Acute calcific tendinitis of the flexor carpi ulnaris causing acute compressive neuropathy of the ulnar nerve: a case report.

This study reports a case of acute calcific tendinitis of the flexor carpi ulnaris in a 64-year-old woman. She presented with symptoms of acute ulnar nerve compression mimicking a volar compartment syndrome. Owing to rapidly progressive symptoms, emergency surgical exploration was carried out. Intra-operatively a large mass of calcium phosphate carbonate was noted in association with the flexor carpi ulnaris near its insertion at the wrist compressing the ulnar nerve and artery in Guyon's canal. Postoperatively the patient had complete resolution of symptoms. Conservative management with non-s

[Ethmoidal mucocele after transpalpebral bony orbital decompression].

We report a case of a late ethmoidal mucocele occurring after transpalpebral bony orbital decompression. A 39-year-old man presented with a recurrence of a right-sided proptosis without signs of orbital inflammation. The patient had undergone bilateral transpalpebral bony orbital decompression for dysthyroid orbitopathy 2 years prior. Orbital CT scan showed a large mucocele in the supero-lateral right ethmoidal sinus with lateral extension to the medial rectus. The patient was therefore referred to an ear, nose and throat (ENT) surgeon, who performed an anterior ethmoidectomy with marsupializa

Cerebellopontine angle endodermal cyst presenting with hemifacial spasm.

Intracranial endodermal cysts presenting with hemifacial spasm (HFS) are extremely rare. We report a right cerebellopontine angle (CPA) endodermal cyst in a 56-year-old man who presented with a 6-month history of right-sided hemifacial spasm. Computed tomography revealed a homogenous, well-demarcated, hyperdense lesion extending from prepontine cistern to right CPA. Magnetic resonance imaging demonstrated a right CPA extra-axial cystic lesion protruding into Meckel's cave, with compression of cranial nerves VII and VIII. Light brown, creamy cystic content was totally removed, and the thin cyst

Predictors of surgical outcomes following anterior transposition of ulnar nerve for cubital tunnel syndrome: a systematic review.

PURPOSE: Although cubital tunnel syndrome is the second most common nerve entrapment neuropathy, few studies explore potential predictor(s) of surgical outcomes. The purpose of this systematic review was to determine which factors affect the postoperative outcome for patients who undertake anterior transposition of the ulnar nerve. METHODS: We included all studies reporting predictor(s) of clinical, electrophysiological study, or functional outcome after any anterior transposition of the ulnar nerve. We searched the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, and CINAHL f

[Sinking skin flap syndrome].

INTRODUCTION: The sinking skin flap syndrome is a complication of decompressive craniectomies. It is of relevance not only due to its frequency, it is often underdiagnosed, but also because of the possibility of reversing the symptoms with the proper treatment. CASE REPORT: A 53-year-old female sustained a severe head injury. The initial Glasgow Coma Scale was 6T. The CT scan showed a severe traumatic injury in the left temporal lobe that required a decompressive craniectomy. After surgery the patient developed aphasia and right hemiparesis but progressively improved. Four months after surger

Thoracic spinal cord epidural hematoma after extracorporeal shock wave lithotripsy.

Extracorporeal shock wave lithotripsy (ESWL) is a well-established, safe, and effective therapeutic alternative to surgical treatment for urolithiasis. Complications of ESWL do occur in a small number of patients, and when they do, they typically involve the kidney. Formation of a spinal epidural hematoma is an extremely rare complication after ESWL for kidney stone removal. The authors present the case of a 61-year-old man in whom a large spinal epidural hematoma developed after ESWL. They discuss the possible pathogenic mechanism of such a complication.

Management of entrapped temporal horn by temporal horn to prepontine cistern shunting.

BACKGROUND: Entrapped temporal horn syndrome secondary to obstructive neoplastic lesions is most frequently treated by surgical excision of the offending lesion. Here we describe an alternate approach involving temporal horn to prepontine cistern shunting followed by radiosurgery of the offending lesion. METHODS: A 41-year-old woman with a history of meningiomatosis presented with progressive, incapacitating headache. Magnetic resonance imaging (MRI) showed growth of a right trigone meningioma, causing entrapment of the right temporal horn. A ventricular catheter was placed using frame-based

Efficacy and necessity of nasojejunal tube after gastrectomy.

BACKGROUND: Nasojejunal tube (NJT) decompression is routinely used for intestinal drainage after total gastrectomy. It is supposed that it would protect anastomosis, but since the stomach should be completely removed, today its efficacy is under question. On the other hand, the tube leads to the discomfort of patients and aspiration disorders or nasopharyngial ulceration. The aim of this study was to evaluate the efficacy and necessity of the nasojejunal tube decompression after gastrectomy. METHODS: In this interventional study, 50 gastric cancer patients who underwent gastrectomy in Ghaem a

Vertical atlantoaxial dislocation as a cause of failure of midline fixation.

We report an 11-year-old girl who had previously undergone an operation for basilar invagination involving a foramen magnum decompression and midline wire fixation. After improving initially, her neurological condition worsened again. Repeated investigations showed a firm midline craniovertebral fixation and bone fusion. However, she was found to have a vertical mobile and reducible atlantoaxial dislocation. Treatment of the vertical dislocation by lateral mass fixation resulted in lasting relief from her symptoms. Vertical instability at the atlantoaxial joints needs to be identified and appr

Angiographic confirmation of brain death after decompressive craniectomy for posttraumatic brain oedema.

In patients who underwent decompressive craniectomy for brain oedema, angiography for the assessment of brain death can show filling of the intracranial supratentorial arteries. This does not exclude brain death, provided circulatory arrest and absence of venous drainage are present.

Spontaneous resolution of a massive pseudomeningocoele.

A pseudomeningocoele is an extradural collection of cerebrospinal fluid which results from a dural or arachnoid tear. The fluid is not contained within a protrusion of the meninges, which is typical for a real meningocoele. Although most pseudomeningocoeles probably go unrecognised, due to lack of symptoms, surgeons should maintain an index of suspicion when reviewing postoperative patients. Symptomatic pseudomeningocoeles warrant intervention, as the patients do not tolerate the symptoms. However, the literature neither suggests a trial of watching and waiting nor a suitable time frame for su

Periarticular calcification causing acute carpal tunnel syndrome: a case report.

We report a case of acute carpal tunnel syndrome caused by periarticular calcification (hydroxyapatite deposition disease) around the wrist joint in a 64-year-old woman. She had acute severe pain, exacerbated by wrist movements and extension of the fingers. Her full blood count, urea, electrolytes, uric acid, calcium, phosphate, alkaline phosphatase, and thyroid function levels were all within normal ranges, and her serum was negative for rheumatoid factor. Computed tomography revealed lobulated calcification close to the volar capsule. She underwent an emergency surgical decompression of the

Ramsay hunt syndrome: a histopathologic observation of a facial sequelae.

In 1907, James Ramsay Hunt (1) described a syndrome characterized by facial nerve paralysis associated with herpetic eruption of the pinna and suggested that herpes zoster oticus resulted from a geniculate ganglionitis. However, many contemporary authors agree that this condition represents a polycranial neuronitis and not simply a geniculate ganglionitis. Thus, it is a more complex disease, and the precise pathogenesis in the temporal bone that leads to facial palsy is not yet known.
