An Unusual Outcome Using the TightRope® System in Syndesmotic Injury Management: A Case Report.

Syndesmotic injuries of the ankle are critical for maintaining joint stability and function, especially during weight-bearing activities. While traditional management involves rigid fixation with trans-syndesmotic screws, limitations such as restricted micromovement, potential screw breakage, and the need for secondary procedures have spurred the adoption of dynamic fixation methods like the TightRope® system (Arthrex, Naples, Florida, US). This report presents a case of a 54-year-old female with complications related to TightRope fixation following open reduction and internal fixation (ORIF)

Subluxation of the hip joint after internal fixation of a trochanteric fracture.

The authors report an unusual case of hip subluxation after internal fixation without associated sepsis. We report one recently treated case in which a 75-year-old female experienced subluxation of her hip joint after open reduction and internal fixation for a trochanteric fracture. In this paper, we describe a case of progressive, spontaneous subluxation of the hip joint over several weeks. Most previously reported cases are associated with cerebral palsy. This entity has not been reported previously. Our patient was treated by hemi-arthroplasty and repair of the disrupted capsule, and achiev

[Old unreduced traumatic patellar dislocation].

The old unreduced traumatic patellar dislocation is very rare and the results of the treatment are not always satisfactory. We report a 11 year-old boy with an unreduced patellar dislocation and a healed ipsilateral femoral fracture. The dislocation was reduced by an operative procedure. A long leg cast was applied for 6 weeks then the knee exercises were started. One year after surgery, he still has satisfactory results. This case suggests that if the dislocation is not of long duration and degenerative changes are absent in the patella and the knee joint, open reduction may be helpful in the

Fractures and dislocations about the hip in head-injured adults.

Twenty-nine of 591 head-injured adults (4.9%) sustained concomitant injuries about the hip. Three hip injuries were undetected during initial hospitalization (10%). Sixteen patients (55%) developed varying amounts of heterotopic ossification about the injured hip. Eleven of 23 hips (41%) treated by nonoperative methods developed heterotopic ossification. Six hips underwent open reduction and five developed heterotopic bone (83%). The severity of injury, especially central acetabular and posterior fracture-dislocations, increased the occurrence and amount of heterotopic bone. Ipsilateral hemipl

Functional outcome of internal fixation for pelvic ring fractures.

OBJECTIVE: Evaluation of the functional outcome after unstable pelvic ring fractures stabilized with internal fixation. METHODS: Between January 1, 1990, and September 1, 1997, 37 patients were treated with internal fixation for unstable pelvic fracture. Demographic data, type of accident, Hospital Trauma Index-Injury Severity Score, and fracture type according to Tile classification were scored. One patient died the day after the accident from neurologic injury. A Short Form-36 health questionnaire and a form regarding functional result after pelvic trauma, adapted from Majeed et al., were r

Extensile Lateral Approach for the Operative Management of a Displaced Intra-Articular Calcaneus Fracture.

Calcaneal fractures account for approximately 1.2% of all fractures and 60% of all tarsal bone fractures. Almost 75% present as displaced, intra-articular fractures. Because of the complex articular and osseous anatomy, the vulnerable soft-tissue envelope, and the technically challenging approach needed for fixation, these fractures are often treated nonoperatively, resulting in poor outcomes. These poor outcomes can include entrapment of the posterior tibial and sural nerves, impingement, dislocation or entrapment of peroneal tendons, a widened heel with a loss of height, hindfoot varus or va

Characteristics and management of teeth in the line of mandibular fractures treated with internal fixation.

BACKGROUND/AIMS: The ideal management of teeth in the line of mandibular fractures is unclear and controversial. The aim of this study was to analyze the characteristics and management of teeth in the line of mandibular fractures treated with open reduction and internal fixation. MATERIAL AND METHODS: Medical records of patients with mandibular fractures in the symphysis, body and angle regions seen between July 2019 and January 2021 were evaluated. Personal data, etiology, location of fractures, characteristics of the teeth involved in the fracture lines and management were collected. The re

Traditional Chinese medicine manipulative reduction combined with percutaneous vertebroplasty for treating type III Kummell's disease: A case report.

