Stress shielding following radial head arthroplasty: the impact of preoperative bone quality.

BACKGROUND: We sought to assess if the medullary diameter to cortical width ratio (MD:CW), canal flair index (CFI), and canal fill (CF) of the proximal radius were associated with the presence of stress shielding (SS) after a MoPyC radial head arthroplasty. MATERIALS AND METHODS: We conducted a retrospective, international, multicenter (4 centers) study. A total of 100 radial head arthroplasties in 64 women and 36 men with a mean age of 58.40 years ± 14.90 (range, 25.00-91.00) were included. Radiographic measurements, including MD:CW, CFI, CF, and postoperative SS were captured at a mean foll

Factors associated with the choice of treatment for coronoid fractures.

AIMS: This study aimed to gather insights from elbow experts using the Delphi method to evaluate the influence of patient characteristics and fracture morphology on the choice between operative and nonoperative treatment for coronoid fractures. METHODS: A three-round electronic (e-)modified Delphi survey study was performed between March and December 2023. A total of 55 elbow surgeons from Asia, Australia, Europe, and North America participated, with 48 completing all questionnaires (87%). The panellists evaluated the factors identified as important in literature for treatment decision-making…

Postoperative mobilization after terrible triad injury: systematic review and single-arm meta-analysis.

BACKGROUND: Terrible triad injury is a complex injury of the elbow, involving elbow dislocation with associated fracture of the radial head, avulsion or tear of the lateral ulnar collateral ligament, and fracture of the coronoid. These injuries are commonly managed surgically with fixation or replacement of the radial head and repair of the collateral ligaments with or without fixation of the coronoid. Postoperative mobilization is a significant factor that may affect patient outcomes; however, the optimal postoperative mobilization protocol is unclear. This study aimed to systematically revie…

Successful Strategies for Managing Monteggia Injuries.

Monteggia fracture-dislocations refer to traumatic ulnar fractures associated with proximal radioulnar and radiocapitellar joint instability. Careful clinical and radiographic evaluation of the entire limb in the acutely injured child is critical for timely diagnosis and appropriate treatment. Treatment principles include restoring and maintaining both ulnar length and alignment as well as radiocapitellar joint reduction. Recent information suggests that surgical treatment of acute injuries associated with complete ulnar fractures is safe and effective in maintaining bone and joint alignment.

Functional Loss With Displacement of Medial Epicondyle Humerus Fractures: A Computer Simulation Study.

BACKGROUND: Assessment and management of the medial humeral epicondyle fracture remains controversial, with conflicting reports of displacem … s from 1 to 20 mm in the anterior, medial, and inferior directions relative to the humerus with the elbow at 90-degree flexion and neutral forearm rotation (a replication of accepted positions for clinical strength testing). Muscle length and force were calculated following displacement. Maximum isometric wrist flexion moments were calculated over the f …

Biomechanical Analysis of Screws Versus K-Wires for Lateral Humeral Condyle Fractures.

BACKGROUND: Good outcomes have been described for pediatric lateral condyle fractures treated by open reduction and fixation using either screws or Kirschner wires (K-wires). No studies have compared the biomechanical properties of the 2 fixation methods. We hypothesized that screw fixation would be more biomechanically stable than K-wire fixation. METHODS: Synthetic humerus models were used for biomechanical testing, following a previously published protocol. A miter saw was used to make an oblique cut to simulate a Milch type II fracture. Fractures were anatomically reduced and fixed with e

Transcortical screw fixation of the olecranon shows equivalent strength and improved stability compared with tension band fixation.

OBJECTIVES: Currently, the standard for 21-B3.1 olecranon fracture fixation is the tension band wire construct described by the AO foundation. Although this technique effectively repairs displaced olecranon fractures and osteotomies, it is associated with a high rate of secondary surgery for implant removal due to hardware "back out," prominence, and discomfort. The senior author of this study has used transcortical screw fixation for olecranon fractures and osteotomies to avoid hardware discomfort but has been unable to find literature documenting the strength of this method. Accordingly, we

Pediatric upper extremity stress injuries.

PURPOSE OF REVIEW: To provide primary care physicians with strategies to evaluate and manage pediatric upper extremity stress injuries related to overuse with a focus on anatomic location, patient history, physical examination, and imaging. RECENT FINDINGS: Children are susceptible to stress injuries due to their open physes. The physeal cartilage of epiphyses and apophyses is the weakest structure in the developing skeleton. The differential diagnosis for pediatric stress injuries includes injury to the bone, physis, epiphysis, and apophysis. Most stress injuries in children improve with res

Biceps activity during windmill softball pitching: injury implications and comparison with overhand throwing.

