A first reported case of clear cell carcinoma associated with delayed extrusion of midurethral tape.

We present the first reported case of clear cell carcinoma associated with a midurethral tape (MUT), the possible hypotheses and the management pitfalls we encountered. We report a 58-year-old woman who presented with symptoms of urinary tract infection and acute retention of urine associated with vaginal tape exposure 10 years after placement of an inside-out transobturator tape. She subsequently had a partial transobturator tape excision and a diagnostic cystoscopy, which revealed inflammatory changes within the urethra. Postoperatively, her symptoms persisted and the vaginal epithelium heal…

Reoperative urethroplasty. A systematic review.

OBJECTIVES: Limited articles are published on reoperative urethroplasty outcomes. We sought to perform a systematic review of re-operative urethroplasty articles over the past fifteen years. METHODS: A systematic review was performed on PubMed using the search terms "Urethra" AND "Surgical Procedures, Operative" OR "Urethroplasty". RESULTS: Five articles out of 3,541 articles identified between 1998 and 2012 specifically addressed re-operative urethroplasty patients. A total of 212 patients were included in these five studies. Re-operative urethroplasty success rates ranged from 35% to 84%. …

Lichen planus and lichenoid reactions of the oral mucosa.

Oral lichenoid reactions represent a common end point in response to extrinsic agents (drugs, allergens), altered self-antigens, or superant … lts in conjunctival scarring; vaginal stenosis; vulvar destruction; and stricture of the esophagus, urethra, and external auditory meatus. Although the etiology of LP is idiopathic, oral lichenoid reactions may be caused by medications or exogenous agents such as cinnamates and other flavorings. [Secondary uroliths after surgical correction of urinary bladder extrophy].

The analysis of long-term results of complex surgical correction of urinary bladder extrophy in 83 patients has shown that 21 (25.3%) patients had concrements in the urethra. Multiple bacteriological tests detected urease-producing flora which was managed with specific antibacterial therapy. Two patients were exposed to extracorporeal shock-wave lithotripsy. Low-invasive lithotripsy was low effective and all the patients were operated. Chemical structure of the removed concrements was determined with x-ray difractometry. Recurrent concrements were found in 7 (33.3%) of 21 patients who had not …

Percutaneous sequential bacillus Calmette-Guèrin and mitomycin C for panurothelial carcinomatosis.

A 59 year old male presented with a 4 month history of lower urinary tract symptoms. Exhaustive urological investigations revealed papillary tumors and carcinoma in situ extending from the prostatic urethra, throughout the bladder, up both ureters and into the renal pelves. Tumors were resected where possible and then bacillus Calmette-Guèrin (BCG) and mitomycin C (MMC) were infused sequentially through bilateral nephrostomy tubes for a total of six BCG and three MMC instillations. Follow up 1 month post treatment demonstrated a complete response which persisted for 2 years. Then there appeare…

Pelvic fracture and associated urologic injuries.

Successful management of patients with major pelvic injuries requires a team approach including orthopedic, urologic, and trauma surgeons. Each unstable pelvic disruption must be treated aggressively to minimize complications and maximize long-term functional outcome. Commonly associated urologic injuries include injuries of the urethra, corpora cavernosa (penis), bladder, and bladder neck. Bladder injuries are usually extraperitoneal and result from shearing forces or direct laceration by a bone spicule. Posterior urethral injuries occur more commonly with vertically applied forces, which typ…

Tube cystostomy for treatment of obstructive urolithiasis in small ruminants.

Tube cystostomy was used to treat 13 goats and two sheep with obstructive urolithiasis. The cystostomy tube was intermittently occluded 3 to 4 days after placement to determine if urine could be voided through the urethra. If the animal showed no discomfort during urination after the cystostomy tube had been occluded for several days, the tube was removed. This procedure was successful in relieving urethral obstruction in 12 animals. The mean time until the animal could urinate freely and until the cystostomy tube was removed was 11.5 and 14.4 days respectively. Follow-up was available for 10 …

[Study on E-cadherin expression in relation to invasion and metastasis in urothelial cancer patients].

E-cadherin expression was investigated in 30 patients with urothelial cancer (bladder; 27, ureter; 2, urethra; 1) by an immunohistochemical method. Twelve out of 22 patients with strong and homogeneous expression of E-cadherin (preserved) had grade 1 tumors, while all 8 patients with heterogeneous, weak and homogeneous, or lost expression of E-cadherin (reduced) had grade 2 or grade 3 tumors. Only 2 out of 22 patients with preserved expression had invasive tumors (> or = pT2), whereas 6 out of 8 patients with reduced expression had invasive tumors (p = 0.002). Three out of 4 patients with meta…

Endoluminal US: experiments with nonvascular uses in animals.

