[Chronic disease and sexuality. An interview study on sexual dysfunction in patients with multiple sclerosis].

Many patients with a chronic disease experience that this condition in some way influences their sexual life. In this paper we present the results from an interview survey of ten patients with multiple sclerosis. The aim of the survey was to assess sexual dysfunctions in patients with chronic disease, the patients' opinion about these matters, and the role of the health services. We found the extent of the sexual problems of these patients to be comparable with the findings presented in earlier studies. The majority of patients considered it a problem that the topic was never raised by their m…

Contrast sensitivity in multiple sclerosis. A study in 35 patients with and without optic neuritis.

Binocular and monocular contrast sensitivity (CS) functions were determined for 35 patients with multiple sclerosis (MS) of whom only 21 had a history of optic neuritis. CS was abnormal in all 21 of these patients; alterations of CS affected all spatial frequencies, not only for the eye with optic neuritis, but also for the clinically unaffected eye (p less than 0.01). Approximately one third of the 14 patients with MS but without any history of past visual disturbances showed an optic nerve involvement. Our results show that (1) clinically visual impairment is bilateral, even if optic neuriti…

[Retrobulbar optic neuritis as an initial symptom of multiple sclerosis. Results of a prospective study].

40 patients with idiopathic optic neuritis (ON) between 1st January 1967 and 31st December 1977 have been checked again from oculistic and neurological point of view between 1st January and 31st March 1983. 10 of 40 examined patients (25%) are found suffering from multiple sclerosis (MS) at the time of check up. Our research points out that idiopathic ON evolutive risk towards MS is greater in females, in people between 20 and 40 years old, in people with one or more relapses as well as in people with neurological microsymptoms at the time of ON.

Exacerbation detection in multiple sclerosis by clinical and evoked potential techniques: a preliminary report.

The EP is well demonstrated as a useful indicator of subclinical demyelinating disease. Our study performed on patients well defined as MS and monitored closely for clinical exacerbations demonstrated an unexpectedly high occurrence of significant worsening in EP parameters. These changes occurred in the presence or absence of clinical worsening. The parameter changes may represent new demyelination or other pathologic or neurophysiologic changes in neuropathologically static lesions.

Multiple sclerosis and infants fed on diets deficient in vitamin A or in selenium and vitamin E.

The characteristic features of multiple sclerosis (MS) include indications that its foundations are laid in early childhood as evidence by instances of the clustering of cases in particular localities by place of birth. Then there is the pattern of distribution in which the prevalence of MS among Caucasians progressively diminishes as the equator is approached, and the familial tendency of the disease to occur in close relatives of those who suffer from it. What is perhaps the most intriguing of all is the prevalence in the Orkney and Shetland Islands which is the highest in the world as far a…

Cerebrospinal fluid protein findings in cervical syndromes classified by myelography, and in multiple sclerosis.

Determinations of cerebrospinal fluid (CSF) albumin, IgG, albumin blood brain barrier (BBB) permeability and local IgG synthesis indexes in CNS were carried out on 85 patients with various neck, shoulder and upper extremity pain syndromes. CSF was obtained by lumbar puncture in 29, and by lateral neck puncture in 56 of the patients. The patients were classified into 3 different groups according to varying severity of degenerative changes, or cavitation verified by myelography. CSF protein patterns in these patients were compared with lumbar CSF findings in 18 patients with multiple sclerosis. …

Comparison of two molecular species of ethanolamine plasmalogen in multiple sclerosis and normal myelin.

A TLC procedure which resolves two molecular species of ethanolamine plasmalogen, Pl-PE-1 and Pl-PE-2, was used to compare the ratio of these was species in myelin isolated from normal appearing white matter from brains of 17 multiple sclerosis (MS) patients, 17 normal (N) individuals, 1 patient with subacute sclerosing panencephalitis (SSPE) and 1 patient with a non-demyelinating neurological disease (OND). One of these species (Pl-PE-2) has been reported to be unique to myelin and has primarily 18:1 in both the 1 and 2 positions of glycerol. The other species (Pl-PE-1) is also present in oth…

[Bilateral cysts of the brain, an unusual manifestation of multiple sclerosis in childhood (author's transl)].

In a 14 year old boy symptoms of an acute demyelinating disease (Multiple Sclerosis) were characterized by the history of retrobulbar neuritis, signs of increased intracranial pressure, asymmetric bilateral hemiparesis and generalized convulsion. Computerized tomograms and neuropathologic examinations revealed large bilateral cystic lesions one of which had to be operated. Clinical and pathological relations of this unique acute myelinoclastic form of severe demyelination are shown and discussed.

