Management of eating disorders for people with higher weight: clinical practice guideline.

INTRODUCTION: The prevalence of eating disorders is high in people with higher weight. However, despite this, eating disorders experienced by people with higher weight have been consistently under-recognised and under-treated, and there is little to guide clinicians in the management of eating disorders in this population. AIM: The aim of this guideline is to synthesise the current best practice approaches to the management of eating disorders in people with higher weight and make evidence-based clinical practice recommendations. METHODS: The National Eating Disorders Collaboration Steering

Immersive virtual reality for cognitive rehabilitation in patients who had a stroke: study protocol for a randomised controlled trial.

INTRODUCTION: Poststroke cognitive impairment (PSCI) is a prevalent complication of stroke, characterised by deficits in one or more cognitive domains (eg, memory, attention, executive function). Beyond increasing mortality and disability risks, PSCI frequently co-occurs with motor dysfunction, which impairs activities of daily living and reduces quality of life. Due to the complexity of neural networks involved in PSCI, clinical practice currently lacks targeted therapeutic strategies; existing interventions (eg, pharmacotherapy, traditional cognitive training) are limited in scope and variab

Effect of synbiotic supplementation and psychobiotic diet on prenatal and postnatal depression and anxiety in women with gestational diabetes: a three-arm randomised controlled trial protocol.

BACKGROUND: Gestational diabetes mellitus (GDM) is one of the most prevalent metabolic disorders during pregnancy and may lead to various short-term and long-term complications for both the mother and the fetus. It is also associated with psychological disorders, such as anxiety and depression, the prevalence of which is reported to be two to four times higher than that among healthy pregnant women. Psychobiotics may improve cognitive and emotional function through modulation of the gut-brain axis. Therefore, this study was designed to evaluate the prophylactic effects of synbiotic supplementa

'Time is of the essence': upper-body aerobic exercise to improve cardiovascular health during inpatient rehabilitation within the first year following spinal cord injury - protocol for a randomised clinical trial.

INTRODUCTION: Individuals with spinal cord injury (SCI) face heightened cardiovascular disease (CVD) risks. While general exercise guidelines are promoted for SCI individuals, when and how to incorporate exercise during the subacute phase post-SCI remains unclear. Consequently, early aerobic exercise to reduce CVD risks is not standard practice in subacute SCI care, potentially missing an opportunity for optimal cardiovascular rehabilitation, especially given observed reductions in cardiac structure and function within the first year post-SCI. Addressing this gap could improve long-term cardio

Effect of osteopathic manipulative treatment on comorbid depressive symptoms in patients with chronic low back pain: study protocol for a randomised controlled trial.

INTRODUCTION: Chronic low back pain (CLBP) and depressive symptoms (DS) are highly prevalent, burdensome, costly and comorbid health conditions. Osteopathic manipulative treatment (OMT) was shown to improve pain and disability in patients with CLBP; however, the effect on comorbid DS remains less certain. Interestingly, CLBP and DS seem to be associated with changes in interoception, which may be reversed by OMT. METHODS AND ANALYSIS: The study protocol proposes a single-blinded, parallel-group, randomised controlled trial to investigate the effect of OMT on clinical symptoms (depression, pai

Effectiveness of an internet-based self-help intervention for improving mental health in individuals with obesity: study protocol for a randomised controlled trial.

INTRODUCTION: Patients with chronic somatic diseases such as obesity often develop comorbid depressive symptoms. E-mental health interventions are an innovative and effective treatment option within a stepped care approach. Studies have shown that acceptance and adherence are higher when they are tailored to the specific needs of the target group. This study protocol describes a randomised controlled trial (RCT) of an internet-based self-help intervention, Fit4Mood, to improve mental health in the high-risk group of adults with obesity. The objective of the @ktivPLUS research project is to eva

Preoperative pancreatic stent placement before the enucleation of insulinoma located in the head and neck of the pancreas in proximity to the main pancreatic duct: study protocol for a multicentre randomised clinical trial in Chinese tertiary medical centres.