Catheters containing miniature ultrasound (US) transducers, originally developed for intravascular evaluation of plaque, were used in a series of in vivo and in vitro animal experiments to image a variety of nonvascular lumina. Measurements of the wall thickness and echotexture of the urethra, urinary bladder, ureter, renal pelvis, bile ducts, small bowel, fallopian tubes, and uterus were carried out. Close correlation between the US images and actual anatomic specimens was obtained. Structures outside of the lumen were identified and confirmed with both direct visualization and radiographic l…

Conceptualizing the "High" Bladder Neck - Anatomic Rabbit Hole or Functional Gold Mine?

Describing a "high bladder neck" is common practice, yet it is not a standardized entity, and it lacks an evidence base in diagnostics and therapy. Recent systematic reviews report few trials, yet incision of the bladder neck remains a first-line therapy. The concept insinuates a basis for bladder outlet obstruction, so it is during voiding that such an issue should be assessed. For cystourethroscopy, the urinary tract is in the storage phase. A pressure-flow study in videourodynamics can visualize when the bladder neck is deflected into the proximal prostatic urethra by the force of bladder c…

Urogenital dysfunction in neurological diseases.

Neural damage at any level of the neuraxis can lead to urogenital dysfunction, involving the lower urinary tract (LUT) and/or the sexual organs. The LUT consists of the bladder and the urethra (plus the prostate in males); LUT dysfunction can manifest as an underactive bladder, leading to urinary retention, or as an overactive bladder, leading to urinary incontinence. Manifestations of sexual dysfunction include loss of libido and dysfunction of erection, ejaculation and orgasm. In this Review, we address the physiology and innervation of the LUT and the sexual organs and discuss various aspec…

Anterior Urethral Paraganglioma: An Extremely Rare Case Report and Literature Review.

INTRODUCTION: Urinary tract paragangliomas account for less than 1% of all paragangliomas and are most commonly seen in the bladder, with the prostate being the second most common localization. However, urethral paragangliomas are extremely rare. CASE PRESENTATION: A 52-year-old male patient was admitted to the urology clinic with lower urinary tract symptoms (LUTS), including poor and intermittent stream, straining, frequency, and nocturia. A pedunculated polypoid lesion, approximately 1 cm in size, was detected at the level of the bulbar urethra and was excised from its stalk using cold cup…

[Urethral reconstruction].

A urethral stricture is an abnormal narrowing of the urethra due to spongiofibrosis of the urethral mucosa and the underlying corpus spongiosum. The diagnostics include uroflowmetry, sonography and radiology. For penile strictures the success rate of endoscopic treatment is low. Therefore, urethroplasty should always be performed, preferably using oral mucosa. Depending on the complexity, reconstruction must be carried out in one or multiple stages. For short bulbous strictures endoscopic treatment can primarily be carried out. In the case of recurrence urethroplasty should be carried out. The…

[New WHO classification 2022: urinary bladder cancer].

The new World Health Organization (WHO) classification of urogenital tumors is still primarily based on anatomic location, but is also a hierarchical taxonomic classification without separate chapters for tumors of the upper urinary tract and the urethra. It clarifies aspects regarding grading and noninvasive entities. It consolidates the use of the Paris system for urinary cytology as well as various subtypes/special types of neoplasms, and incorporates general concepts of the 5th edition of the WHO blue book. In addition to mesenchymal tumors, well-differentiated neuroendocrine tumors and ne…

Dorsal oral mucosa graft urethroplasty for female urethral stricture reconstruction: A narrative review.

Female urethral stricture is currently a challenging situation. In general, urethra dilatation can be selected for treatment, but the complications and high recurrence rate urge doctors to consider other treatments. Recently, dorsal oral mucosa graft urethroplasty is concerned by more and more surgeons, but there are not enough reports so far. A comprehensive search of dorsal oral mucosa graft urethroplasty was performed. According to the existing literature, there are applications of buccal mucosa and lingual mucosa, and compared with other kinds of grafts, the success rate is higher. However…

Condyloma acuminatum of the urinary bladder with underlying squamous cell carcinoma: a case report.

BACKGROUND: Condyloma acuminatum is a rare finding of the urinary bladder. There are many morphological variants that exist. It has been previously reported that some of these variants were found to be associated with subsequent or concurrent squamous cell carcinoma. However, there are limited cases that describe this underlying malignancy found in patients with bladder condyloma. CASE PRESENTATION: A 38-year-old African American female presented with condyloma acuminatum of the urinary bladder and underlying squamous cell carcinoma, which also invaded the neighboring uterus, urethra, and ant…

Dioctophimosis: A Parasitic Zoonosis of Public Health Importance.

Dioctophyme renale, the giant kidney worm, is a renal nematode from domestic and wild mammals that has zoonotic potential. In humans, dioctophimosis has been reported in several countries, mainly on the Asian continent, totaling more than 40 cases, which describe the parasite mainly infecting the kidneys, bladder, urethra and skin. Infection in animals and humans is related to the ingestion of the infective larva (L3) present in the aquatic oligochaete annelid (mandatory intermediate host) or fish and anurans (facultative paratenic hosts). Thus, the infection is related to the habit of drinkin…
