Accuracy of freehand pedicle screws versus lateral mass screws in the subaxial cervical spine.

STUDY DESIGN: Radiographic comparative study with prospectively collected data. OBJECTIVES: To assess the accuracy of subaxial cervical pedicle screw (CPS) placement with freehand technique compared to lateral mass screws (LMS). The freehand cervical pedicle screw insertion technique guided by intraoperative lateral C-arm imaging has been shown to be both safe and effective. However, no study has performed a 100% audit of this technique using pre- and postoperative computed tomography (CT) to determine its true accuracy, as well as its reduction capability of CPS and LMS instrumentation. Evaluation of the Efficacy of Titanium Mesh Cages with Posterior C1 Lateral Mass and C2 Pedicle Screw Fixation in Patients with Atlantoaxial Instability.

BACKGROUND: Atlantoaxial fusion is a surgical technique that is performed for the treatment of atlantoaxial instability. The standard surgical procedure is fixation of the atlantoaxial complex via a C1 lateral mass and C2 pedicle screw with posterior wiring. Bone grafting material may still be biomechanically suboptimal, such as autologous bone obtained from osteoporotic patients, and may result in collapse and loosening of posterior wiring. METHODS: Fourteen patients with C1 lateral mass and C2 pedicle fixation as a result of atlantoaxial instability were included in this study. All patients

Proximal femoral bionic nail-a novel internal fixation system for the treatment of femoral neck fractures: a finite element analysis.

Introduction: Currently, cannulated screws (CSs) and dynamic hip screws (DHSs) are widely used for the treatment of femoral neck fractures, but the postoperative complications associated with these internal fixations remain high. In response to this challenge, our team proposes a new approach involving triangular-supported fixation and the development of the proximal femoral bionic nail (PFBN). The primary objective of this study is to investigate the biomechanical differences among CSs, DHSs, and the PFBN in their capacity to stabilize femoral neck fractures. Methods: A normal proximal femur

Comprehensive functional outcome analysis and importance of bone remodelling on personalized cranioplasty treatment using Poly(methyl methacrylate) bone flaps.

Cranioplasty involves the surgical reconstruction of cranial defects arising as a result of various factors, including decompressive craniectomy, cranial malformations, and brain injury due to road traffic accidents. Most of the modern decompressive craniectomies (DC) warrant a future cranioplasty surgery within 6-36 months. The conventional process of capturing the defect impression and polymethyl methacrylate (PMMA) flap fabrication results in a misfit or misalignment at the site of implantation. Equally, the intra-operative graft preparation is arduous and can result in a longer surgical ti

Intermittent internal fixation with a locking plate to preserve epiphyseal growth function during limb-salvage surgery in a child with osteosarcoma of the distal femur: a case report.

Limb shortening is a problem associated with surgery for osteosarcoma of the lower extremity in adolescents, as the tumors frequently occur near the epiphysis. Herein we report the use of a less invasive stabilization system (LISS) and an intermittent fixation method to preserve the growth function of epiphysis in an 11-year-old patient with an osteosarcoma of the distal femur. The 11-year-old male presented with left knee enlargement and pain for 2 weeks, and magnetic resonance imaging (MRI) and biopsy were consistent with osteosarcoma of the left distal femur. After preoperative chemotherapy,

[Plea for accelerated rehabilitation after ligament plasty of the knee by a bone-patellar tendon-bone graft].

Knee rehabilitation after ACL repair with bone-tendon-bone graft is still controversial. While there was a tendency to protect the graft and the donor site in the eighties, actual tendency is to propose more aggressive, so called accelerated rehabilitation protocol. An extensive analysis of the literature shows that this accelerated rehabilitation is justified because of histologic, biomechanic, surgical and clinical arguments. This accelerated rehabilitation is based on seven reasons, at least: 1) the necrosis of the graft, initially observed in animals, does not seem to be as important in hu

Aspergillus osteomyelitis and lymphangitis in immunocompromised patient after toenail clipping.

The development of new percutaneous treatment techniques using a balloon for the reduction and cement for the stabilization for tibial plateau fractures (TPF) are promising. The biomechanical changes brought by the cement in the periarticular fracture are unknown. The objective of this study was to provide elements of understanding of the bone behavior in an epiphyseal fracture treated with cementoplasty and to define the modifications brought about by the presence of this cement in the bone from both an architectural and biomechanical point of view. Computational biomechanical analysis of medial displacement calcaneal osteotomy placement and orientation.

Medial displacement calcaneal osteotomy (MDCO) is commonly used to correct valgus hindfoot alignment in progressive collapsing foot deformity, but the biomechanical effects of osteotomy placement and Chevron apex geometry within the surgical safe zone remain unclear. This study evaluated how oblique cut position and Chevron apex orientation affect construct stiffness and stress distribution using patient-specific finite element models. Weight-bearing CT-derived models of the distal tibia, talus, and calcaneus were generated for seven patients. Nine MDCO configurations were simulated per patien
