Prevalence and incidence of pelvic inflammatory disease in incarcerated adolescents.

BACKGROUND: Few recent studies have determined the prevalence and incidence of pelvic inflammatory disease (PID) among adolescents. GOAL: The goal of this study was to determine these parameters among incarcerated youths. STUDY DESIGN: Both on admission and during incarceration, consecutive adolescents entering the Harris County, Texas, Juvenile Detention Center were evaluated for symptoms of PID. One of two experienced clinicians examined adolescents with possible PID. For the diagnosis of PID, we used the minimal criteria of the CDC. RESULTS: In sexually active heterosexual or bisexual ad

Tuberculosis control in the New York State Department of Correctional Services: a case management approach.

In the New York State Department of Correctional Services (DOCS), the incidence of tuberculosis (TB) increased from 43 per 100,000 persons in 1985 to 225 per 100,000 in 1991. To combat this increase in cases, the NYSDOCS enhanced its collaborative efforts with the New York State Department of Health and the Division of Parole, developing a comprehensive TB control program. This control program focuses on prevention and containment of disease. Policies and directives were developed on the basis of epidemiologic principles, which include surveillance and detection. To assist the NYSDOCS in the i

[Cornea-crystalline lens needle-prick wounds from self-mutilation].

Automutilation is a voluntary autoprovoked lesion, which determines infirmities of different degrees. The paper presents the case of a 25-year-old patient, who, being in detention, pricked his both eyes using a needle. This action provoked endophthalmitis and bilateral traumatic cataract. The particularity of the case consists of the bilaterality of the autoagression, with serious evolution at the both eyes.

Evaluation of two-step tuberculin testing in a Massachusetts correctional facility.

OBJECTIVES: To better identify inmates with tuberculosis infection at admission and to have more accurate estimates of conversion rates with annual retesting, I evaluated two-step tuberculin skin testing of newly admitted inmates at the Addiction Center of the Massachusetts Department of Corrections. METHOD: In a population of inmates admitted to the Addiction Center between January and May 1994, a second tuberculin skin test was performed 1 week after the first for those who initially had a negative response. RESULTS: There were 628 admissions during the study period; 51 of these inmates (8

Expanding Geroscience to Vulnerable Populations in Mid-Late Life: Medicaid Home- and Community-Based Service Users and Formerly Incarcerated Individuals.

A wide range of geroscience-guided interventions, or gerotherapeutics, including repurposed drugs, natural products, and lifestyle changes are now being tested in small-scale proof-of-concept studies. If successful, these efforts may help maintain or restore function across numerous health-related domains, thus extending human healthspan. To date, little attention has been paid to exploring the potential of gerotherapeutics to improve healthspan-related outcomes in vulnerable populations that have accumulated experiences detrimental to health in adulthood and later life. We contend that 2 vuln

Healthcare utilization and perceived substance use-related stigma from healthcare workers among incarcerated women with opioid use disorder.

PURPOSE: Women with substance use disorders often experience stigma from others, including healthcare workers (HCW), in part due to gendered expectations about substance use. Thus, there is likely a positive association between health service utilization and perceptions of stigma, but this has been understudied. This project explores the relationship between healthcare utilization and substance use-related stigma from HCW among incarcerated women with opioid use disorder (OUD). METHODS: Women from nine Kentucky jails were randomly selected, screened for OUD, and interviewed while incarcerated

The Well-Being of Older Offenders on Release in the Community.

In Canada and internationally the number of older offenders on parole and living in the community is increasing rapidly. Older offenders in the community are a vulnerable population at high risk for lack of well-being. Semi-structured interviews were conducted with N = 64 offenders aged 50 years and older on conditional release from custody in Canada, including long-term, recidivist, and first-time older offenders. Compared to their non-offender counterparts, older offenders in the community experience many of the same problems of aging and well-being, but are at greater risk for mental health

The Effect of Peer Influence and Neighborhood Quality on Incarcerated Fathers' Attachment.

Commonly referred to as the "hidden victims" of incarceration, children with a parent who is intermittently or repeatedly incarcerated face various challenges that exacerbate behavioral and psychological development. Using a baseline adaptation of the Multi-site Family Study on Incarceration, Parenting and Partnering (MFS-IP), we sought to clarify how peer influence and neighborhood quality can predict the extent of an incarcerated father's attachment to the focal child and partner. Results showed a negative association between negative peer influence and poor neighborhood quality. Conversely,

Employment Outcomes of a Distance Learning System for Formerly Incarcerated Veterans With Mental Illness.

