[Adeno-tonsillar hyperplasia and obstructive sleep related respiratory disorders].

The most common cause of obstructive apnoeas in children is adenotonsillar hyperplasia. Possibilities of therapy are surgical intervention (adeniodectomy, tonsillectomy) or nasal CPAP. The decision on the most appropriate therapy must be individual. 26 children aged 2-5 years were investigated polysomnographically before therapy, after operation or with nCPAP-therapy. Polygraphy is a helpful aid in making up one's mind in favour of one of the possibilities of therapy, but also for controlling the therapeutic effect.

Predictors of sleepiness in epilepsy patients.

Sleepiness, a common complaint of epilepsy patients, is frequently attributed to antiepileptic medications. To determine predictors of subjective sleepiness in epilepsy patients, we gave self-administered, validated surveys of sleepiness [Epworth sleepiness scale (our major outcome measure)] and sleep apnea [sleep apnea scale of the sleep disorders questionnaire (SA/SDQ)] to 158 epilepsy patients and 68 neurology patients without epilepsy (controls). An elevated Epworth score (>10) was more likely in epilepsy patients compared to controls after controlling for age and gender (p < 0.05). When E

[The characteristics of the structure of night sleep in patients who have had a stroke].

The aim of the study was investigation of the structure of the night sleep in relation to the disease stage and location of the destructive focus. 18 patients with ischemic stroke (in the right hemisphere-8 cases, in the left one-6 persons, with brain stem location-4 patients) were observed as well as 5 practically healthy individuals. The diagnosis was verified by computer tomography in all cases. Clinical neurological investigation and polygraphy of night sleep (electroencephalography, electrooculography, electromyography) were performed. Analysis of sleep parameters was carried out accordin

Risk of traffic accidents in patients with sleep-disordered breathing: reduction with nasal CPAP.

Sleepiness whilst driving constitutes a road safety risk. Sleep-related breathing disorders are the most frequent medical cause of daytime sleepiness, and untreated patients with this condition have been shown to be at a higher risk of having accidents while driving. This study addressed the question of the extent to which treatment of sleep-disordered breathing by nasal continuous positive airway pressure (nCPAP) is related to changes in patient's accident risk. Seventy eight male patients requiring treatment of sleep-related breathing disorders with nCPAP were enrolled in the study. The prot

Effects of overnight supplemental oxygen in obstructive sleep apnea in children.

Supplemental oxygen during sleep may be useful as a temporary palliative treatment in children with obstructive sleep apnea syndrome (OSAS) associated with significant hypoxemia. However, supplemental O2 may also blunt hypoxic ventilatory drive and worsen ventilation. To assess the safety of the use of supplemental O2 in children with OSAS, we studied 16 children ages 2-8 (mean: 4.28 +/- 2.88 yr) with OSAS secondary to adenotonsillar hypertrophy. Patients underwent two overnight polysomnograms within 1 mo, one on room air (RA) and one while receiving supplemental O2 via nasal cannula titrated

[Apnea characteristics of children who later died: comparison of sudden infant death with other causes of death].

Apnea characteristics of infants, who were destined to die, were compared with control infants, matched for gestational age, sex, birth weight and age at sleep study. From 1989 to 1994 polygraphic recordings were performed in 4268 infants. In this population 7 cases of sudden infant death (SID) and 13 deaths from other causes (non-SID) occurred. Concerning all forms of apneas a higher frequency and maximal duration was found in the SID group. The number of infants with obstructive apneas and the number of obstructive apneas per infant were higher in future SID victims, as was the frequency, me

Tranylcypromine in narcolepsy.

The narcolepsy syndrome consists of excessive daytime somnolence, cataplexy, hallucinations, and sleep paralysis. Data on the effects of tranylcypromine are scant. We report on a patient with severe narcolepsy in whom administration of tranylcypromine led to freedom from hallucinations, nightmares, sleep paralysis, and rapid eye movements (REM), with considerable amelioration of cataplexy and increased daytime alertness. Muscle atonia and low-voltage desynchronized cortical activity during REM sleep did not change. Tranylcypromine warrants further study in narcolepsy.

Effects of submental stimulation for several consecutive nights in patients with obstructive sleep apnoea.

BACKGROUND: It has previously been reported that short term submental stimulation can reduce the frequency of apnoea and improve sleep architecture in patients with obstructive sleep apnoea. The effects of submental stimulation during consecutive nights on apnoea or on daytime sleepiness have not, however, been studied. METHODS: Patients with obstructive sleep apnoea were studied by polysomnography on a control night, for five consecutive nights of submental stimulation, and on three following nights (n = 8). A multiple sleep latency test (MSLT) (n = 8) and measurement of the upper airway res…

Serotoninergic agents in the treatment of Gilles de la Tourette's syndrome.

A preliminary trial with fluoxetine, a 5-HT reuptake inhibitor, was carried out on two young male patients (21 and 32 years old) affected by Gilles de la Tourette syndrome. They both underwent a complete neurological evaluation also including neuroradiological, neurophysiological and neuropsychological assessment. Both patients had already been treated with benzodiazepines and amitriptyline; the older one was also given haloperidol and chlorimipramine with definite, but short-lasting improvement. During hospitalization a therapeutic trial with fluoxetine (20 mg/day in the younger patient and 4

Obstructive sleep apnea, control of ventilation, and anesthesia in children.

