Diurnal variation of immunosuppressive acidic protein.

Diurnal variations of serum immunosuppressive acidic protein (IAP) were studied in four healthy individuals and in four patients with complete type Behçet's disease with active uveitis. The circadian rhythm of the serum level of the protein was recognized in both groups. The peak value was at 1800 hours in the healthy subjects and at 2200 hours in those with Behçet's disease. Although the exact mechanism underlying the rhythmic variation in this clinical marker of acute phase reaction is unknown, the close interaction between the immune system and the rhythmicity of corticosteroids should be c…

Choroiditis compatible with the histopathologic diagnosis of sympathetic ophthalmia following cyclocryotherapy of neovascular glaucoma.

A 44-year-old man had a blind painful right eye secondary to neovascular glaucoma. Cyclocryotherapy was performed because medical therapy was ineffective in lowering the intraocular pressure or reducing the pain. The eye became comfortable, but 5 months later vision decreased in the left eye. Examination revealed a marked posterior uveitis suggestive of sympathetic ophthalmia. Enucleation of the right eye and corticosteroid therapy were effective in eliminating inflammation in the left eye. Histopathologic examination of the exciting right eye revealed classic signs of sympathetic ophthalmia. …

Trabeculodialysis for inflammatory glaucoma in children and young adults.

A modified goniotomy (trabeculodialysis) was performed on 30 eyes of 23 patients with secondary glaucoma due to chronic anterior uveitis. All but two patients also suffered from juvenile chronic arthritis (Still's disease, juvenile rheumatoid arthritis). Trabeculodialysis was unsuccessful in lowering intraocular pressure to below 21 mmHg in 12 (40%) of eyes and successful in 18 (60%) of eyes. Of the 18 successful cases, 5 required no additional medication, but in 13 cases the intraocular pressure could only be maintained at a normal level by concomitant anti-glaucoma therapy. The presence of a…

Role of tromaril--a new anti-inflammatory drug in eye diseases.

In this clinical study, the role of enfenamic acid (Tromaril), a new anti-inflammatory and nonsteroidal drug, on various eye diseases such as trauma, on inflammatory diseases such as uveitis, and after cataract surgery was evaluated. The drug was administered to 30 patients (group A) while 29 patients (group B) were given a placebo. Twenty-one patients showed good response, and of these, 15 patients (76%) belonged to group A, to whom Tromaril had been administered. Of the patients who were given Tromaril, none showed any side effects in general. It can be concluded that Tromaril is a strong an…

Complications of herpes zoster ophthalmicus.

Of 86 patients with herpes zoster ophthalmicus seen at the Mayo Clinic, Rochester, Minn, from 1975 to 1980, 61 had some form of ocular involvement. Corneal disease was seen in 47, uveitis in 37, postherpetic neuralgia in 15, scleritis in three, and ocular motor palsies in three. No case of optic nerve or retinal involvement was found. Of serious concern were four patients with neurologic complications, including two with contralateral hemiplegia and two with segmental cerebral arteritis. Because the neurologic complications occur several months after the episode of herpes zoster ophthalmicus, …

Ocular findings in mycosis fungoides.

Thirty consecutive patients with biopsy-proved mycosis fungoides were examined ophthalmologically. Specific ocular changes, judged directly related to the mycosis fungoides, were found in 11 of the 30 subjects, usually in the late plaque or tumor stages of the disease. Tumors, especially involving the lids, were the most common ophthalmic finding. Keratitis, uveitis, and optic atrophy were also described. A review of the literature is given, and changes found in patients in this series correlate well with ophthalmic findings presented in diverse isolated case reports. It seems that the frequen…

Classical rheumatoid arthritis in a patient with Reiter's syndrome.

A 44 year old white male presented with sterile urethritis, anterior uveitis, painless oral ulcers, low back pain, and synovitis of the knees and left ankle. Unilateral sacroiliitis and limited spondylitis further supported a diagnosis of Reiter's syndrome. Serum rheumatoid factor was present. The patient subsequently developed morning stiffness, symmetrical polyarthritis of the hands and wrists, and an olecranon subcutaneous nodule. Histology of the nodule and synovium was consistent with rheumatoid arthritis. Histocompatibility antigen typing established the presence of HLA B27. The occurren…

Acute-Onset Postoperative Herpetic Endophthalmitis: A Case Report.

Herpes simplex virus uveitis without corneal reactivation is more frequent than previously thought. Although herpes simplex virus has been implicated as a cause of postoperative uveitis and endophthalmitis, it has not been reported as a cause of acute postoperative endophthalmitis within the early postoperative period, specifically within one week following cataract extraction. A 55-year-old man with vascularized irregular central disc-shaped stromal corneal opacity with complicated cataracts underwent cataract surgery. Intraoperatively, there was posterior capsular rent, requiring anterior vi…
