In 15 pigs an auxiliary liver was transplanted and cholescintigraphy with 99mTc-diethyl-IDA was performed to study the function of the livers separately. Serial scintigraphy of the recipient and donor liver was performed on days 3, 8 and 18 after transplantation and time-activity curves over 1 h after injection of the radio-tracer were generated for each liver. Fasting serum levels of total 3 alpha-hydroxy bile acids were measured in 163 consecutive patients, who clinically were suspected of having a hepatobiliary disorder. In order to assess the role of 99mTc-iminodiacetic acid (IDA) cholescintigraphy in evaluating postoperative patients, a total of 213 studies were performed in 189 patients over a 3-year time period. Of these, 130 studies were obtained in 125 cases with signs and/or symptoms suggesting postcholecystectomy syndrome. The present trend towards a more aggressive operative policy in acute cholecystitis has led to a search for more accurate methods of diagnosis. A possible contender in this field is hepatobiliary isotope scanning. Cushing's syndrome preceded the appearance of the overt Zollinger-Ellison syndrome by 2 years and was treated by bilateral adrenalectomy. Technetium 99m-labelled hepatobiliary imaging agents provide high resolution images not previously possible with rose bengal labelled with iodine 131. This has prompted a re-examination of the utility of these radioactive pharmaceuticals. In a consecutive series of 174 patients the biliary and pancreatic ducts were demonstrated by endoscopic retrograde cholangio-pancreatography (ERCP). In 84 patients with normal duct systems, juxtapapillary diverticula were found in 5 patients (6%).
