Crouzon Syndrome and Acanthosis Nigricans With Fibrous Dysplasia of the Maxilla: An Unreported Suggested Triad.

The aim of this report is to describe the combination of Crouzon syndrome and acanthosis nigricans with fibrous dysplasia of the maxilla. The diagnosis of fibrous dysplasia was confirmed clinically and pathologically during Le Fort III osteotomy and midface advancement with distraction osteogenesis. Crouzon syndrome with acanthosis nigricans is a known syndrome with an incidence of 1:1,000,000. This is the first report in the literature of Crouzon syndrome and acanthosis nigricans combined with fibrous dysplasia. As all 3 pathologies are related to fibroblasts, they may be different manifestat…

Anterior Fontanelle Wormian Bone/ Fontanellar Bone: A Review of this Rare Anomaly with Case Illustration.

Wormian bones are a relatively rare skeletal anomaly that present as accessory bone(s) within the sutures of the cranium and even more rarely within the fontanelles. It is believed that they arise from the formation of abnormal cranial ossification centers. Although not extensively reported in the literature, this anomaly is thought to be associated with other anatomical anomalies such as osteogenesis imperfecta, rickets, and other bone dysplasias. When located within the fontanelles, the most likely site of occurrence is the posterior fontanelle. This case report describes a rare incidence of…

Wire fixation of internal distractor for cranial vault remodeling.

BACKGROUND: Posterior cranial vault distraction osteogenesis is currently used to enlarge the cranial volume and control the intracranial pressure. This procedure carries the risk of hardware dislocation, and especially in infants, as their skull being thin, the screws may damage the underlying dura. TECHNICAL NOTE: In this paper, the authors describe a simple method to fixate the internal distractor to the skull vault in case of thin calvaria using metallic wires. DISCUSSION: This method allows a strong and tight application of the distractor to the osteotomy margins and reduces the risks o…

Phalangeal Lengthening Techniques for Brachydactily and Posttraumatic Digital Stumps With the Use of a Modified External Mini-Fixator.

Distraction osteogenesis has become the most used method for bone lengthening and deformity correction including the bones of the hand and foot. The principle techniques used for hand bone lengthening in posttraumatic and congenital conditions using available external fixators are based on this method. We present the technical specifications and surgical techniques with the use of a mini-fixator that was specially designed for short tubular bones. The surgical techniques of lengthening and web space skin stock creation for future web-pasty are supplied with the diagrams of their application to…

Review article: oral surgery and TMJ-related papers published in BJOMS in 2008 and 2009.

This review summarises the 81 papers relating to oral surgery and to the temporomandibular joint (TMJ) published in the British Journal of Oral and Maxillofacial Surgery (BJOMS) January 2008-December 2009. They include full length articles on distraction osteogenesis, imaging, extraction techniques, implants, and the management of facial pain and disorders of the TMJ including joint replacement. Many short communications were also published, but an editorial decision was taken in August 2009 to publish most of these in future in the online journal only. As BJOMS encompasses the whole remit of …

The association of connective tissue disorders with cervical artery dissections.

A predisposing weakness of the vessel wall has been assumed in patients with spontaneous cervical artery dissections (sCAD). Skin biopsies from many patients with sCAD show mild connective tissue alterations. However, their assessment depends on an invasive and highly specialized technique. Clinical signs of connective tissue disease are absent in the majority of CAD patients. In this review we document that only very few CAD patients are affected by known inherited connective tissue disorders like Ehlers-Danlos syndrome, Marfan syndrome or Osteogenesis Imperfecta. In a second part of this rev…

Sleep disturbances in 22q11.2 deletion syndrome: a case with obstructive and central sleep apnea.

The 22q11.2 deletion syndrome is characterized by wide phenotypic variability, frequently involving characteristic craniofacial features, cardiac malformations, and learning difficulties. Skeletal anomalies are also common and include an obtuse angle of the cranial base, retrognathia, and cervical spine abnormalities. Despite these anomalies, sleep-disturbed breathing is not reported frequently in patients with 22q11.2 deletion syndrome. We describe a patient with an obstructive sleep disturbance that was successfully treated with a tonsillectomy followed by mandibular distraction osteogenesis…

FT-IR imaging of native and tissue-engineered bone and cartilage.

Fourier transform infrared (FT-IR) imaging and microspectroscopy have been extensively applied to the analyses of tissues in health and disease. Spatially resolved mid-IR data has provided insights into molecular changes that occur in diseases of connective or collagen-based tissues, including, osteoporosis, osteogenesis imperfecta, osteopetrosis and pathologic calcifications. These techniques have also been used to probe chemical changes associated with load, disuse, and micro-damage in bone, and with degradation and repair in cartilage. This review summarizes the applications of FT-IR micros…

Identification of partially disordered peptide intermediates through residue-specific NMR diffusion measurements.

It is technically challenging to detect low-population partially disordered species that are in equilibrium with the folded and unfolded states. Residue-specific translational diffusion experiments measured by pulsed field gradient NMR have been used to detect the presence, and define the conformation, of such equilibrium intermediates. The experiment is demonstrated for equilibrium solutions of related triple helical peptides that model a small region of type I collagen with and without a mutation known to cause osteogenesis imperfecta. The data show that residue-specific diffusion coefficien…

Computer-aided procedures in implantology, distraction and cranio-maxillofacial surgery.