INTRODUCTION: The surgical intervention approach to insulinomas in proximity to the main pancreatic duct remains controversial. Standard pancreatic resection is recommended by several guidelines; however, enucleation (EN) still attracts surgeons with less risk of late exocrine/endocrine insufficiency, despite a higher postoperative pancreatic fistula (POPF) rate. Recently, the efficacy and safety of preoperative pancreatic stent placement before the EN have been demonstrated. Thus, a multicentre open-label study is being conducted to evaluate the efficacy and safety of stent placement in impro

Exploration of effects of galvanic vestibular stimulation on circadian rhythms and its associations with sleep and spatial memory in patients with breast cancer: The ICANSLEEP-2 protocol.

BACKGROUND: Patients with breast cancer (BC) exhibit circadian rhythm disruptions, mainly of rest-activity rhythm (RAR), of which sleep is an essential component, and cortisol rhythm. Sleep complaints such as insomnia and cognitive impairments are prevalent in BC. In general population, sleep is known to contribute greatly to cognition. Thus, improving RAR (and particularly sleep) could help limiting cognitive impairments in BC patients. It has recently been suggested that, in addition to its essential role in spatial memory, the vestibular system contributes to RAR synchronization. Its stimul

Intensive group-based cognitive therapy in patients with cardiac disease and psychological distress-a randomized controlled trial protocol.

BACKGROUND: Many patients with coronary artery disease (CAD) and valvular heart disease (VHD) suffer from psychological distress. Such stress is associated with increased morbidity, reduced quality of life and delayed return to work. European guidelines emphasize recognition and intervention, but evidence-based treatment options are limited and perceived as costly. The present study will test the effect of brief, group-based cognitive therapy as an adjunct to usual cardiac rehabilitation in a randomized design. METHODS: A total of 148 patients with CAD and/or VHD after surgical intervention a

Comparison of blood pressure measurements using an automated blood pressure device in community pharmacies and family physicians' offices: a randomized controlled trial.

BACKGROUND: Accurate measurement of blood pressure is the foundation of appropriate diagnosis, treatment and ongoing management of hypertension. The use of automated blood pressure devices in community settings such as pharmacies provide opportunities for additional blood pressure measurement; however, it is important to ensure that these measurements are comparable to those taken in physicians' offices using the same devices. We conducted a randomized controlled trial to assess whether blood pressure readings assessed by use of an automated device differed according to the setting, specifical

Behavioural intervention and guided stepping training early post-stroke (BIG STEPS): protocol for a multicentre randomised controlled trial.

INTRODUCTION: Physical inactivity and prolonged sedentary time are associated with lower functional and quality of life outcomes after stroke, and may increase the risk of stroke recurrence. Yet, the best way to manage these movement behaviours after stroke remains uncertain. This study aims to test the effects of a 3-month behavioural intervention and guided stepping training early post-stroke (BIG STEPS) programme on daily step counts, sedentary time, timed walking outcomes and patient-reported fatigue, cognitive function and quality of life. METHODS AND ANALYSIS: This is a multicentre, ass

Does omission of a loose seton before sphincter-preserving surgery achieve non-inferior fistula closure compared with routine loose seton placement in adults with single-tract cryptoglandular anal fistula in the Netherlands? Study protocol for the SETON multicentre randomised controlled non-inferiority trial and cost-utility analysis.

INTRODUCTION: Cryptoglandular anal fistulas are burdensome, often recurrent and frequently treated in two stages using a loose seton before definitive sphincter-preserving surgery. Although this practice is common, high-quality evidence supporting improved healing or functional outcomes is lacking. A trial is needed to determine whether omitting routine seton placement can reduce procedures, patient burden and costs without compromising fistula closure. METHODS AND ANALYSIS: The SETON trial is a multicentre, randomised, unblinded non-inferiority trial with an integrated cost-utility analysis.