Pathologic disturbances in breathing in children become clinically apparent in the syndrome of obstructive sleep apnea. The surgical approach to this syndrome is often removal of hypertrophied tonsils and adenoids. The preoperative, operative, and postoperative concerns with special attention to the role of anesthesia are reviewed.

Concurrent nocturnal and diurnal paroxysmal dystonia.

The authors describe a case of a 15-year-old girl who presented with a progressive sequence of nocturnal dystonic episodes, generalized seizures, and diurnal dystonic episodes. A diagnosis of nocturnal paroxysmal dystonia has been proposed in view of the association between nocturnal episodes and epileptic seizures, while the diurnal episodes are atypical. Neurophysiological data recorded during critical episodes are reported. A linkage between dystonic attacks and epilepsy is hypothesized.

Antimineralocorticoid canrenoate enhances secretory activity of the hypothalamus-pituitary-adrenocortical (HPA) axis in humans.

In rats, both hippocampal glucocorticoid and mineralocorticoid receptors (MR) have been shown to participate in the regulation of basal hypothalamus-pituitary-adrenocortical (HPA) secretory activity. Inhibition of hippocampal MRs enhanced the activity of the HPA axis in these animals. We tested the influence of potassium cancrenoate, a selective MR antagonist, on basal cortisol secretion in 10 healthy young men during sleep. Cortisol, ACTH, vasopressin and growth hormone (GH) were determined at 22.00, 23.00, 01.00, 04.00 and 07.00 h. Sleep was monitored by somnopolygraphy. Potassium canrenoate

Fast-computed tomographic evaluation of the effect of route of breathing on upper airway size and function in normal men.

Nasal obstruction is associated with increased sleep disordered breathing (SDB), even in normal subjects. This increase in SDB may result from narrowing of the orohypopharyngeal (OHP) or retroglossal segment of the upper airway (UA) due to retropositioning of the jaw and tongue base as the mouth is opened and route of breathing changed from nasal to oral. It is postulated that significant narrowing of the OHP occurs with oral breathing even in the awake state. To ascertain the effect of route of breathing on the UA, fast-CT was used to study the UA response to the route of breathing in 30 norm

Abnormal resting-state EEG microstates correlate with objective sleep parameters in insomnia disorder.

BACKGROUND: Insomnia is a common sleep disorder, and Electroencephalogram (EEG) microstates, which mirror transient cerebral cortical functional activity, are linked to sleep disorder pathophysiology. The microstate characteristics in insomnia patients and correlations with objective sleep parameters remain to be further explored. METHOD: This study enrolled 45 insomnia disorder (ID) patients and 48 healthy controls (HC), collecting awake resting-state EEG for microstate analysis; 27 ID and 13 H C participants additionally completed overnight polysomnography (PSG), and exploratory correlation…

Hypoxic Burden Is an Important Determinant of Height in Nonobese Children With Sleep-Disordered Breathing.

The relationship between linear growth and obstructive sleep-disordered breathing (SDB) has not been extensively studied. Hypoxic burden (HB) is a new metric that may better capture end-organ damage in paediatric SDB compared to the traditional apnea-hypopnea index (AHI). This observational cross-sectional study examined the association between HB and linear growth in nonobese children aged 3-9 years with SDB. HB was assessed as the area under the peripheral oxygen desaturation curve, alongside standard polysomnography indices. A cohort of 164 children without comorbidities except for asthma w…

The sleep EEG spectral profile of narcolepsy with comorbid obstructive sleep apnea more closely resembles narcolepsy than obstructive sleep apnea.

BACKGROUND: Narcolepsy (NT) and obstructive sleep apnea (OSA) frequently coexist, yet the sleep electroencephalographic (EEG) spectral profile of narcolepsy with comorbid OSA (NT-OSA) remains unclear. We compared spectral power among NT-OSA, NT, and OSA across polysomnography (PSG) and Multiple Sleep Latency Test (MSLT). METHODS: We retrospectively analyzed 224 treatment-naïve adults with excessive daytime sleepiness (37 NT-OSA, 77 NT, and 110 OSA); 39 subjectively sleepy controls (SSC) were included in a secondary analysis. Relative EEG spectral power was calculated in central and occipital …

Seismo-Mat: mattress-embedded inertial sensors for unobtrusive monitoring of heart rate, respiratory rate, and bed occupancy during sleep.

OBJECTIVE: Continuous monitoring of heart rate (HR) and respiratory rate (RR) during sleep may support longitudinal assessment and the identification of clinically relevant trends, while current gold-standard techniques such as polysomnography are intrusive and unsuitable for prolonged home use. This study aims to develop and assess the feasibility of an unobtrusive system for overnight monitoring of HR, RR, and preliminary bed occupancy detection in real-world settings. METHODS: A novel nearable system is presented, based on multiple mattress-embedded inertial sensors capturing the mechanica…

Sleep dependent autonomic correlates of affect and arousal in younger and older adults.

STUDY OBJECTIVES: Aging alters affect, with older vs. younger adults exhibiting greater positive and reduced negative affect. Sleep and autonomic factors contribute to this change, yet it is untested whether sleep-dependent autonomic activity plays a role. The current study examined bidirectional associations between affect, arousal, sleep architecture, and cardiac autonomic activity across a daytime nap in healthy younger and older adults. MATERIALS AND METHODS: Polysomnography and electrocardiogram were collected during a midday nap. Sleep architecture and autonomic activity outputs (i.e. h…