BACKGROUND: A patient with type III Kummell's disease had a ruptured posterior cortex of the fractured vertebral body, which caused spinal cord compression. An open surgery was considered the best choice of operation. However, the patient and her family refused open surgery and instead demanded a minimally invasive surgical treatment such as percutaneous vertebroplasty (PVP). After preoperative discussion, we finally adopted the novel therapy of traditional Chinese medicine manipulative reduction (TCMMR) combined with PVP. CASE SUMMARY: A patient with type III Kummell's disease exhibiting bon

Do Splints Play a Role in the Management of Condylar Postfracture Syndrome After Mandibular Angle Fractures?-A Randomized Controlled Clinical Trial.

PURPOSE: Fractures of the mandibular angle can result in condylar postfracture syndrome (CPFS) due to indirect injury to the condylar region. Studies evaluating early intervention for CPFS are lacking. In the present study, we evaluated the efficacy of interocclusal splint therapy for the prevention and early management of CPFS. PATIENTS AND METHODS: A prospective randomized controlled clinical trial was designed and included patients who required open reduction and internal fixation for a unilateral angle fracture. The patients were divided into 2 groups. The study group received interocclus

Are There Racial and Ethnic Disparities in Management and Outcomes of Surgically Treated Distal Radius Fractures?

BACKGROUND: Racial/ethnic disparities have been demonstrated across multiple orthopedic sub-specialties. There is a paucity of literature examining disparities in distal radius fracture (DRF) management. METHODS: Using the National Surgical Quality Improvement Program database, we analyzed 15 559 non-Hispanic (NH) White, NH Black, NH Asian, and Hispanic adults who underwent open reduction and internal fixation for DRF from 2013 to 2019. We evaluated time from hospital admission to surgery and length of stay using Poisson regression. Deep venous thrombosis, pulmonary embolism (PE), and wound c

Surgical Approach to Bone Metastases.

PURPOSE OF REVIEW: The purpose of this review was to examine the recent changes in the surgical treatment of bone metastases and how the treatment paradigm has shifted with the improvement of adjuvant therapies. How surgery fits into the local and systemic treatment was reviewed for bone metastases in different areas. RECENT FINDINGS: The more common use of targeted chemotherapies and focused high-dose radiation have altered the treatment paradigm of bone metastases. Overall changes in the surgical treatment of bone metastases have been driven by an increased multidisciplinary approach to met

Treatment of intra-articular fracture of distal radius fractures with fluoroscopic only or combined with arthroscopic control: A prospective tomodensitometric comparative study of 40 patients.

BACKGROUND: Considering articular distal radius fractures treated with volar plate, we hypothesized that articular radio-carpal displacement was better reduced with arthroscopic control than with only fluoroscopic control. METHODS: Forty patients with similar articular radius fracture (type C according to AO classification) and high functional needs were treated with volar plate fixation. They were divided in two comparative groups: 20 patients in Fluoroscopic group and 20 patients in Arthroscopic group. Pre and postoperative radiographs and tomodensitometric images were analysed by an indepe

[Management of forearm shaft fractures : Challenges and solution approaches].

Forearm shaft fractures are the most common fractures of the upper extremity in young adults. By definition, these fractures are diaphyseal fractures; however, due to the complex functional unity formed by the forearm shaft during motion both bone forearm fractures are treated as intra-articular fractures [1, 3]. This is why the gold standard of treatment in adults is osteosynthesis. The aim of the surgical intervention is the anatomical reduction with exact reconstruction of length, shaft axis and rotation. This particularly concerns Monteggia and Galeazzi fractures where the adjacent joint n

General principles for the clinical use of the LCP.

The basic principles of an internal fixation procedure using a conventional plate and screw system (compression method) are direct, anatomical reduction and stable internal fixation of the fracture. Wide exposure of the bone is usually necessary to gain access to and provide good visibility of the fracture zone to allow reduction and plate fixation to be performed. This procedure requires pre-contouring of the plate to match the anatomy of the bone. The screws are tightened to fix the plate onto the bone, which then compresses the plate onto the bone. The actual stability results from the fric

A Posterior Approach to Open Reduction and Internal Fixation of Displaced Posterior Cruciate Ligament Tibial Osseous Avulsions.

BACKGROUND: The true prevalence of displaced tibial-sided avulsion injuries of the posterior cruciate ligament (PCL; Video 1) is unknown, and the majority of data regarding management has been published in areas other than the Western world, such as China and India (perhaps due to the higher prevalence of two-wheeler motorcycle accidents in these areas). Despite the rarity of data, a better understanding of the approach, fixation techniques, and outcomes is necessary to provide quality patient care. These injuries generally require surgical intervention as nonoperative management leads to comp
