Incidence of temporal thumb sign in adult patients with idiopathic seizures with and without elevated opening cerebrospinal fluid pressure.

OBJECTIVES: The temporal thumb sign (TTS) has emerged as a potential distinct magnetic resonance imaging (MRI) finding in pediatric patients with new-onset idiopathic seizures which is characterized by the protrusion of the inferior temporal gyrus into the skull base. This study investigated the prevalence and clinical implications of TTS in adult patients with idiopathic seizures, with or without elevated opening cerebrospinal fluid (CSF) pressure, and those diagnosed with idiopathic intracranial hypertension (IIH) without seizures. MATERIALS AND METHODS: A retrospective study was conducted …

The lateral transorbital approach to the medial sphenoid wing, anterior clinoid, middle fossa, cavernous sinus, and Meckel's cave: target-based classification, approach-related complications, and intermediate-term ocular outcomes.

OBJECTIVE: The lateral transorbital approach (LTOA) is a relatively new minimal access skull base approach suited for addressing paramedian pathology of the anterior and middle fossa. The authors define target zones for this approach and describe a series of cases with detailed measurements of visual outcomes, including those obtained with exophthalmometry. METHODS: The authors performed a retrospective analysis of a consecutive series of LTOA patients. Seven target zones were identified: 1) the orbit, 2) the lesser sphenoid wing and anterior clinoid, 3) the middle fossa, 4) the lateral wall …

Brainstem auditory evoked potential combined with high resolution cranial base CT can optimize the diagnosis of auditory nerve injury.

PURPOSE: Auditory nerve injury is one of the most common nerve injury complications of skull base fractures. However, there is currently a lack of auxiliary examination methods for its direct diagnosis. The purpose of this study was to find a more efficient and accurate means of diagnosis for auditory nerve injury. METHODS: Through retrospectively analyzing the results of brainstem auditory evoked potential (BAEP) and high-resolution CT (HRCT) in 37 patients with hearing impairment following trauma from January 1, 2018 to July 31, 2020, the role of the two inspection methods in the diagnosis …

Endoscopic transsphenoidal anterior petrosal approach for locally aggressive tumors involving the internal auditory canal, jugular fossa, and cavernous sinus.

OBJECTIVE Reports about endoscopic endonasal surgery for skull base tumors involving the lateral part of petrous apex remain scarce. The authors present their experience with the endoscopic transsphenoidal anterior petrosal (ETAP) approach through the retrocarotid space for tumors involving the internal auditory canal, jugular fossa, and cavernous sinus. METHODS The authors performed the ETAP approach in 10 patients with 11 tumors (bilateral in 1 patient) that extensively occupied the lateral part of petrous apex, e.g., the internal auditory canal and jugular fossa. Eight patients presented wi…

Visual recovery following optic nerve decompression for chronic compressive neuropathy.

PURPOSE: Visual failure due to optic nerve compression is a common indication for decompressive surgery. Most data only refer to the odds of improvement, deterioration or remaining the same. However, patients frequently wish to know more detail about the outcomes of surgery. Our aim was to assess the visual outcome from optic nerve decompression for visual failure in detail in order to help counsel patients pre-operatively. METHODS: Sixty-eight patients undergoing 71 operations to decompress 87 optic nerves between 1991 and 2007 were identified. Thirty-four decompressions were performed via a

The Transposed Sagittal Bandeau Technique for Correcting Scaphocephaly.

Correcting the scaphocephalic skull shape caused by sagittal craniosynostosis has likely produced a wider variety of treatment approaches than any other single-suture synostosis. Considering that the goals of surgery are to enlarge the intracranial space to facilitate cerebral blood flow and to normalize appearance in all 3 dimensions, the ideal repair should safely and effectively accomplish both objectives. A technical variation on a remodeling procedure is presented, based on the creation of a sagittal bandeau, which provides the foundation for a correction that not only widens the posterio

[Skull deformities following unilateral mandibular dysfunction. 1. General review of secondary changes].

Communications 1 to 5 will deal with the significance of the masticatory muscles as a local factor affecting the shaping of the skull. The reports are based on experiments with dogs (Beagles) on which unilateral partial resection of the mandible was performed at different ages. The animals were slaughtered one year after the operation. 1st Bulletin: More or less severe scoliosis was observed in all skulls and proved to be the principal secondary change. The deviation was found to be particularly great in the region of the viscerocranium in all cases. Other findings include a flattening of the

Using the head as a mould for cranioplasty with methylmethacrylate.

