[Submental swelling in an Ethiopian man].

A 27-year-old man from Ethiopia had undergone an extraction of a molar in Libya ten months earlier, after which a submental swelling developed. For that reason, an oral and maxillofacial surgery department was consulted. Clinical examination showed a tender, firm-to-the-touch, non-mobile swelling with central ulceration. There was no pus drainage or evident lymphadenopathy. Intraoral examination showed a sanitized dentition with a healed diastema in the area of the extracted tooth. The patient suffered from fatigue, weight loss, night sweats, fever, headache and a cough. A recent chest X-ray, …

Multi-Drug-Resistant Tuberculosis Peritonitis: A Case Report.

The increasing incidence of tuberculosis raises concerns globally, impacting both developing and developed nations. Abdominal tuberculosis stands out as the most prevalent form of extrapulmonary tuberculosis. This case report details the diagnostic journey of a young male with abdominal TB complicated by concurrent HIV infection. The patient presented with night sweats and substantial weight loss, concurrently receiving a naive human immunodeficiency virus (HIV) diagnosis with an undetectable CD4 count. Imaging revealed abdominal lymphadenopathy concealing the pancreatic head while bronchoscop…

Bladder Paraganglioma Presenting as Post-Micturition Palpitations: A Case Report.

INTRODUCTION: Paraganglioma of the urinary bladder (PUB) is an extremely rare extra-adrenal catecholamine-secreting neuroendocrine tumor, accounting for only 0.05% of all bladder tumors and 1% of all pheochromocytomas. The clinical presentation of PUB can be diverse and challenging to diagnose. CASE PRESENTATION: This case report presents a 37-year-old man with post-micturition palpitation, headache, and sweating, with no history of hematuria or other irritative urinary symptoms. Ultrasound and contrast-enhanced computed tomography revealed a suspicious mass in the right posterolateral wall o…

Paroxysmal Hypertension in Congenital Hypothyroidism: An Unusual Case of Pheochromocytoma.

Pheochromocytomas are rare tumors arising from the chromaffin cells of the adrenal medulla that result in the secretion of excessive catecholamines. They are an uncommon cause of hypertension in the pediatric population. We present a case of a six-year-old male with hypertension, rapid weight loss, palpitations, excessive sweating, and vomiting. Their previous medical history was significant for congenital hypothyroidism, and upon the emergence of his symptoms, his levothyroxine dosing was reduced to half. Presenting symptoms and initial history were suspicious for thyroid-related disease; how…

Neonatal Abstinence Syndrome: A Case Series.

The increasing prevalence of substance misuse in modern culture is contributing to the growth in neonatal abstinence syndrome (NAS) cases in India. NAS can be challenging to diagnose due to nonspecific symptoms and maternal suppression of drug history. Only a few reports of NAS have been published from India. This is a case series of three newborns from India who all had symptoms like restlessness, high-pitched crying, excessive sweating, vigorous sucking, tremors, and diarrhea. The investigations did not lead to any conclusions. In the first case, the mother was treated with a combination of …

Skin Biopsy Targeting Senile Hemangiomas for the Diagnosis of Intravascular Large B-cell Lymphoma.

Intravascular large B-cell lymphoma (IVLBCL) is characterized by clinical presentations described as B symptoms, consisting of fever, night sweats, and weight loss. Intravascular lymphomas are more frequently diagnosed in elderly patients and are challenging to diagnose because of their nonspecific clinical presentation. Malignant lymphomas are recognized as the leading cause of fever of unknown origin. Although random skin biopsy is useful for diagnosing intravascular lymphomas, no standardized method for random skin biopsy has been established. Methods to avoid unnecessary skin biopsies and …

B-cell acute lymphoblastic leukemia associated with hypereosinophilia: a case report and brief literature review.

Few cases of B-cell acute lymphoblastic leukemia (B-ALL)-eosinophilia (ALL-eo) association have been reported. The lack or absence of blasts in the peripheral blood smear (PBS) along with urticarial rash, fever, arthralgia, myalgia, sweating, and dyspnea are common features of this condition. Herein, we report a 16-year-old male patient admitted to the emergency department with urticaria and generalized itching. PBS was examined, and eosinophils (90%) were seen in different fields. However, blast cells were not seen in PBS. In a bone marrow examination, terminal deoxynucleotidyl transferase-po…

Cartap Poisoning with Paroxysmal Supraventricular Tachycardia: A Case Report.

Cartap, a nereistoxin derivative from the marine annelid Lumbriconeresis heteropoda, is widely used as a pesticide, targeting pests like caterpillars. While the WHO classifies it as moderately hazardous, with a recommended daily intake of 0.05 mg/kg, human toxicity reports are limited. A 34-year-old female was admitted after consuming an unknown poison. She reported epigastric pain, burning sensations, sweating, vomiting, dyspnea, palpitations, and restlessness, compounded by alcohol intake. Initially treated for organophosphate poisoning, her cholinesterase level was 8.91. It was later confir…

Transient Intussusception Mimicking Acute Coronary Syndrome.

Intussusception in adults is rare and usually associated with organic lesions. However, in the current era of computed tomography (CT), cases of idiopathic and transient intussusceptions are being increasingly diagnosed. Herein, we present a case of ileocecal intussusception with symptoms mimicking those of acute coronary syndrome. A male patient in his 80s with a history of myocardial infarction presented to the emergency department with acute onset of severe precordial and epigastric pain, cold sweating, and vomiting. Coronary angiography did not reveal any significant new lesion, while abdo…

Long-term Effectiveness of Microwave Thermoablation in Persons With Residual Limb Hyperhidrosis: A Case Series.

Excessive sweating or focal hyperhidrosis (HH) of the residual limb in persons with amputations is thought to be a significant contributor to residual limb dermatoses, prosthesis dysfunction, and decreased quality of life. Treatment algorithms used for HH in persons without amputation do not effectively translate to residual limb HH, and research in this area is sparse. We discuss the long-term outcomes of three patients treated with microwave thermoablation (MT) on their residual limbs who had previously been unsuccessfully managed with topical antiperspirants and botulinum toxin injections. …

Neuroleptic Malignant Syndrome Improved with Intramuscular Administration of the Anticholinergic Agent, Biperiden.

Anticholinergic drugs, such as biperiden, benztropine, and diphenhydramine, were used for neuroleptic malignant syndrome (NMS) in the 1980s and 1990s. However, they have not been recommended for pharmacotherapy in NMS since 2000, as they may prevent a decrease in body temperature by suppressing sweating. However, whether anticholinergic drugs actually exacerbate NMS remains unclear. This study highlights the usefulness of anticholinergic drugs, which are no longer attracting attention as current pharmacological treatments for NMS. I treated four NMS patients using anticholinergic drugs. Two pa…

Immunomodulation through vaccination as a promising therapeutic strategy to mitigate malaria-related neurocognitive sequelae.

Malaria, an ancient infectious parasitic disease, is caused by protozoa of the genus Plasmodium, whose erythrocytic cycle is accompanied by fever, headache, sweating and chills and a systemic inflammation that can progress to severe forms of disease, including cerebral malaria. Approximately 25% of survivors of this syndrome develop sequelae that may include neurological, neurocognitive, behavioral alterations and poor school performance. Furthermore, some outcomes have also been recorded following episodes of non-severe malaria, which correspond to the most common clinical form of the disease…
