Blisters, ulceration and autonomic neuropathy in carpal tunnel syndrome.

We describe three patients with trophic ulceration and blistering of the fingertips associated with carpal tunnel syndrome. One of the patients also had non-insulin-dependent diabetes mellitus. Autonomic neuropathy distal to the carpal tunnel was probably present in all subjects at the time of presentation; in the patient with recent symptoms the skin was warm, and sweating was virtually absent, whilst the other two patients described cold skin, consistent with prolonged autonomic neuropathy.

Heart rate variability. Frequency domain analysis.

Experience with frequency domain analysis over the past two decades strongly suggests that it represents a unique, noninvasive tool for achieving a more precise assessment of autonomic function in both the experimental and clinical settings. Available studies indicate that the significance of the HF component is far better understood than that of the lower frequency components. In general, it is considered to reflect vagal activity, and because it is readily manipulated pharmacologically, is used as a an index of that activity. However, some caution is required because this parameter also is s

Autonomic neurone degeneration in equine dysautonomia (grass sickness).

Histological investigations were undertaken on four sympathetic autonomic ganglia and on the myenteric and sub-mucosal plexuses of the jejunum in healthy animals, in naturally occurring cases of acute, sub-acute and chronic equine dysautonomia and in ponies in which neuronal damage had been induced by the injection of acute grass sickness sera. The degree of neuronal damage is related to the type of dysautonomia. The coeliac-mesenteric ganglion reacts differently from other ganglia and is less severely damaged in cases of short duration. Extensive experimentally induced damage to the coeliac-m

Cardiovascular complications in renal failure.

Cardiovascular diseases are a leading cause of death in end-stage renal disease (ESRD) largely as a result of the progressively increasing age of ESRD patients and the broad constellation of uremia-associated factors that can adversely affect cardiac function. Hypertension, one of the leading causes of renal failure, is a major culprit in this process, causing left ventricular hypertrophy, cardiac chamber dilation, increased left ventricular wall stress, redistribution of coronary blood flow, reduced coronary artery vasodilator reserve, ischemia, myocardial fibrosis, heart failure, and arrhyth

Sleep apneas, convulsive syncopes and autonomic impairment in type I Arnold-Chiari malformation.

A 51-year-old man with type I Arnold-Chiari malformation presented with severe autonomic impairment. Polysomnography disclosed central and obstructive sleep apneas. During REM sleep apneas were accompanied by hypotension and led to convulsive nonepileptic attacks. Marked blood pressure oscillations and central apneas persisted after tracheostomy.

Pressor effect of inhaled ergotamine in orthostatic hypotension.

Treatment of orthostatic hypotension due to autonomic failure frequently necessitates use of pressor agents. Because venous pooling contributes significantly to this disorder, the venoconstrictive properties of ergotamine offer theoretical advantages over pure arteriolar pressor agents. However, the low and erratic bioavailability of oral preparations has hindered the use of ergotamine. Accordingly, the efficacy of inhaled ergotamine tartrate (1 puff, 0.36 mg) was compared to placebo in 8 patients with severe autonomic failure. Blood pressure was monitored in the seated position with an automa

Ocular autonomic nerve function in Lambert-Eaton myasthenic syndrome.

Autonomic innervation to the eye and ocular adnexae was assessed in seven patients with Lambert-Eaton Myasthenic Syndrome and 50 age-matched control subjects. Pupil responses to light were abnormal in 69% of LEMS patients, compared with 18% of the control group. Reflex tear production--a screening test for parasympathetic innervation to the lacrimal gland--was below the accepted normal limit in 69% of LEMS patients. Parasympathetic and sympathetic denervation hypersensitivity of the iris musculature were individually present in 57% of LEMS patients, compared with 6% of control subjects. The as

[Nosological correlation between autonomic failure with Lewy bodies and Parkinson's disease--comparative assessment of clinical features in both diseases].

Autonomic failure with Lewy bodies (AF-LB) was first described by Fichefet et al. in 1965, and more than ten cases have been reported to date. AF-LB and Parkinson's disease (PD) share the neuropathological findings characterized by widely distributed Lewy bodies in the central nervous system including the substantia nigra and locus coeruleus. However, clinical manifestations of AF-LB are far different from PD in which autonomic dysfunction, if present, is not a predominant feature. In the present study, clinical features were comparatively analysed in AF-LB and PD to investigate the nosologica

Trends and perspectives of the biological prophylaxis of silicosis.

