Surgery computer: a quiet revolution for general practice.

This paper shows how a well planned computer system can transform a practice's programme of prevention and surveillance, for long the Cinderella subjects in our Health Service. It was not necessary to employ specially trained staff to operate the computer and our receptionists adapted quickly and successfully: in fact, a new and interesting job coordinating the new routines has evolved which will include the preparation and presentation of a wide range of practice statistics. After initial hostility the computer has been accepted by the staff and its ability to increase the efficiency of the p

Gastroesophageal reflux in children: a clinical review.

The diagnosis of gastroesophageal reflux requires careful consideration of the patient's clinical history and initial evaluation of presenting symptoms. In cases where overt vomiting in noted, the initial evaluation should include a barium esophagram and upper gastrointestinal series. The diagnosis of gastroesophageal reflux may not be established by one test alone, but may require many tests to confirm the presence of significant reflux and to assess its sequellae. It is imperative to demonstrate that the extent and timing of GER is not merely physiologic. Continuous intraesophageal pH monito

Caring for adults with congenital cardiac disease: successes and challenges for 2007 and beyond.

Patients with congenital cardiac disease require lifelong medical care. Current challenges that face practitioners who care for adults with congenital heart disease include identifying the best location for procedures, which could be a children's hospital, an adult hospital, or a tertiary care facility; providing appropriate antenatal management of pregnant women with congenitally malformed hearts, and continuing this care in the peripartum period; and securing the infrastructure and expertise of the non-cardiac subspecialties, such as nephrology, hepatology, pulmonary medicine, and haematolog

Population health outcomes in Qatar 1990-2023: a systematic analysis for the Global Burden of Disease Study 2023.

BACKGROUND: Patterns of health and disease in Qatar have shifted substantially over the past three decades, reflecting demographic expansion, economic transition, and significant improvements in health system capacity. A comprehensive, up-to-date evaluation of disease burden is essential to guide priority-setting, resource allocation, and national health planning. However, no prior study has systematically assessed long-term trends in morbidity, mortality, and risk factors in Qatar using the most recent epidemiological data consistent with the Global Burden of Disease (GBD) Study framework. Robotic nephrectomy for central renal tumors with intraoperative evaluation of tumor histology.

Patients undergoing nephrectomy for central renal tumors suspicious for renal cell carcinoma (RCC) may carry a small risk of having transitional cell carcinoma (TCC) on final pathology, even in the absence of filling defects or abnormal cytology. We describe outcomes in such patients undergoing robotic nephrectomy for suspected RCC, with intraoperative specimen assessment to guide completion ureterectomy if TCC is present. Between September 2010 and August 2015, ten patients had central renal masses suspicious for RCC, which were not amenable to nephron-sparing surgery. Patients underwent a fo

Diagnostic Ureterorenoscopy Is Associated with Increased Intravesical Recurrence following Radical Nephroureterectomy in Upper Tract Urothelial Carcinoma.

Diagnostic ureterorenoscopy is powerful tool to confirm upper tract urothelial cancer (UTUC). However, URS and associated manipulation may be related to the risk of intravesical recurrence (IVR) following radical nephroureterectomy (RNU). We aimed to investigate whether preoperative ureterorenoscopy would increase IVR after RNU in patients with UTUC. We performed a retrospective analysis of 630 patients who had RNU with bladder cuff excision due to UTUC. Diagnostic URS was performed in 282 patients (44.7%). Patients were divided into two groups according to the URS. Survival analysis and multi

CLINICO-PATHOLOGICAL PATTERN, CLASSIFICATION AND STAGING OF URINARY BLADDER CARCINOMAS--A FIVE YEARS EXPERIENCE AT A TERTIARY CARE HOSPITAL IN CENTRAL PUNJAB.

BACKGROUND: In Pakistan, urinary bladder carcinoma is the 8th commonest malignancy while being the fourth commonest cancer in men. The relative occurrence of a particular histological type of bladder carcinoma depends on the clinical setting. Both grade and stage of these cancers are highly correlated with recurrence, progression and patient survival rates. METHODS: This cross- sectional study comprised of 122 patients with newly diagnosed operable primary bladder carcinomas who underwent cystoscopy associated transurethral resection of bladder tumour at the Urology Department of Punjab Emplo

Consensus document of the Neuroendocrinology area of the Spanish Society of Endocrinology and Nutrition on management of hypopituitarism during transition.

The transition period from child to adult represents a crucial phase in the growth process where multiple physical and psychosocial changes occur. It has been arbitrarily defined as the period extending from late puberty to full adult maturity (i.e., from mid to late teenage years until 6-7 years after achievement of final height). The aim of this guideline is to emphasize the importance of adequate hormone replacement during this period and to review reassessment of pituitary function. In patients with GH deficiency diagnosed in childhood, an attempt is made to answer when to retest GH secret

[Gastrointestinal involvement in systemic sclerosis. An underestimated complication].

Systemic sclerosis (SSc) belongs to the family of autoimmune connective tissue diseases and is still a challenge to every practicing physician. The disorder is characterized by progressing fibrosis of the skin and internal organs, abnormal activation of the immune system, and distinct changes in microcirculation. Although it is rare--with a prevalence of about 20:100000--patients need to be cared for in a daily setting. In general thickening of the skin is the first sign of the disease, so dermatologists are most frequently consulted first. Two subtypes exist, limited and diffuse forms. Both e

Optimal management of recurrent and metastatic upper tract urothelial carcinoma: Implications of intensity modulated radiation therapy.

