Intratendinous metal markers were used to study gap formation in 36 flexor digitorum profundus repairs during and after early controlled motion with a programme combining dynamic traction and passive flexion to all four digits. The mean gap 3 weeks after operation was 2.5 mm. The mean final gap was 2.6 mm. This study is a review of 127 hands in 100 patients in whom one or two FDS tendons were used to correct claw-hand deformity and/or loss of opposition of the thumb. In lumbrical replacement the results were graded as excellent in 16 hands (21%) and good in 43 hands (57%). For opponensplasty the results were excellent in 26 hands (32%) and good in 42 hands (51%). From 1972 to 1980, 153 patients with acute rupture of the Achilles tendon were treated operatively in the Trauma Department of Hannover Medical School, 127 of whom were assessed postoperatively according to the Trillat scale at an average of 9.8 years after injury. During the last few decades, the incidence of tendon ruptures has increased in civilized countries. Our material comprises 749 patients who had 832 tendon ruptures treated surgically between 1972 and 1985. There were no competitive athletes among the patients studied. There were 292 single ruptures of the Achilles tendon, 274 of the proximal biceps brachii, 113 of the extensor pollicis longus, and 70 of other tendons. Tendomyopathies (TM) comprise two subgroups. Both are caused by the irritation of nociceptors (IN). Secondary reconstruction of the torn anterior cruciate ligament has been accomplished by semitendinosus tenodesis in 78 athletes (average age, 20 years; range, 13 to 45 years old). This series was limited to patients who had isolated tears of the ligament or tears of the anterior cruciate ligament associated with tears of one or both menisci. The purpose of this paper is to describe the author's personal experiences in using the Patella Tendon Bearing Cast (P.T.B.) as described by Sarmiento (6), in the management of tibial shaft fractures.