OBJECTIVE: People with previous incarceration and a mental disorder, a substance use disorder, or both are less likely to obtain employment than are those without these characteristics. Distance learning and education (DLE) systems are increasingly being integrated into clinical care and provide a pathway to disseminate vocational services. DLE systems can help reach people with barriers to accessing services, including people without transportation, experiencing homelessness, living in rural areas, or who are homebound. This study evaluated the effectiveness of one DLE system, the COMPASS pro

Use of harm reduction strategies by individuals with a history of incarceration: A short report using baseline data collected from the STAMINA clinical trial.

BACKGROUND: Due to the considerably heightened risk of overdose immediately following jail or prison release, expansion of harm reduction interventions for citizens returning to the community after an incarceration episode should be of utmost concern. However, there are no studies examining the adoption and use of harm reduction among this population. This short report examines the use of individual-level harm reduction strategies (HRS) among people who use drugs, comparing those who have lifetime histories of incarceration with those who do not. METHODS: This study included baseline data (N

The Patient Protection and Affordable Care Act and oral health care use among formerly incarcerated people in the United States.

BACKGROUND: Formerly incarcerated people report less frequent oral health care use, despite having more substantial oral health problems. This study aimed to determine whether the adoption of the Patient Protection and Affordable Care Act (ACA) has improved oral health care use among formerly incarcerated people in the United States. METHOD: Data were from Wave I (1994-1995), Wave IV (2008), and Wave V (2016-2018) of the National Longitudinal Study of Adolescent to Adult Health (n = 9,108), a nationally representative cohort study in the United States. RESULTS: On the basis of the results of

Our Health Counts: Examining associations between colonialism and ever being incarcerated among First Nations, Inuit, and Métis people in London, Thunder Bay, and Toronto, Canada.

OBJECTIVES: Indigenous peoples have a disproportionately high prevalence of incarceration in the Canadian justice system. However, there is limited Indigenous-driven research examining colonialism and the justice system, specifically associations between racism, externally imposed family disruptions, and history of ever being incarcerated. Therefore, this study examined the association between the proportion of previous incarceration and family disruption, experiences of racism, and victimization for Indigenous adults in London, Thunder Bay, and Toronto, Ontario, Canada. The three communities

Characteristics of Deaths Among Individuals in US Immigration and Customs Enforcement Detention Facilities, 2011-2018.

IMPORTANCE: Concerns have been raised that substandard medical care has contributed to deaths in US Immigration and Customs Enforcement (ICE) detention facilities. After each in-custody death, ICE produces detainee death reviews, which describe the circumstances of the death and determine whether ICE Performance-Based National Detention Standards (PBNDS) were violated. OBJECTIVE: To describe factors associated with deaths in ICE detention facilities. DESIGN, SETTING, AND PARTICIPANTS: This case series used data extracted from detainee death reviews of deaths among individuals detained in ICE

The Effects of Inmate Medical Issues on Correctional Staff Job Involvement and Organizational Commitment.

In the United States, institutional corrections is a major undertaking that requires substantial financial resources. Correctional staff are the essential element to ensuring the safety and security of these institutions, which house approximately 2.2 million adults. Thus, it is invaluable to explore work environment variables that contribute to their success. One area that has received scant attention is staff views of inmate medical care and their effect on job involvement and organizational commitment. This study focused on the effects of views concerning the proper handling of inmate infec

Closing the (incarceration) gap: assessing the socio-economic and clinical indicators of indigenous males by lifetime incarceration status.

BACKGROUND: Approximately 1 in 5 to 1 in 6 Indigenous Australian males are currently imprisoned or have previously been imprisoned. Recent work has also pointed to a widening socio-economic gap within the Indigenous population. Given the myriad social, wellbeing and environmental risk factors associated with justice-involvement, it is conceivable that incarceration may contribute to the increasing disparities found within the Indigenous population. This study aimed to explore the presence and extent of an 'incarceration gap' within the Indigenous population and to uncover which social factors

Confined to Obscurity: Health Challenges of Pregnant Women in Jail.

Research about pregnant women in jail is scant. This exploratory study begins to fill this gap by examining the demographics; background characteristics; and self-reported physical health, mental health, and substance use challenges reported by 27 pregnant women incarcerated in a large midwestern county jail. It further reports on the prenatal care before and during their incarceration, plans of these pregnant women for delivery of their child, caring for their infant after their release from jail, and their expectations of paternal or family support post-release. Among the sample, 66.7 percen

Reimagining VAWA: Why Criminalization Is a Failed Policy and What a Non-Carceral VAWA Could Look Like.

The Violence Against Women Act (VAWA) is the signature federal legislative accomplishment of the anti-violence movement and has ensured that criminalization is the primary response to intimate partner violence in the United States. But at the time of its passage, some anti-violence activists, particularly women of color, warned that criminalization would be problematic for a number of reasons, a caution that has borne fruit in the 25 years since VAWA's passage. This article critiques the effectiveness of criminalization as anti-domestic violence policy and imagines what a non-carceral VAWA cou
