG) monitoring. Whether these aECGs could be used to identify worsening heart failure (HF) is unknown. METHODS AND RESULTS: We linked ILR aECG from Medtronic device database to the left ventricular ejection fraction (LVEF) measurements in Optum® de-identified electronic health record dataset. We trained an artificial intelligence (AI) algorithm [aECG-convolutional neural network (CNN)] on a dataset of 35 741 aECGs from 2247 patients to identify LVEF ≤ 40% and assessed its performance using t…

Gender-Specific Effects on the Cardiorespiratory System and Neurotoxicity of Intermittent and Permanent Low-Level Lead Exposures.

Lead exposure is a significant health concern, ranking among the top 10 most harmful substances for humans. There are no safe levels of lead exposure, and it affects multiple body systems, especially the cardiovascular and neurological systems, leading to problems such as hypertension, heart disease, cognitive deficits, and developmental delays, particularly in children. Gender differences are a crucial factor, with women's reproductive systems being especially vulnerable, resulting in fertility issues, pregnancy complications, miscarriages, and premature births. The globalization of lead expo…

Delayed Right Ventricular Automatic Implantable Cardioverter Defibrillator (AICD) Lead Perforation Resulting in Cardiac Tamponade: A Case Study.

Automatic implantable cardioverter defibrillator (AICD) lead perforation is a rare but potentially life-threatening complication. AICD lead perforations are rare, occurring in approximately 0.1%-0.8% of patients, most commonly within 24 hours of the implantation. ICD lead perforations can be acute (within 24 hours of implantation), subacute (between day 1 and day 30), or delayed (>30 days postimplantation). Delayed lead perforations are rare compared to acute and subacute lead perforations and are not as well-studied because patients are often asymptomatic and are not diagnosed. Here, we repor…

Rescue Lead Implantation After Deep Brain Stimulation for Parkinson's Disease: A Single-Center Experience and Case Series.

BACKGROUND AND OBJECTIVES: Despite the well-established efficacy of deep brain stimulation (DBS) of the subthalamic nucleus (STN) for Parkinson's Disease (PD), there remains a subset of patients with only a moderate improvement in symptoms even with appropriate lead placement and optimal programming. In patients with persistent tremor or dyskinesias, one consideration is the addition of a second "rescue lead" to provide dual stimulation to primary and secondary targets to address the refractory component. This study aimed to assess all "rescue lead" cases from our institution and characterize …

Exposure risks of lead and other metals to humans: A consideration of specific size fraction and methodology.

Particle size is a critical influencing factor in assessing human exposure risk as fine particles are generally more hazardous than larger coarse particles. However, how particle composition influences human health risk is only poorly understood as different studies have different utilised different definitions and as a consequence there is no consensus. Here, with a new methodology taking insights of each size fraction load (%GSFload), metal bioaccessibility, we classify which specific particle size can reliably estimate the human exposure risk of lead and other metals. We then validate these…