BACKGROUND: Windmill pitching produces high forces and torques at the shoulder and elbow, making the biceps labrum complex susceptible to overuse injury. Little is known about the muscle firing patterns during a windmill pitch. HYPOTHESIS: Biceps muscle activity is greater during a windmill pitch than during an overhand throw. STUDY DESIGN: Descriptive laboratory study. METHODS: Seven female windmill pitchers underwent motion analysis and surface electromyography evaluation of their biceps muscles during windmill and overhand throwing. Marker motion analysis, muscle activity, and ball relea…

Olecranon fractures repaired with FiberWire or metal wire tension banding: a biomechanical comparison.

PURPOSE: Tension band constructs are commonly used for olecranon fracture fixation. The purpose of this study was to determine if a tension band constructed of FiberWire (Arthrex, Naples, FL), a high-strength polyester and polyethylene suture, will provide fixation that is equivalent to an 18-gauge metal wire tension band. METHODS: The following 4 fixation methods were biomechanically tested in cadaveric elbows with a simulated transverse olecranon fracture: (1) Kirschner wires (K-wires) with an 18-gauge metal wire tension band, (2) K-wires with a FiberWire tension band, (3) intramedullary sc…

The Essex-Lopresti fracture-dislocation factors in early management and salvage alternatives.

Treatment recommendations for the Essex-Lopresti lesion have not come very far in 50 years. Although there have been multiple biomechanical studies, the biomechanics of forearm loading and stability remain somewhat elusive. Clinical studies have yielded some insight, but predictable outcomes are exceptional. More studies are needed to further understand the biomechanics of the forearm and provide a basis for reconstruction of the IOL. Although current clinical studies regarding IOL reconstruction and radial head replacement seem promising, long-term results with substantial patient numbers are

[Proximal and distal humerus fractures in advanced age].

In contrast to distal humeral fractures, humeral head fractures represent a common injury to the old patient. For both fracture localizations osteoporosis and multi-morbidity are of significant importance. The classification for humeral head fractures in one-, two-, three- and four-part fractures is generally accepted. Valgus impacted head fractures as well as head-splitting fractures are considered a separate entity. In none or minimally displaced fractures good functional results are achieved by conservative means. Although numerous therapeutical procedure are offered in the treatment of dis

[Essex-Lopresti injury combined with forearm fracture].

The Essex-Lopresti injury is uncommon and often overlooked. A case is presented in which a 22-year old male sustained an injury that was a combination of an Essex-Lopresti injury and a fracture of the forearm. The primary treatment was osteosynthesis of the forearm. Ten weeks after injury the fractured radial head was removed because of pain and poor function of the elbow. The full extent of the injury was not recognized until sixteen weeks after injury. Final treatment consisted of excision of the ulnar head. The final results when seen at eight years follow-up were good.

[Monteggia injury in childhood: a frequent cause of chronic dislocation of the head of the radius].

Two cases of chronic dislocation of the radial head after Monteggia lesions in children are described. The late reconstruction with open reduction of the radial head combined with osteotomy of the ulna is mentioned as the therapy of choice before skeletal maturity, because the pathophysiology of the lesion is best considered. The operative procedure in detail is given. Finally, alternative procedures are presented and discussed.

Cat-scratch disease mimicking rhabdomyosarcoma.

A 12-year-old boy who had injured his elbow presented with a lump in the epitrochlear area. Originally thought to be a hematoma, then later myositis ossificans, this mass was actually a lymph node with large, reactive germinal centers. Although stains for acid-fast bacilli, fungal organisms, bacteria, and cat-scratch disease were negative, serological testing for detection of Rochalimaea henselae organisms found evidence of cat-scratch disease. We would like to remind orthopaedic surgeons to consider this diagnosis in evaluating masses of the soft tissues of the extremities.

Computed tomography of joint injuries.

CT has become the imaging modality of choice in most acute and chronic injuries involving the articular surfaces. Today's technology allows high resolution, fast scanning, and rapid reformatting, all of which contribute to prompt orthopedic evaluation and decision making. Our orthopedic colleagues have come to rely on high-quality CT scans in order to evaluate calcaneal and tibial plateau fractures, stress fractures, osteochondritis dissecans, acute and chronic shoulder trauma, and problems of the painful wrist--especially the distal radioulnar joint.
