Reverse arthroplasty for osteoarthritis and rotator cuff deficiency after previous surgery for recurrent anterior shoulder instability.

PURPOSE: Osteoarthritis in combination with rotator cuff deficiency following previous shoulder stabilisation surgery and after failed surgical treatment for chronic anterior shoulder dislocation is a challenging condition. The aim of this study was to analyse the results of reverse shoulder arthroplasty in such patients. METHODS: Thirteen patients with a median follow-up of 3.5 (range two to eight) years and a median age of 70 (range 48-82) years were included. In all shoulders a tear of at least one rotator cuff tendon in combination with osteoarthritis was present at the time of arthroplas

A history of reverse total shoulder arthroplasty.

BACKGROUND: Management of the cuff-deficient arthritic shoulder has long been challenging. Early unconstrained shoulder arthroplasty systems were associated with high complication and implant failure rates. The evolution toward the modern reverse shoulder arthroplasty includes many variables of constrained shoulder arthroplasty designs. QUESTIONS/PURPOSES: This review explores the development of reverse shoulder arthroplasty, specifically describing (1) the evolution of reverse shoulder arthroplasty designs, (2) the biomechanical variations in the evolution of this arthroplasty, and (3) the c

The influence of altering push force effectiveness on upper extremity demand during wheelchair propulsion.

Manual wheelchair propulsion has been linked to a high incidence of overuse injury and pain in the upper extremity, which may be caused by the high load requirements and low mechanical efficiency of the task. Previous studies have suggested that poor mechanical efficiency may be due to a low effective handrim force (i.e. applied force that is not directed tangential to the handrim). As a result, studies attempting to reduce upper extremity demand have used various measures of force effectiveness (e.g., fraction effective force, FEF) as a guide for modifying propulsion technique, developing reh

Sever-L'Episcopo tendon transfer in obstetric brachial plexus palsy.

Obstetric brachial plexus palsy produces functional and cosmetic impairment. The Sever-L'Episcopo procedure has been successfully used to improve external rotation, primarily in younger patients. Previous studies have shown steady improvement in motion and function for 1 year from the date of surgery, with little additional change beyond this period. This is a retrospective study of 7 patients who have had the procedure at the Shriners Hospital for Children in Tampa, Florida. Functional and cosmetic results have been uniformly excellent at an average follow-up of 1 year 3 months. No complicati

Arthroscopic management of refractory shoulder stiffness.

Glenohumeral stiffness is a major cause of shoulder disability and pain. Conventional management strategies often fail to yield consistent or prompt return of comfort and function. Over the past 5 years, we have employed a prospective approach to the evaluation and management of glenohumeral stiffness using arthroscopic release of capsular contractures for the most refractory. This report concerns the first 30 patients who failed at least 6 months (28 month mean) of nonoperative management for unilateral refractory shoulder stiffness and who were managed by arthroscopic capsular release. Fourt

Pseudoaneurysm as a complication of shoulder arthroscopy.

A case of pseudoaneurysm that is a rare complication of shoulder arthroscopy is reported. A 73-year-old man with rotator cuff tear underwent arthroscopic rotator cuff repair. On post-operative day 3, a pulsating mass (diameter 5 mm) was detected near the anterior portal. Then, the mass gradually enlarged, which was diagnosed as pseudoaneurysm. After the consultation with vascular surgeons, the mass was surgically excised following ligation of feeding artery 6 weeks after the arthroscopic surgery. Thus far, no recurrence has been observed. Level of evidence V.

Internal consistency, test-retest reliability and concurrent validity of a questionnaire on work-related exposure related to arm, shoulder and neck symptoms in computer workers.

The aim of this study was to determine the internal consistency, test-retest reliability and concurrent validity of the RSI QuickScan, a newly developed questionnaire that aims to identify the presumed risk factors for neck, shoulder and arm symptoms in a population of computer workers. The internal consistency was calculated using item analysis. The test-retest reliability and concurrent validity were analysed by calculating the percentage of agreement, Cohen's Kappa and the Ppositive and Pnegative. The concurrent validity was also tested by comparing the results from the new questionnaire wi

[Complications and failures through negative selection in replantation of amputated extremities].

Two years of experience with our replantation service in Vienna serves to define the criteria by which an amputated member is deemed worthy of replantation. 68 cases are analyzed and the complications and failures are discussed. The criteria took into account the type of injury, the ischemia time, the method adopted to preserve the severed part as well as the functional need of the amputee. We believe that by today's standard of replantation surgery, with the aid of microsurgical methods a responsible approach to indications would improve the functional results. Replantation capability, replan

Distal radius fractures result in alterations in scapular kinematics: a three-dimensional motion analysis.

BACKGROUND: Scapular motion is closely integrated with arm motion. Injury to a distal segment requires compensatory changes in the proximal segments leading to alterations in scapular motion. Since the effects of distal injuries on scapular kinematics remain unknown, in the present study we investigated the influences on scapular motion in patients with distal injuries. METHODS: Sixteen subjects with a history of distal radius fracture and 20 asymptomatic healthy subjects (controls) participated in the study. Three-dimensional scapular and humeral kinematic data were collected on all 3 planes

Rotator cuff injuries in adolescent athletes.

The cause of rotator cuff injuries in the young athlete has been described as an overuse injury related to internal impingement. Abduction coupled with external rotation is believed to impinge on the rotator cuff, specifically the supraspinatus, and lead to undersurface tears that can progress to full-thickness tears. This impingement is believed to be worsened with increased range of motion and instability in overhead athletes. A retrospective review of seven patients diagnosed with rotator cuff injuries was performed to better understand this shoulder injury pattern. The type of sport played
