Adult progressive sensorineural hearing loss: is preoperative imaging necessary before cochlear implantation?

OBJECTIVE: Preoperative evaluation of cochlear implant candidate includes routine imaging to identify anatomic abnormalities that may preclude or complicate implantation, such as cochlear aplasia, absent/narrowed internal auditory canals, cochlear ossificans, or significant traumatic fracture. The aim of this study is to determine if preoperative imaging is necessary in select cochlear implant candidates, thus defraying cost and ionizing radiation. STUDY DESIGN: Retrospective chart review. SETTING: Tertiary referral facility. PATIENTS: Adult patients with progressive sensorineural hearing l

Removal of large or giant sporadic vestibular schwannomas via translabyrinthine approach: a report of 115 cases.

BACKGROUND/AIMS: The present study was a retrospective analysis of surgical outcomes of large or giant vestibular schwannomas (VSs) via a translabyrinthine approach (TLA). METHODS: One hundred and fifteen sporadic large or giant VSs (maximal diameter in the cerebellopontine angle ≥31 mm) were operated via TLA between January 2001 and December 2010. We divided patients into two groups: cystic VS (CVS) and solid VS (SVS) group. The surgical outcomes were recorded and compared. RESULTS: Total, near total, and subtotal tumor removal rate was 89.6, 7.8, and 2.6%, respectively. The near total remo

Development of the vestibular system and balance function: differential diagnosis in the pediatric population.

Dizziness is a rare complaint among children. In this article, the authors present the embryology and development of the vestibular system, and offer a rational approach to taking a careful history and ordering and interpreting appropriate vestibular and balance testing in children. A differential diagnosis is presented, so that the likely cause of the balance disorder can be elucidated even in the most complex pediatric patients.

Vertebral artery dissection in a patient with Wildervanck syndrome.

Vertebral artery dissection as a cause of stroke is rarely reported in children. The association between vertebral artery dissection and Klippel-Feil syndrome is also very rare. We report on a case of vertebral artery dissection with posterior circulation involvement in a child with Klippel-Feil syndrome after a hard physical-training lesson. She was also diagnosed with Wildervanck syndrome, with additional clinical findings. Vertebral artery dissection should be considered in patients with Klippel-Feil syndrome who present with acute-onset neurologic signs. Movements such as hyperextension wi

Expression patterns of the Tmem16 gene family during cephalic development in the mouse.

Tmem16a, Tmem16c, Tmem16f, Tmem16h and Tmem16k belong to the newly identified Tmem16 gene family encoding eight-pass transmembrane proteins. We have analyzed the expression patterns of these genes during mouse cephalic development. In the central nervous system, Tmem16a transcripts were abundant in the ventricular neuroepithelium, whereas the other Tmem16 family members were readily detectable in the subventricular zone and differentiating fields. In the rostral spinal cord, Tmem16f expression was highest in the motor neuron area. In the developing eye, the highest amounts of Tmem16a transcrip

A mesenchyme-free culture system to elucidate the mechanism of otic vesicle morphogenesis.

The vertebrate inner ear has been extensively studied as a model system of morphogenesis and differentiation. The interactions between epithelium and surrounding mesenchyme have not previously been studied directly, because an appropriate experimental system had not been established. Here we describe a mesenchyme-free culture system of E11.5 mouse otic vesicle which retains the ability for (1) formation of the cochlear loop, (2) emigration of ganglion cells from the epithelium and (3) invagination of semicircular canal epithelium. E10.5 otic vesicle was maintained using the same method, but mo

Correlation between auditory function and internal auditory canal pressure in patients with vestibular schwannomas.

OBJECT: Hearing loss is the most common presenting symptom in patients who harbor a vestibular schwannoma (VS). Although mechanical injury to the cochlear nerve and vascular compromise of the auditory apparatus have been proposed, the exact mechanism of this hearing loss remains unclear. To test whether pressure on the cochlear nerve from tumor growth in the internal auditory canal (IAC) is responsible for this clinical finding, the authors prospectively evaluated intracanalicular pressure (ICaP) in patients with VS and correlated this with preoperative brainstem response. METHODS: In 40 cons

[Trans-labyrinthine approach for the resection large and giant acoustic nerve neuromas].

The standard translabyrinthine approach for acoustic neuromas removal was introduced by W. House in 1964. After several years of experience the original translabyrinthine approach has been progressively modified into the current "enlarged" approach by extending the area of bone removal. This increased surgical field has made the translabyrinthine approach suitable for the removal of tumours of all sizes. We present our serie of 71 large (52) and giant (19) neuromas of the VIIIth nerve removed through a translabyrinthine approach between 1993 and 1998 at the ENT Department of Legnano.

Facial nerve paralysis in temporal bone fractures: outcomes after late decompression surgery.

The aim of this paper was to address some of the unanswered questions regarding management of facial nerve paralysis in temporal bone fractures (TBF), such as the outcomes after late facial nerve decompression surgery. The study design was a retrospective review of a consecutive clinical series. Thirteen patients who underwent late decompression surgery for facial nerve paralysis due to TBF involving the perigeniculate ganglion region were analyzed. Patients were operated on 27-90 days after trauma. A transmastoid extralabyrinthine approach was used in all cases. Facial nerve-sheath slitting w

Treatment of ipsilateral delayed endolymphatic hydrops.