Based on earlier paper in this journal, as well as on new data concerning factors predetermining individual susceptibility to silicosis, we propose a comprehensive system of recommendations aimed at increasing the working population's resistance against the harmful action of silica dust. These recommendations consider, on one hand, the criteria of possible selection for employment in dusty industries of individuals who do not possess characteristics making them especially sensitive to this harmful action (collective biological prophylaxis), and on the other hand, the influences that could dimi

"Mild" cluster headache(?).

Three cases of what could be considered "mild" cluster headache have been described. All three patients were generally able to carry out their work during attacks; all three were men and had unilateral headache with the predominant pain in the ocular region. Relatively few symptoms and signs indicated autonomic system involvement, but at least tearing was invariably present on the symptomatic side. The bouts were generally short-lasting in two of the patients (and partly in the third one), fitting the pattern of "mini-bouts". Thus, in one of the cases four of the five major criteria (male sex,

Emotional fainting and the psychophysiologic response to blood and injury: autonomic mechanisms and coping strategies.

This experiment was designed to examine the psychophysiologic response pattern associated with emotional fainting. A survey was carried out among students, from which 30 volunteers who reported that they felt faint at the sight of blood or injury were recruited, together with 26 nonfainters. Blood pressure, heart rate, and respiration rate were monitored while subjects viewed a film depicting open-heart surgery and a neutral control film. Parasympathetic cardiac control was indexed by respiratory sinus arrhythmia (RSA), and coping was assessed by questionnaire. Responders (n = 17, group FPOS)

[Psycho- and neurovegetative effects on the ECG of children and adolescents].

In six patients between 10 and 21 years of age vegetative function disorders were diagnosed. All of them showed an inversion of T in lead II and/or III and in V4 to V6. After exercise-testing the ECG signs normalized. Other clinical investigations, echocardiography, and in one case heart-catheterization revealed no pathological findings. It is necessary to distinguish these rare ECG findings from pathological ECG changes.

A case of hereditary motor and sensory neuropathy type III with a decrease in unmyelinated fibers.

We report a 3-year-old girl with hereditary motor and sensory neuropathy type III with a decrease in unmyelinated fibers. There have been few reports of such cases. The present findings suggest the possibility that the primary lesion in this disease is in the axons. We consider that more attention should be paid to the changes in unmyelinated fibers and axons in further studies on this disease.

[Intestinal pseudo-obstruction and cytomegalovirus infection of myenteric plexuses].

The authors report an intestinal pseudo-obstruction syndrome occurring in a 2 month-old boy, with acquired major and persistent abdominal distension, leading to total parenteral nutrition. Rectal biopsy revealed hypoganglionosis with thickening of nerve processes and intranuclear inclusions in some neurons. Suspected cytomegalovirus infection was confirmed by viruria and specific IgM and IgG antibodies.

Alterations of pancreatic islet size and growth following substantia nigra lesions in the rat.

Lesions of certain autonomic centers in the brain are known to alter feeding behavior, body weight, and influence the morphology and function of the pancreatic islets. Because marked reductions of food intake and body weight have been reported following damage to the substantia nigra (SN), we investigated the role of this brain area as a potential regulator of the endocrine pancreas. Rats were given bilateral SN lesions using the neurotoxin 6-hydroxydopamine (6-OHDA) at a dose of either 6 or 12 micrograms/hemisphere. Animals given sham lesions served as controls. Both the control and experimen

Distribution of sympathetic nerve endings within the rabbit carotid body: a histochemical and ultrastructural study.

The distribution of sympathetic (noradrenergic) nerves within the rabbit carotid body was investigated by fluorescence microscopy and electron microscopy. With the Falck-Hillarp method, fluorescent nerve fibres were found associated with blood vessels and type I/type II cell groups. After the injection of 6-hydroxydopamine, degenerating nerve profiles were found near blood vessels, near type I/type II cell groups and in contact with type II cells. Following the administration of [3H]noradrenaline, labelled nerve profiles were observed in close association with blood vessels and around cell gro

Stridor and obstructive sleep apnea in Shy-Drager syndrome treated by laryngofissure and cord lateralization.

A patient with obstructive sleep apnea due to Shy-Drager syndrome, who has been successfully managed by cord lateralization, is presented. This has allowed successful rehabilitation of the patient without prejudicing subsequent surgery, and appears to be an effective procedure in this rare condition.