Smaller implants and less invasive techniques for osteotomies using an endoscope assisted approach may facilitate the surgical procedures in cranio-maxillofacial distraction osteogenesis. Although sophisticated distraction devices are routinely used, there are still certain problems which need to be addressed. The preoperative planning, even of complex cases can be facilitated by using modern imaging techniques such as computer assisted surgery (CAS). The placement of the devices and the outline of complex osteotomies can be controlled intra-operatively. CAS allows for postoperative monitoring…

Lethal hypophosphatasia, spur type: case report and fetopathological study.

Lethal hypophosphatasia, spur type: case report and fetopathological study: Hypophosphatasia (HP) is characterised by severe undermineralisation of the skeleton owing to deficiency of tissue nonspecific alkaline phosphatase. Clinically a perinatal, infantile, childhood and adult type is distinguished. Clinical signs in the perinatal type of HP show considerable overlap with other skeletal dysplasias such as osteogenesis imperfecta type IIA and type IIC, and achondrogenesis type IA. If present, "spurs" of the limbs are diagnostic for HP. We present a prenatally diagnosed case of HP and discuss …

Heritable dentin defects: nosology, pathology, and treatment.

Heritable dentin defects have been divided into 2 main categories: dentinogenesis imperfecta (DI) and dentin dysplasia (DD). Recent studies have shown that they share many features in common. Of the connective tissue diseases, only osteogenesis imperfecta (OI) has been linked to these disorders. So far, no definitive relation between the type of OI and the dental involvement can be established. Familial occurrence of DI with OI cannot be comprehensively explained by mutations in type I collagen genes. No information about the gene defects in DD is available. At the ultrastructural level, the o…

Collagen, genes and the skeletal dysplasias on the edge of a new era: a review and update.

This article reviews the newly described biochemical (type I and II collagen) abnormalities and specific gene defects in the skeletal dysplasias. The model of the collagen molecule is described and how collagen is processed from procollagen, where and how abnormalities occur, and the types of abnormalities produced (quantitative and qualitative). The only known type I collagen defects producing skeletal dysplasias--osteogenesis imperfecta, as well as the 'family' of established type II collagen disorders--achondrogenesis type II, hypochondrogenesis and spondyloepiphyseal dysplasia congenita ar…

Pathology of the otic capsule.

The otic capsule is unique in retaining calcified cartilage, known as globuli interossei, throughout life and shows changes consequently, which are peculiar to it. In Paget's disease of bone, the otic capsule appears resistant to involvement and this occurs with extensive disease at a late stage. In contrast, otosclerosis is a new bone formation of unknown cause that is limited to the otic capsule. In osteogenesis imperfecta, the poor formation of collagen leads to abnormally thin bony trabeculae with a poorly formed otic capsule. In osteopetrosis, the otic capsule is greatly expanded by incre…

Effects of electric current on chondrogenesis in vitro.

The effect of direct electric current (5 muamp) on mesenchymal cell differentiation into cartilage was investigated in explants of fetal rat muscle and bone matrix gelatin. In this system, chondrogenesis in vitro corresponds in developmental potential to osteogenesis in vivo. Application of electric current inhibited differentiation of muscle derived mesenchymal cells, but enhanced proliferation of differentiated chondrocytes and formation of cartilage at the cathode region, and had no effect on mature hypertrophic chondrocytes. Application of electric current may be contraindicated in fractur…

Application of a lateral compression clamp in the management of mandibular fractures.

A lateral compression clamp system has been used in the treatment of fractures of the mandible. It provides rigid fixation and promotes earlier osteogenesis at the fracture site. Intermaxillary fixation can often be eliminated in edentulous persons and greatly reduced in duration when necessary as an adjunct to fixation in patients with teeth. These significant qualities make the compression clamp quite beneficial in the treatment plan and postoperative management of many otherwise difficult cases. Having evaluated our experiences with compression clamps, we believe that this approach deserves…

Lack of TRIC-B dysregulates cytoskeleton assembly, trapping β-catenin at osteoblast adhesion sites.

The trimeric intracellular cation channel B (TRIC-B), encoded by TMEM38B, is a potassium (K+) channel present in the endoplasmic reticulum membrane, where it counterbalances calcium (Ca2+) exit. Lack of TRIC-B activity causes a recessive form of the skeletal disease osteogenesis imperfecta (OI), namely OI type XIV, characterized by impaired intracellular Ca2+ flux and defects in osteoblast (OB) differentiation and activity. Taking advantage of the OB-specific Tmem38b knockout mouse (Runx2Cre;Tmem38bfl/fl; cKO), we investigated how the ion imbalance affects the osteogenetic process. We found an…

Does Whole-Body Vibration Treatment Make Children's Bones Stronger?

PURPOSE OF REVIEW: To summarize the last 10 years of literature regarding the effects of whole-body vibration (WBV) on bone in children, and if WBV results in increased bone acquisition. RECENT FINDINGS: WBV intervention appears to be a safe intervention with beneficial effects on bone mass in some diseases and syndromes, but there is still low evidence for WBV in clinical practice. The positive effects on muscle strength, balance, and walking speed are more conclusive. One of the takeaways of this review is that well-trained individuals may not further improve bone mass with WBV; thus, inter

Aqueous extract of Ormocarpum cochinchinense promotes fracture healing through coordinated angiogenic, osteogenic, and cytoprotective mechanisms.

Ormocarpum cochinchinense has long been used in traditional medicine for the management of bone fractures. Despite its widespread ethnomedicinal use, scientific evidence supporting its efficacy and underlying mechanisms in fracture healing remains limited. This study aimed to investigate the safety and fracture-healing potential of the aqueous extract of O. cochinchinense (OCE) with emphasis on angiogenesis and osteogenesis, and to elucidate its role in modulating endothelial cell inflammation. The biological activity of OCE was assessed using a combination of in vitro and in vivo approaches. …
