Construction and validation of a machine learning model to predict the sarcopenic obesity population.

Sarcopenic obesity (SO) imposes a heavy medical burden on both individuals and society, highlighting the importance of early identification and timely intervention. This study aims to construct a clinical prediction model based on machine learning (ML) methods to assist in the early screening of SO. Data from the National Health and Nutrition Survey from 2011 to 2018 were used for model development. The included data were randomly divided into a training set and an internal test set with a ratio of 7:3. Eight ML methods - logistic regression, decision tree, neural network, K-nearest neighbor,

Frontline immunotherapy with response-guided subsequent treatment (FIRST) in cutaneous squamous cell carcinoma: a Bayesian causal analysis of dose intensity, early benefit, and treatment de-escalation.

BACKGROUND: Programmed death receptor 1 (PD-1) blockade produces high response rates in resectable and locally advanced cutaneous squamous cell carcinoma (CSCC), but how many doses are needed and whether surgery or radiation after immunotherapy (consolidation) adds benefit in deep responders (patients with substantial clinical responses) remain unclear. Observational inference is challenging because treatment decisions are response-guided, doses accumulate over time, and treatment selection depends on patient characteristics that also affect outcomes. METHODS: We conducted a retrospective coh

Temsirolimus safety evaluation: real-world adverse event analysis from the FDA adverse event reporting system database.

Temsirolimus is a mammalian target of rapamycin inhibitor primarily used to treat not only various types of renal carcinoma, but also various types of lymphomas and other tumors. However, its real-world safety profile remains inadequately characterized. By analyzing the Food and Drug Administration Adverse Event Reporting System (FAERS) database, we identified risk signals associated with temsirolimus adverse reactions and gained valuable insights for clinical decision-making and risk management. We extracted temsirolimus-related reports of adverse events (AEs) from FAERS across 68 quarters (Q

Leveraging perturbations to infer the population dynamics of human rhinovirus and interaction of influenza A virus.

Many respiratory pathogens co-circulate within human populations. Yet, how pathogen community structure shapes the dynamics of infectious diseases remains poorly understood. At the population level, investigating polymicrobial dynamics, with potential underlying competitive or cooperative interactions, is challenging, because of confounding factors such as differing seasonality. This is particularly true for endemic pathogens which typically exhibit stable periodic dynamics. Their disruption due to the implementation of non-pharmaceutical interventions during the COVID-19 pandemic thus represe

Protecting the brain from post-stroke cognitive impairment and dementia with multimodal exercise training: study protocol for a Bayesian adaptive trial (PROTECT).

INTRODUCTION: Stroke triggers acute vascular and inflammatory mechanisms that predispose the brain to rapid neurodegeneration. Up to 52% of stroke survivors develop cognitive impairment within 6 months and 20% receive a clinical diagnosis of dementia within 5 years. The subacute phase (<6 months) represents a critical window in which the brain may be most responsive to neuroprotective interventions. Multimodal aerobic and resistance training improves cognition in chronic stroke, but whether it improves cognition, neuroimaging markers and blood biomarkers of dementia risk when delivered during

Computational modelling and neural correlates of reinforcement learning following three-week escitalopram: a double-blind, placebo-controlled semi-randomised study.

Reinforcement learning is a fundamental aspect of adaptive behaviour, since it involves the acquisition and updating of associations between actions and their outcomes based on the rewarding or punishing consequences. Acute experimental manipulations of serotonin have provided compelling evidence for its role in reinforcement learning. However, it remains unknown how more chronic manipulation of serotonin, which holds greater clinical relevance, affects reinforcement learning and the underlying neural mechanisms. Consequently, we aimed to investigate the effect of a three-week administration o

A semi-empirical Bayes approach for calibrating weak instrumental bias in sex-specific Mendelian randomization studies.

Strong sex differences exist in sleep phenotypes and also cardiovascular diseases (CVDs). However, sex-specific causal effects of sleep phenotypes on CVD-related outcomes have not been thoroughly examined. Mendelian randomization (MR) analysis is a useful approach for estimating the causal effect of a risk factor on an outcome of interest when interventional studies are not available. We first conducted sex-specific genome-wide association studies (GWASs) for suboptimal-sleep phenotypes (insomnia, obstructive sleep apnea [OSA], short and long sleep durations, and excessive daytime sleepiness)

Model-Based Prediction of Clinically Relevant Thrombocytopenia after Allogeneic Hematopoietic Stem Cell Transplantation.

Platelet reconstitution after allogeneic hematopoietic cell transplantation (allo-HCT) is heterogeneous and influenced by various patient- and transplantation-related factors, associated with poor prognoses for poor graft function (PGF) and isolated thrombocytopenia. Tailored interventions could improve the outcome of patients with PGF and post-HCT thrombocytopenia. To provide individual predictions of 180-day platelet counts from early phase data, we developed a model of long-term platelet reconstitution after allo-HCT. A large cohort (n = 1949) of adult patients undergoing their first allo-H

The Influence of Dexmedetomidine Added to Ropivacaine on Its Pharmacokinetic and Pharmacodynamic Effects During Lumbar Plexus Block.

BACKGROUND: While ropivacaine-dexmedetomidine coadministration is widely used in surgical anesthesia, its pharmacokinetic effects in lumbar plexus blocks (LPB) remain uncharacterized. We therefore conducted a prospective randomized trial to evaluate dexmedetomidine's modulation of ropivacaine's pharmacokinetics and pharmacodynamics during unilateral lower limb surgery. Additionally, this study seeks to establish a dosing regimen for ropivacaine in combination with dexmedetomidine for LPB. METHODS: Patients undergoing unilateral lower limb surgery were randomly allocated to 1 of 3 groups using

Bayesian Counterfactual Machine Learning Individualizes Radiation Modality Selection to Mitigate Immunosuppression.

PURPOSE: Lymphocytes play critical roles in cancer immunity and tumor surveillance. Radiation-induced lymphopenia (RIL) is a common side effect observed in patients with cancer undergoing chemoradiation therapy (CRT), leading to impaired immunity and worse clinical outcomes. Although proton beam therapy (PBT) has been suggested to reduce RIL risk compared with intensity-modulated radiation therapy (IMRT), this study used Bayesian counterfactual machine learning to identify distinct patient profiles and inform personalized radiation modality choice. METHODS: A novel Bayesian causal inferential
