Effects of basic and dual-task training programs on physical function in Parkinson's disease: The PARKEX study.

Exercise is a promising non-pharmacological intervention for motor and non-motor symptoms in Parkinson's disease (PD). However, direct comparisons between basic physical training (BPT) and dual-task training remain limited. The objective of the current clinical trial is to evaluate clinical and functional effects of BPT and a BPT combined with dual-task functional exercises (BPT + FE) in individuals with PD. In this randomized controlled trial, 24 participants with idiopathic PD were allocated to BPT, BPT + FE, or a no-intervention control group (Con). Interventions lasted 12 weeks (three 60-m

Associations Between Health-Related Quality of Life and Clinical Factors in Lumbar Disc Herniation: Evidence from a Romanian Cohort Using the SF-36.

Background: Lumbar disc herniation (LDH) has a substantial impact on health-related quality of life (HRQoL), mainly through pain, reduced mobility, and functional limitations. To better reflect these outcomes, we utilized the SF-36 survey, a well-established tool commonly used in both clinical and population research. The purpose was twofold: to evaluate HRQoL and to explore the main factors linked with activity limitation in a community-based group of Romanian patients. Methods: This cross-sectional study included 120 participants with LDH who completed the SF-36 questionnaire. Internal consi

Nursing Diagnoses, Planned Outcomes and Associated Interventions with Highly Complex Chronic Patients in Primary Care Settings: An Observational Study.

The information logged by nurses on electronic health records (EHRs) using standardised nursing languages can help us identify the characteristics of highly complex chronic patients (HCCP) by focusing on care in terms of patients' health needs. The aim of this study was to describe the profile of HCCPs using EHRs from primary care (PC) facilities, presenting patients' characteristics, functional status based on health patterns, NANDA-I nursing diagnoses, health goals based on Nursing Outcomes Classification (NOC), and care interventions using Nursing Interventions Classification (NIC). Can patient and pain characteristics predict manometric sphincter of Oddi dysfunction in patients with clinically suspected sphincter of Oddi dysfunction?

BACKGROUND: Biliopancreatic-type postcholecystectomy pain, without significant abnormalities on imaging and laboratory test results, has been categorized as "suspected" sphincter of Oddi dysfunction (SOD) type III. Clinical predictors of "manometric" SOD are important to avoid unnecessary ERCP, but are unknown. OBJECTIVE: To assess which clinical factors are associated with abnormal sphincter of Oddi manometry (SOM). DESIGN: Prospective, cross-sectional. SETTING: Tertiary. PATIENTS: A total of 214 patients with suspected SOD type III underwent ERCP and pancreatic SOM (pSOM; 85% dual SOM),

Benign Proliferative Nodules in a Giant Congenital Melanocytic Nevus: Diagnostic Challenge and Management.

Congenital melanocytic nevi are cutaneous malformations present at birth, resulting from a benign proliferation of melanocytes. Although generally benign, they can have significant psychological impacts and carry a non-negligible risk of progression to malignant melanoma. We report a case of a giant congenital melanocytic nevus associated with a perianal mass in an infant, two and half-month-old infant, with no notable medical history, born at term. He was referred for a pigmented perianal swelling located on a large hyperpigmented plaque covering the lower back, buttocks, perineum, external g

Sense of coherence as a protective factor in sleep and awake bruxism effects on adolescents quality of life.

This study aimed to assess whether the sense of coherence (SOC) moderates the association between self-reported sleep bruxism (SB) and self-reported awake bruxism (AB) on oral health-related quality of life (OHRQoL). A cross-sectional analysis was conducted with a random sample of 406 adolescents aged 11 to 17 years from Southern Brazil. OHRQoL was measured using the Child Perception Questionnaire (CPQ11-14). Both SB and AB were assessed by self-report (weekly frequency). SOC was measured using the 13-item Sense of Coherence Scale (SOC-13). Demographic, socioeconomic, and clinical variables we

Decoding chronic pain: integrating genetics, neuroimaging, and AI for precision management.

Chronic pain is increasingly recognised as a standalone medical condition shaped by interacting biological, psychological, and social determinants, affecting nearly one in three adults worldwide. This review synthesises contemporary evidence on the epidemiology, mechanisms, and management of chronic pain, with emphasis on the convergence of genetic, neurobiological, and psychosocial factors. We draw on recent population-based studies, clinical trials, neuroimaging research, and multi-omic genetic analyses to highlight the complexity and heterogeneity of this condition. Chronic pain disproporti

Partners, children, friends, and beyond: psychosocial pathways to life satisfaction among older adults aged 77.

OBJECTIVES: Social relationships are fundamental to well-being in later life, yet few studies differentiate between relationship types and the mechanisms through which they operate. This study models how different types of social relationships are associated with life satisfaction in very old age, considering the psychosocial pathways of perceived emotional and practical support, and loneliness. METHOD: Direct and indirect associations between relationships and life satisfaction were tested using structural equation modelling on 1,213 community-dwelling older adults (77+) participating in the
