[Associated heterophoria and the asymmetry of ocular prevalence for stereo images in front of or behind a reference plane].

BACKGROUND: An unequal weighting of the eyes in the directional perception of stereodisparate objects, is referred to as ocular prevalence of the right or left eye, respectively. Between 1962 and 1964 H.-J. Haase developed a valence test for the prismatic correction of heterophoria. He suggested that there would be less prevalence for stereo images presented in front of or behind a reference plane, and that this asymmetry of prevalence may be related to the direction of the associated heterophoria (eso- or exophoria). According to H.-J. Haase, the asymmetry may indicate a fixation disparity wi

Repeated measurements of contrast sensitivity reveal limits to visual plasticity after early binocular deprivation in humans.

Contrast sensitivity improves in visually normal children until 7 years of age and is impaired in children who experienced early visual deprivation from bilateral congenital cataracts. Here, we investigated whether the deficits after early visual deprivation change during childhood by retesting the contrast sensitivity of seven patients treated for bilateral congenital cataract who had been first tested before 7.5 years of age, and of two patients first tested after 11 years of age. For the younger group, contrast sensitivity at low spatial frequencies improved after 1- and 2-year intervals, w

Survey of ophthalmic conditions in a Labrador community: II. Ocular disease.

An ophthalmic examination of 87% of the resident population of Nain, Labrador (latitude 56 degrees 33'N) was carried out, and the findings in 330 Inuit, 236 persons of mixed racial ancestry (Inuit-Caucasian) and 80 Caucasians were analysed. Legal blindness was present in 1.2% of the survey population, compared with 1.6% of the study population of the Arctic Ophthalmological Survey of 1970-71. Of the age group 52 to 85 years, 10.8% were legally blind, compared with 0.6% of the same age group surveyed in the Framingham (Massachusetts) Eye Study. The leading causes of unilateral or bilateral blin

Outcomes of inferior oblique muscle myectomy with tucking into Tenon's capsule, cohort prospective study.

Background: Inferior oblique muscle overaction (IOOA) is a prevalent ocular muscle motility disorder. This study aimed to investigate the outcomes and impact of IO muscle myectomy with and without tucking the proximal end of the muscle into Tenon's capsule in two surgical groups as a treatment for IOOA. Methods: Randomized cohort study that involves two surgical groups. A total of 33 patients (55 eyes). Patients were randomly divided into two groups: first group underwent myectomy without tucking the proximal end of the muscle, whereas the second group underwent myectomy with tucking of the pr

Results of combined resection-recession on a single rectus muscle for incomitant deviations-an alternative to the posterior fixation suture.

PURPOSE: To compare outcomes of combined resection and recession on previously unoperated single horizontal or vertical rectus muscles in consecutive adult patients with acquired incomitant deviations minimal in primary gaze and maximal in an eccentric gaze position. METHODS: The surgical goal was to decrease the incomitance by 50%, expand the field of binocular single vision, and relieve diplopia. Recessions for the maximal deviations were combined with smaller resections using hang-back, nonadjustable sutures. Pre- and postoperative incomitance was compared using the paired t test. RESULTS

Surgical results of patients with unilateral superior oblique palsy presenting with large hypertropias.

PURPOSE: Surgical management of superior oblique palsy (SOP) is challenging because of combined vertical, horizontal, and torsional misalignment. The authors report the surgical results of patients with large primary position hypertropias (> 20 prism diopters [PD]) due to unilateral SOP. METHODS: Criteria for success included correction of the anomalous head posture, primary position alignment between orthotropia and 6 PD of undercorrection, and no reoperation required for residual deviations in any direction of gaze. RESULTS: Forty-five patients met inclusion criteria. Mean preoperative ali

Germline Mosaicism for KIF21A Mutation (p.R954L) Mimicking Recessive Inheritance for Congenital Fibrosis of the Extraocular Muscles.

