A long-term longitudinal follow-up of depressed patients treated with ECT with special focus on development of dementia.

BACKGROUND: In this study, the long term effects of ECT on patients with depression were investigated through repeated rCBF and EEG measures as well as clinical characteristics over several years. The aim of the investigation was to establish an association with the eventual development of dementia. METHOD: A cohort of forty-nine patients (21 men and 28 women) with a mean age of 61 years underwent ECT. A subsequent evaluation from medical records and three rating-scales for diagnosis of Alzheimer´s disease (AD), fronto-temporal dementia (FTD), and for vascular dementia (VaD), revealed that 17

A nine-year population-based cohort study on the risk of multiple sclerosis in patients with optic neuritis.

Patients with optic neuritis (ON) are at an increased risk of developing multiple sclerosis (MS), an illness that may result in physical dysfunction and short life expectancy. Information on the conversion rate to MS of patients with ON is essential in determining the impact of ON on the incidence of MS. Previous Taiwanese studies on the risk of MS in patients with ON were all hospital based, thereby limiting the generalizability of the findings. We aimed to estimate the risk of MS in patients with ON using a nationally representative sample. A cohort of 2,741 patients who sought outpatient ca

The lifetime risk of adult-onset rheumatoid arthritis and other inflammatory autoimmune rheumatic diseases.

OBJECTIVE: Understanding of the personal risks for rheumatoid arthritis (RA) and other rheumatic diseases remains poor, despite advances in knowledge with regard to their pathogenesis, therapeutics, and clinical impact, in part because the personal lifetime risk of developing these diseases is unknown. This study was undertaken to estimate the lifetime risk of RA, as well as other inflammatory autoimmune rheumatic diseases, including systemic lupus erythematosus, psoriatic arthritis, polymyalgia rheumatica (PMR), giant cell arteritis, ankylosing spondylitis, and Sjögren's syndrome, and to prov

Immediate-type hypersensitivity and other clinical reactions in volunteers immunized with a synthetic multi-antigen peptide vaccine (PfCS-MAP1NYU) against Plasmodium falciparum sporozoites.

We tested the clinical reactions to a synthetic, Plasmodium falciparum, circumsporozoite multiple antigen peptide (MAP) vaccine in 39 volunteers immunized two to three times over 2-8 months using a dose escalation design. Immediate pain at the injection site was associated with the adjuvant QS-21 (P<0.001), and delayed local inflammatory reactions were associated with high-titered circulating IgG anti-MAP antibody (P=0.03). Because two volunteers developed acute, systemic urticaria after the third immunization associated with development of serum IgE MAP antibody, we employed immediate-type hy

Tuberculosis progression rates in U.S. Immigrants following screening with interferon-gamma release assays.

BACKGROUND: Interferon-gamma release assays may be used as an alternative to the tuberculin skin test for detection of M. tuberculosis infection. However, the risk of active tuberculosis disease following screening using interferon-gamma release assays in immigrants is not well defined. To address these uncertainties, we determined the incidence rates of active tuberculosis disease in a cohort of high-risk immigrants with Class B TB screened with interferon-gamma release assays (IGRAs) upon arrival in the United States. METHODS: Using a retrospective cohort design, we enrolled recent U.S. imm

Keeping the body in mind: Development of the Transdiagnostic Global Impression-Somatic Symptoms and Signs (TGI-3S) scale.

BACKGROUND: It is well-established that mental illness is associated with excess and premature mortality. Consequently, the assessment of physical health should be routine in the evaluation of persons living with mental disorders. Herein, we introduce the development of a global instrument to assess physical health status in persons with mental disorders. METHODS: Following the development of the Transdiagnostic Global Impression-Psychopathology (TGI-P) scale, a novel scale of physical health status, the Transdiagnostic Global Impression-Somatic Symptoms and Signs (TGI-3S), was created using

Decoding fibrotic encapsulation in plastic surgery implants: From biological mechanisms to precision interventions frontiers.

Mitigating fibrotic encapsulation around implants is critical to ensuring the long-term success of implant-based procedures in plastic surgery. Fibrous capsule formation is a pathological foreign body reaction (FBR) that not only causes capsular contracture and implant failure but also creates a permissive microenvironment for bacterial colonization, thereby increasing the risk of infection. These adverse outcomes may ultimately necessitate implant removal or replacement, significantly compromising surgical results and patient satisfaction. Therefore, a thorough understanding of the mechanisms

A simpler definition of MAFLD precisely predicts incident metabolic diseases: a 7-year cohort study.

BACKGROUND: Metabolic dysfunction-associated fatty liver disease (MAFLD) is a novel definition proposed in 2020 with a relatively complex set of criteria. Thus, simplified criteria that are more applicable are required. This study aimed to develop a simplified set of criteria for identifying MAFLD and predicting MAFLD-related metabolic diseases. METHODS: We developed a simplified set of metabolic syndrome-based criteria for MAFLD, and compared the performance of the simplified criteria with that of the original criteria in predicting MAFLD-related metabolic diseases in a 7-year follow-up.