Methacrylate is a valuable tool to the neurosurgeon, even though it is currently being replaced by custom bone. During cranioplasty in the absence of custom bone, which is preformed based on the patients imaging, one has to make a cast to cover the cranial defect with or without the use of a mould. A good artificial skull outline is necessary for prevention of implant extrusion and acceptable cosmetic outcome. Using the patients head as a mould is a simple, cheap, and useful technique. An incision is made, and either a craniectomy or an attempt at skull elevation or separation of the scalp fro

Effect of cephalometer misalignment on calculations of facial asymmetry.

INTRODUCTION: In this study, we evaluated errors introduced into the interpretation of facial asymmetry on posteroanterior (PA) cephalograms due to malpositioning of the x-ray emitter focal spot. We tested the hypothesis that horizontal displacements of the emitter from its ideal position would produce systematic displacements of skull landmarks that could be fully accounted for by the rules of projective geometry alone. METHODS: A representative dry skull with 22 metal markers was used to generate a series of PA images from different emitter positions by using a fully calibrated stereo cepha

Combining 3D printing and elastographic characterization: A novel sphenoid wing meningioma simulation model for neurosurgical training.

High-fidelity simulation models are crucial for advancing neurosurgical training, particularly for complex skull base pathologies such as sphenoid wing meningiomas. This study introduces and validates a novel, cost-effective 3D-printed simulator specifically designed for sphenoid wing meningioma resection. A key innovation of this model is the integration of shear wave elastography (SWE) to enable objective biomechanical validation of tumor-mimicking materials. A patient-specific skull model was created using fused deposition modeling (FDM) 3D printing and paired with custom-molded tumor repli…

Three-Dimensional PET Imaging Reveals Canal-like Networks for Amyloid Beta Clearance to the Peripheral Lymphatic System.

18F-Florbetapir PET imaging is widely used to assess amyloid-β (Aβ) burden in the brain, particularly in the context of Alzheimer's disease … NS fluid compartments, forming canal-like networks that extend from the brain parenchyma toward the skull base, particularly the occipital clivus, and connect to the cervical lymph nodes. Additional Aβ signals were observed along the internal carotid plexus. These findings suggest that, when reconstruction in 3D and enhanced with AI, 18F-florbetapir PET imag …

Rare case report and literature review of intracranial mesenchymal chondrosarcoma.

Intracranial mesenchymal chondrosarcoma (IMC) is a rare primary malignant tumor in the skull, but mostly originates from the abnormal residual chondrocytes in the embryonic period, which grow slowly, and primarily occurs at the junction of the cartilage of the skull base. IMC is difficult to diagnose by preoperative imaging and is easily misdiagnosed. It needs to be differentiated from meningiomas, gliomas, hemangioma, fibroids, etc.; this article introduces a case of primary IMC in a 38-year-old female teacher, and reviews the literature on the diagnosis and treatment of symptoms. The patient…

The ethmoid and presphenoid of cetaceans.

A cribriform plate, a perpendicular plate, and two lateral masses are major components of the ethmoid bone of mammals. Notwithstanding the noticeable bone, virtually sitting in the center of the skull, extensive modifications of the skull of modern cetaceans, especially odontocetes (toothed whales), and the lack of clarity as to what characteristics delimit each element of the ethmoid has made the problem of the nature of the cetacean ethmoid more complicated and elusive than in other, less modified mammals. Furthermore, contention as to whether a perpendicular plate of the ethmoid, or the mes

Clival chordomas: considerations after 16 years of endoscopic endonasal surgery.

OBJECTIVE In the past decade, the role of the endoscopic endonasal approach (EEA) has relevantly evolved for skull base tumors. In this study, the authors review their surgical experience with using an EEA in the treatment of clival chordomas, which are deep and infiltrative skull base lesions, and they highlight the advantages and limitations of this ventral approach. METHODS All consecutive cases of chordoma treated with an EEA between 1998 and 2015 at a single institution are included in this study. Preoperative assessment consisted of neuroimaging (MRI and CT with angiography sequences) an…