OBJECTIVE: To document the genotype for familial congenital fibrosis of the extraocular muscles (CFEOM) with apparent autosomal recessive inheritance. DESIGN: Interventional family study. PARTICIPANTS: Two affected siblings, 3 asymptomatic siblings, and their 2 asymptomatic parents. METHODS: Ophthalmologic examination and candidate gene analysis (KIF21A and PHOX2A from venous blood samples) of the 2 affected siblings and their parents; confirmatory testing for 3 available asymptomatic siblings. MAIN OUTCOME MEASURES: Significant clinical observations and results of gene testing. RESULTS:

Pathological eye-head coordination in roll: tonic ocular tilt reaction in mesencephalic and medullary lesions.

The fundamental pattern of coordinated eye-head roll motion is based upon utricular/saccular as well as vertical canal input, and mediated by the graviceptive pathways from the labyrinths via pontomedullary vestibular nuclei to the rostral midbrain tegmentum. The tonic bilateral graviceptive input stabilizes the eyes and head in the normal upright position. A unilateral lesion causes imbalance in vestibular tone in the roll plane which results in a tonic ipsiversive ocular tilt reaction (OTR). OTR, the triad of ipsilateral head tilt, skew deviation and ocular torsion, occurred as a tonic respo

Case Report: Ocular Tilt Reaction with Internuclear Ophthalmoplegia and Multiple Cranial Nerve Palsies.

SIGNIFICANCE: Ocular tilt reaction (OTR) is an abnormal eye-head postural reaction that consists of skew deviation, head tilt, and bilateral ocular torsion. Understanding of the pathway of the vestibulo-ocular reflex (VOR) is essential because this will help to localize the pathology. PURPOSE: The aim of this study was to report a case of OTR with contralateral internuclear ophthalmoplegia (INO) and fifth and seventh cranial nerve palsies. CASE REPORT: A 51-year-old gentleman with underlying diabetes mellitus presented with sudden onset of diplopia for 3 days. On examination, his visual acui

Sub-Tenon block does not provide superior postoperative analgesia vs intravenous fentanyl in pediatric squint surgery.

PURPOSE: We evaluated the efficacy of sub-Tenon block in decreasing perioperative pain, incidence of intraoperative oculocardiac reflex (OCR), and postoperative nausea and vomiting (PONV) in pediatric squint surgery. METHODS: A total of 67 children age 2-12 years, American Society of Anesthesiologists Physical Status 1 and 2, were randomized to receive either sub-Tenon block (ST) in the operative eye or 2 mcg/kg of intravenous fentanyl (F) for squint surgery after induction of general anesthesia in this double-blind study. Postoperative pain was measured by either modified Children's Hospital

Intraoperative monitoring of torsion to prevent vertical deviations during augmented vertical rectus transposition surgery.

BACKGROUND: Total transposition of the superior and inferior rectus muscle laterally, with augmentation sutures, may be complicated by induction of an undesirable vertical deviation. Induced vertical misalignment may be associated with changes in torsion. We have developed a simple method to monitor intraoperative torsion that may reduce the incidence of vertical deviations. METHODS: We reviewed consecutive cases of total abducens palsy or esotropic Duane syndrome treated with augmented lateral transposition of the superior and inferior rectus muscles, where the 12 o'clock and 6 o'clock intra

Clinical findings, orbital imaging, and intraoperative findings in patients with isolated inferior rectus muscle paresis or underaction.

PURPOSE: To present the clinical findings, orbital imaging, and intraoperative findings of patients with inferior rectus muscle underaction and to determine whether specific findings can help discern the underlying cause. METHODS: A retrospective observational study was performed on patients presenting with isolated inferior rectus muscle underaction between January 2007 and October 2011. Patient history, ocular motility, fundus torsion, Lancaster red-green plots, and radiographic findings were analyzed. For patients who had surgery, intraoperative findings also were considered. RESULTS: A t