BACKGROUND: Upper tract urothelial carcinoma (UTUC) is rare and the treatment for recurrent or metastatic UTUC is unclear. We evaluated the outcomes of salvage and palliative radiotherapy (RT) and prognostic factors in UTUC patients and find implications for salvage and palliative RT. METHODS: Between August 2006 and February 2021, 174 patients (median age, 68 years; range, 37-90) underwent salvage and palliative RT. Disease status at RT included initially diagnosed advanced disease (n = 8, 4.6%), local recurrence only (n = 56, 32.2%), distant metastasis only (n = 59, 33.9%), and local recurr

Upper ureteric transitional cell carcinoma, extending to the renal pelvis, presenting as duodenal obstruction.

A 61-year-old man presented with weight loss, dysphagia and vomiting. A barium swallow revealed a duodenal obstruction at D3. CT of the abdomen and pelvis showed a left upper ureteric tumour extending to the renal pelvis compressing the duodenum and causing left-sided hydronephrosis. Cystoscopy and left-sided ureteroscopy proved difficult and were unable to visualise or biopsy the mass, but a left ureteric stent was placed. Laparoscopic biopsy of the mass was completed and histology revealed transitional cell carcinoma. The patient went on to receive palliative chemotherapy, which relieved the

The prognostic impact of perioperative blood transfusion on survival in patients with bladder urothelial carcinoma treated with radical cystectomy.

PURPOSE: The aim of our study was to assess the influence of perioperative blood transfusion (PBT) on survival outcomes following radical cystectomy (RC) and pelvic lymph node dissection (PLND). MATERIALS AND METHODS: We reviewed and analyzed the clinical data of 432 patients who underwent RC for bladder cancer from 1991 to 2012. PBT was defined as the transfusion of allogeneic red blood cells during RC or postoperative hospitalization. RESULTS: Of all patients, 315 patients (72.9%) received PBT. On multivariate logistic regression analysis, female gender (p=0.015), a lower preoperative hemo

Patients with a negative cystoscopy and negative Nmp22® Bladderchek® test are at low risk of missed transitional cell carcinoma of the bladder: a prospective evaluation.

OBJECTIVES: Urine based tumor markers have uncertain utility in diagnosis or surveillance of patients with bladder cancer while cytology is commonly used. We evaluated whether cytology provides additional diagnostic information in patients with a negative NMP22® BladderChek® test (BladderChek) and negative cystoscopy. MATERIALS AND METHODS: We performed subset analyses of 2 large prospective multi-center databases evaluating BladderChek for UCB detection and surveillance. These cohorts were analyzed for presence of cancer and result of urine cytology in setting of a negative cystoscopy and ne

Palliative care in urology.

Urological malignancies, especially prostate cancer, are relatively common, but patients may live many years before eventually dying of the disease. Caring for these patients is an important role for urologists, although medical training often does not adequately prepare urologists for the palliative care of patients with advanced malignancies. Palliative care is no longer equated with end-of-life care, but rather integrated throughout illness, even when cure is impossible. This article focuses on the various palliative treatments available for the 3 most common urological malignancies: prosta

Clinical Presentation and Mortality Among Hospitalized People Living With HIV at a Tertiary Care Institute in Mumbai, India.

INTRODUCTION: Despite global advancements in antiretroviral treatment and the widespread adoption of universal antiretroviral therapy (ART) initiation, hospitalization among people living with HIV (PLHIV) infection remains a persistent concern in many settings. A substantial proportion of hospitalized patients present with low CD4 lymphocyte count, opportunistic infections, or complications that arise from treatment interruption or untreated HIV infections. With increasing age among this population, the presence of non-communicable diseases (NCDs) has also become more common, making clinical m

Phased rehabilitation nursing improves hip function, reduces complications, and enhances quality of life following total hip arthroplasty: A prospective quasi-experimental cohort study.

Total hip arthroplasty (THA) is the definitive treatment for advanced hip disease; however, postoperative rehabilitation remains challenging, particularly in elderly patients with multiple comorbidities. This study evaluated the effectiveness of phased rehabilitation (PR) nursing versus routine care on recovery outcomes following THA. This prospective quasi-experimental cohort study included 100 patients who underwent primary unilateral THA at Ankang People's Hospital from January 2023 to December 2024 and were assigned to either a study group (n = 50) receiving a 3-phase rehabilitation nursin

Sustainability in Radiology: Position Paper and Call to Action from ACR, AOSR, ASR, CAR, CIR, ESR, ESRNM, ISR, IS3R, RANZCR, and RSNA.

The urgency for climate action is recognized by international government and healthcare organizations, including the United Nations (UN) and World Health Organization (WHO). Climate change, biodiversity loss, and pollution negatively impact all life on earth. All populations are impacted but not equally; the most vulnerable are at highest risk, an inequity further exacerbated by differences in access to healthcare globally. The delivery of healthcare exacerbates the planetary health crisis through greenhouse gas emissions, largely due to combustion of fossil fuels for medical equipment product
