Dupuytren's and Ledderhose disease can be a cumbersome condition in patients with a severe diathesis with a very early onset. Two brothers are described with a reversed radial forearm flap on both hands and two upper lateral arm flaps on both feet with a long-term follow-up ranging from 14 to 25 years. They had multiple procedures of both hands before the flaps were considered. No recurrence occurred under the flap. In very severe diathesis flaps should be considered in an earlier phase to prevent multiple procedures and early recurrence.

Barrel explosions (or "bursts") can occur when different gauge cartridges are inadvertently placed into the breech of a shotgun. This article describes two cases in which injuries were sustained to the left hand and forearm as a result of the placement of 12- and 20-gauge cartridges into 12-bore shotguns. Osteomyelitis caused by Salmonella typhi is rare in patients with no haemoglobinopathies or other diseases causing immunosuppression. Brodie's abscess is a special variety of subacute or chronic osteomyelitis. An otherwise healthy woman who presented with forearm swelling for 6 months was diagnosed with a Brodie's abscess of the ulna caused by Salmonella typhi. In this report divergent dislocation of the elbow in an 11-year-old boy and its outcome is presented. His right upper limb was trapped in a water irrigation machine and sustained an extreme pronation thrust through the hyperextended elbow. He had also suffered from a segmental fracture of both bones of the forearm and a fracture of the humerus. The median and ulnar nerves were functionless at his presentation. Considering the 16 cases described in the literature, different types of divergent dislocation of the elbow and the plausible mechanisms in producing this type of injury are reviewed.

We report the case of a 46-year-old veterinarian who developed rhinoconjunctivitis and contact urticaria (CU) on his hands and forearms after contact with dog's milk. Prick tests results revealed a positive reactions to Parietaria pollens, Dermatophagoides farinae and D.pteronyssinus, dander of cat, grass, olive (+++), and to undiluted dog's milk (++++). Control prick tests with dog's milk performed in 5 healthy veterinarians were negative. We consider it important to take into account the possibility that dog's milk may be the aetiological agent of CU in veterinarians.

A 71-year-old woman was admitted to our hospital with prominent abdominal distention. The diagnosis of systemic sclerosis (SSc) was made based on the sclerosis on her face, forearms, fingers, and toes as well as pitting ulcers on fingers and toes. After conservative therapy, she presented more severe signs of peritoneal irritation. Abdominal surgery was performed, revealing stenosis at the terminal ileum. Pathologically, this region showed muscularis propria becoming fibrotic. She was discharged from the hospital, returning about once a month as an outpatient.

Isolated rupture of of the ventricular septum after blunt chest trauma is a very rare traumatic affection. A 21-year-old man was admitted to our hospital because of blunt chest trauma and a forearm fracture. Initial echocardiography did not show any intracardiac or extracardiac pathologic lesions, but 12 hours later this examination was repeated because of the onset of a holosystolic murmur. An unusual traumatic rupture of the ventricular septum was demonstrated. The hemodynamically stable condition of the patient allowed surgical repair to be performed 3 months later.

We here report the first case of postmortem injury caused by a centipede. An old man was found dead in his bedroom. The death was estimated to be due to intracranial hemorrhage and to have occurred two days before the police inspection. A centipede about 12 cm long emerged from a subcutaneous cavity on the victim's forearm. Obviously, the centipede had dug the cavity on the intact skin. A police inspector was bitten by the centipede, so he stepped on the centipede on the floor. The exudate from the insect was identified to be derived from the victim's blood.

Adverse reactions in two patients who received HMG CoA reductase inhibitor therapy were reinvestigated because of their rarity. A case of permanent forearm myalgia was thought to be caused by atorvastatin. We present a case and review of the literature of an amniotic band causing malformation in the distal forearm in a baby born to a methadone-dependent mother. This diagnosis was made prenatally at the routine 18-20-week gestation anomaly ultrasound scan. Histology of the amniotic band postpartum revealed hair shafts within its structure. We report on the third patient with serpentine fibula-polycystic kidney syndrome. Main features in the three reported cases were growth retardation, abnormal face, hirsutism, short neck, bowed forearms and lower legs due to bowed radii and elongated serpentine fibulae, and metatarsus adductus. Two patients including our own were deaf. All were mentally normal, all were female and sporadic. In addition, we report on a girl with Melnick-Needles syndrome and illustrate the similarities and differences between these syndromes.

The susceptibility of the skin of patients with seborrhoeic dermatitis to surfactant irritation was investigated and compared to that of a group of normal subjects and patients with a history of atopic eczema.