Nine patients with ipsilateral delayed endolymphatic hydrops were treated. All cases were initially treated by conservative therapy using isosorbide or other drugs and 4 patients have been doing well without surgical intervention, though they occasionally have slight dizziness. Five patients required surgery to control vertigo. Three patients first underwent cochleosacculotomy to maintain the vestibular function in the diseased ear, but vertigo recurred after 6 months in all 3 cases. These 3 patients and another patient who wanted definitive treatment right from the start underwent transmastoi

Effect of hemilabyrinthectomy on monoamine metabolism in the medial vestibular nucleus, locus coeruleus, and other brainstem nuclei of albino and pigmented rats.

We compared in albino and pigmented rats the early effect of unilateral labyrinthectomy (UL) on the concentrations of monoamines (norepinephrine, dopamine, serotonin) and their respective metabolites--3-methoxy,4-hydroxyphenylglycol (MHPG), 3,4-dihydroxy-phenylacetic acid (DOPAC), and 5-hydroxyindoleacetic acid (5HIAA)--in medial vestibular nuclei (MVN), locus coeruleus, raphe dorsalis, and cochlear nuclei. The study was conducted 6 hours after UL in both strains, differing by the functional optokinetic responses of their central vestibular neurons and the time-course of their vestibular compe

[Enlarged retro-labyrinthic approach: application to surgery of acoustic neurinoma. Apropos of 55 cases].

The enlarged retro-labyrinthic route described here allows opening of the internal auditory canal, exeresis of tumors localized in the ponto-cerebellous angle and in the internal auditory canal. We used this route for 55 neurinomas of the auditory nerve (7.4% stage I, 23.6% stage II, 45.4% stage III and 23.6% stage IV). Most of the tumors in our series were > 2 cm. There were no post-operative deaths. Risk of fistulization was low, 3.8%. Post-operative meningitis (3.8%) regressed rapidly with treatment. Facial function outcome was equivalent to that obtained with the enlarged trans-labyrinthic

[Chronic myeloid leukemia--a rare cause of labyrinth apoplexy].

BACKGROUND: Cochleovestibular lesions in patients with acute myeloid leukemia or the blastic phase of chronic myeloid leukemia are usually due to leukemic infiltration, infection or hemorrhage. In contrast, the most likely cause of cochleovestibular lesions in the chronic phase of the chronic myeloid leukemia seems to be disturbed microvascular perfusion and vascular occlusion. CASE REPORT: A 45-year old patient with a chronic myeloid leukemia presented with the sudden onset of profound deafness in his right ear combined with a total loss of vestibular function on the same side. The hemoglobi

Unusual lesions of the internal auditory canal.

Tumor types other than acoustic neuromas are uncommonly encountered in the cerebellopontine angle, and unusual lesions limited to the internal auditory canal (IAC) are given more rare. We present five patients with unusual tumors of the IAC including two meningiomas, two arachnoid cysts, and a hemangioma. These unusual tumors, and lipomas, are discussed. Particular attention is paid to the pathophysiology of the lesions and treatment recommendations. The differential diagnosis of intracanalicular lesions using magnetic resonance imaging is also presented.

Unilateral acoustic tumors: how often do they recur after translabyrinthine removal?

Unilateral acoustic tumors rarely recur after total translabyrinthine removal. Review of the patient records of the House Ear Clinic revealed five recurrent tumors, for an approximate incidence of 0.3%. A questionnaire was mailed to 857 patients who were at least 9 years postoperative and failed to find any additional recurrent tumors. No preoperative or intraoperative factors were identified to predict recurrence. The average time interval from initial removal to recurrence was approximately 10 years. Flow cytometric analysis did not reveal any fundamental differences between the recurrent ac

Ciprofloxacin. Use as a topical otic preparation.

Most common topical otic preparations have been shown to cause sensorineural hearing loss and hair-cell damage in experimental animals. Ciprofloxacin is a relatively new fluoroquinolone with excellent activity against Pseudomonas and methicillin-resistant Staphylococcus aureus. Recent studies have shown oral ciprofloxacin to be effective in the treatment of chronic serous otitis media and malignant external otitis. However, this drug has never been used as a topical otic preparation. Thirty-five albino female guinea pigs were used to investigate the ototoxicity of topical ciprofloxacin hydroch

The anterior inferior cerebellar artery in the internal auditory canal.

It has been proposed that compression of the auditory and vestibular nerve trunks by vascular loops might be the cause of otherwise unexplained hearing loss, tinnitus, and vertigo, as well as Meniere's disease. We studied the human temporal bone histological collection at the Massachusetts Eye and Ear Infirmary to determine whether audiovestibular symptoms could be correlated with the presence of the anterior inferior cerebellar artery within the internal auditory canal. Anterior inferior cerebellar artery loops were found within the internal auditory canals of 12.3% of 1327 temporal bones, ab