Multiple sclerosis: presence of lymphatic capillaries and lymphoid tissue in the brain and spinal cord.

Thin-walled channels resembling lymphatic capillaries and containing lymphocytes were observed in perivascular spaces throughout the central nervous system of patients with various neurological disorders. This suggests that immunological surveillance in the central nervous system may normally involve lymphocyte circulation through the perivascular compartment. In some old multiple sclerosis plaques, perivascular lymphoid tissue was found which was organized in a manner similar to the antibody-producing medullary region of lymph nodes. This may indicate continuous processing of the putative mul…

Sclerosing spinal pachymeningitis. A complication of intrathecal administration of Depo-Medrol for multiple sclerosis.

Reported complications of intrathecal steroid therapy include aseptic meningitis, infectious meningitis, and arachnoiditis. We report a case of sclerosing spinal pachymeningitis complicating the attempted intrathecal administration of Depo-Medrol for multiple sclerosis. The lesion is characterised by concentric laminar proliferation of neomembranes within the subdural space of the entire spinal cord and cauda equina, resulting from repeated episodes of injury and repair to the spinal dura mater by Depo-Medrol. There is clinical and laboratory evidence that Depo-Medrol produces meningeal irrita…

[Topodiagnostic ERA results at a female patient with multiple sclerosis in the brainstem (author's transl)].

At a female patient suffering from multiple sclerosis with demyelinisation mainly in the pontine and mesencephalic tegmentum, the five early potentials consisting of the compound action potential of the acoustic nerve (Pot. I) and the four brainstem potentials (Pot. II-V) as well as the slow evoked vertex-potential were registered. The threshold behaviour, the characteristic curve and the latency curve of all potentials were obtained, and thereby damages of the central acoustic pathways found out. A topodiagnostic is tried by means of the alterations of the peripheral and central ERA, and by m…

Use of the Pulfrich pendulum for detecting abnormal delay in the visual pathway in multiple sclerosis.

(1) Pulfrich's pendulum has been developed as a method for detecting abnormal delay in the visual pathway of patients with suspected multiple sclerosis. (2) Forty-one normal subjects gave a mean interocular latency difference of 2-4 +/- 1-8 msec on Pulfrich's pendulum. This mean +2 SD (6 msec) was taken as the limit of normal interocular latency difference by this method. (3) The test was assessed on 58 patients. Eighteen of these had definite multiple sclerosis, 9 had probable multiple sclerosis, and 8 had possible multiple sclerosis, according to clinical criteria. The remaining 23 patients …

The use of intrathecal phenol for muscle spasms in multiple sclerosis. A description of two cases.

Two cases of multiple sclerosis are described, in both of whom the disease started in yound adult life. This disability gradually progressed to the stage of paraplegia-in-flexion in which the lower limbs were fixed in adduction-and-flexion. Both patients developed painful muscle spasms which made life intolerable. These patients were treated by intrathecal phenol in glycerine in an effort to convert this spastic paralysis into a flaccid paralysis. The three advantages sought were: 1. To relieve the muscle spasms so that the patient could sit in a wheelchair and propel herself. 2. To relieve th…

Movements that matter: A photo-elicitation exploration into the walking experience of persons with multiple sclerosis.

BACKGROUND: Walking impairment in multiple sclerosis (MS) can limit participation in community life. Describing the walking experience of persons with MS will help ensure clinical and research impact of clinical trials, technology design, and policy. However, methods exploring the walking experience are homogenous and rarely co-designed. Here, we aim to use co-designed photo-elicitation methods to describe the walking experiences of persons with MS. METHODS: A community-based photo-elicitation study was conducted. Participants took photos of situations in their home and community where diffic…

Real-world applicability of the 2024 McDonald criteria for multiple sclerosis diagnosis in Portugal.

BACKGROUND: The revised McDonald criteria (2024) introduced new imaging and laboratory biomarkers, allowing earlier and more accurate diagnosis of Multiple Sclerosis (MS). OBJECTIVES: To assess the applicability of the new criteria in Portugal and identify diagnostic tool availability and implementation barriers. METHODS: We conducted a national survey of neurologists from public hospitals involved in MS diagnosis and follow-up. Descriptive statistics and Pearson's chi-square tests identified factors associated with systematic implementation. RESULTS: 24 of 30 centres responded (80.0%). MRI…
