Missed Jeffery Type II Injury: Treatment in Two Cases and Literature Review.

The diagnosis and management of Type 2 Jeffery injuries, particularly when missed in the acute phase, present significant challenges. There are few documented cases of Jeffery fractures in the literature, and limited evidence regarding outcomes following treatment of delayed or missed injuries. We present two cases of undiagnosed Jeffrey Type 2 lesions. Both patients, aged 10 and 8 years, initially presented with elbow pain after falls. The injuries were unrecognised at initial assessment, but were identified at follow-up 3-4 weeks later. Surgery was performed promptly after diagnosis. In both

A Systematic Approach to the Evaluation of Acute Musculoskeletal Injuries in the Emergency Department.

Musculoskeletal (MSK) complaints account for a significant proportion of emergency department (ED) visits and range from minor soft tissue injuries to fractures, tendon ruptures, dislocations, septic arthritis, and compartment syndrome. Despite their frequency, these presentations can be diagnostically challenging because of variable mechanisms of injury, overlapping examination findings, and limitations of plain radiography for detecting soft tissue and occult bony injuries. Missed diagnoses, such as Achilles tendon rupture, scaphoid fracture, Lisfranc injury, and tibial plateau fracture, may

Management of Acetabular Fractures with Total Hip Replacement: A Narrative Literature Review.

Open reduction and internal fixation (ORIF) is widely regarded as the primary treatment for acetabular fractures, but limitations arise in complex cases, leading to non-anatomical reductions and increased risk of post-traumatic osteoarthritis. Given the high incidence of secondary arthritis (12-57%) following ORIF, total hip arthroplasty (THA) is often necessitated, particularly in scenarios unsuitable for ORIF, such as extensive comminution or combined femoral head and neck fractures. The surgical landscape has shifted from a traditional "fix or replace" to a more integrated "fix and replace"

Musculoskeletal Computed Tomography: How to Add Value When Reporting Adult Upper Limb Trauma.

There is increasing reliance on computed tomography to evaluate fractures and dislocations following routine evaluation with plain radiography, critical in preoperative planning; computed tomography can provide multiplanar reformats and 3-dimensional volume-rendered imaging, providing a better global assessment for the orthopedic surgeon. The radiologist plays a critical role in appropriately reformatting the raw axial images to illustrate best the findings that will help determine further management. In addition, the radiologist must succinctly report the pertinent findings that will have the

Unstable Middle Phalanx Base Fractures Treated With an Internal Joint Stabilizer: Preliminary Results.

BACKGROUND: Unstable fractures of the base of the middle phalanx are notorious for causing chronic loss of proximal interphalangeal (PIP) joint function, and they remain a challenge for the hand surgeon. We report on a temporary intraoperatively constructed internal joint stabilizer for unstable PIP joint injuries. METHODS: Across 2 institutions, a retrospective chart review was performed for cases with acute presentation of pilon fracture or fracture-dislocation of the base of the middle phalanx which were surgically treated with an internal joint stabilizer. Information collected included t

Cemented versus uncemented hemiarthroplasty for femoral neck fractures in patients with neuromuscular diseases: a minimum of 2 years' follow-up study.

BACKGROUND: The aim of this study was to compare the outcomes of cemented and uncemented hemiarthroplasty for femoral neck fractures in patients with neuromuscular disease. METHODS: We reviewed 156 patients with neuromuscular disease who underwent hemiarthroplasty between June 2015 and December 2019. Patients were divided into cemented group (n = 105) and uncemented group (n = 51), with a minimum follow-up of 2 years. Factors including preoperative features, duration of surgery, intraoperative blood loss, complications, pain, Harris hip scores (HHS), and quality of life were compared across g

Optimal Prosthesis Sizing for Radial Head Arthroplasty: A Review of Current Evidence and Guidelines.

»: Radial head arthroplasty is a viable surgical option when a radial head fracture cannot be reconstructed. Radial head arthroplasty provides a load-bearing articular structure against the capitellum in unstable fractured elbows. »: Studies have emphasized the importance of choosing the correct implant size to replicate the native radial head anatomy, citing various consequences of improperly sized radial head prostheses. Overstuffing of the radiocapitellar joint, or lengthening of the radius, has been extensively studied because of its detrimental effects on elbow biomechanics, but other ty

A detaching, V-shaped tibial tubercle osteotomy is a safe procedure with a low complication rate.

INTRODUCTION: In patients with recurrent patellar dislocations, a tibial tubercle osteotomy (TTO) can be indicated to correct patella alta or an increased trochlear groove-tibial tubercle distance. Several surgical techniques are described. Previous studies emphasize that detaching osteotomies results in devascularisation, which can lead to non-union and tibial shaft fractures. The aim of this study was to report the complication rates directly related to the surgical technique of a V-shaped TTO, where the tubercle is completely released from its periosteum using a step-cut osteotomy. METHODS

Short-term results of total hip replacement due to acetabular fractures.

OBJECTIVES: This study aims to evaluate short-term results of total hip replacement performed for post-traumatic osteoarthritis and/or femoral head necrosis caused by acetabular fractures. PATIENTS AND METHODS: We performed 39 arthroplasties due to acetabular fractures on 25 male and 14 female patients during the course of a five-year period. The mean age of the patients was 45 years (range 25 to 73 years) at the time of surgery. Cemented cup was implanted in 29 patients and uncemented cup in 10 patients. Bone replacement was indicated in eight patients for different degrees of acetabular def

[Ex-vivo gene therapy with BMP-4 for critically sized defects and enhancement of fracture healing in an osteoporotic animal model].

Fractures in osteoporotic bones or segment defects are problematic bone lesions with a reduced biological capability of regeneration. We tested the hypothesis that cell-mediated ex vivo gene therapy to deliver BMP4 can heal critically sized defects and improve bone healing in osteoporotic rats. Primary muscle-derived cells were isolated from the hindlimb muscle of rats and retrovirally transduced to express bone morphogenic protein 4 (BMP4). The bone formation was evaluated following local application of these cells in critically sized defects and in fractures of osteoporotic bones. Radiograph

[Functional postoperative treatment of internally fixed ankle fractures with a flexible arthrodesis boot (Variostabil)].

AIM: Postoperative treatment following osteosynthesis of ankle fractures in a flexible arthrodesis boot (Variostabil) aims at fast restoration of the function of the injured extremity while allowing early full weight bearing. This treatment regimen was validated in a clinical and experimental study. METHODS: (1) In the clinical study part, 56 patients with internally fixed ankle fractures received after treatment with the arthrodesis boot for 6 weeks. (2) In the experimental study part, the intravascular pressure was recorded in a foot vein of 8 healthy volunteers during knee bends. RESULTS:

[MRI in dislocation and instability of the shoulder joint].

The value of magnetic resonance imaging (MRI) in shoulder instability depends on its ability to demonstrate pathologic lesions within the joint. Traumatic dislocation in particular often results in capsulo-labral-avulsion, stretching of the joint capsule and impression fracture at the humeral head. Various MR weightings, including spin echo and gradient echo, are useful at the shoulder. A surface coil should be used. Image planes are oriented at the scapula. A capsulo-labral-avulsion can not be reliably demonstrated on non-contrast imaging in the absence of intra-articular effusion. With indir

Hemiarthroplasty in the treatment of comminuted intraarticular fractures of the proximal humerus.

Thirty-eight patients with four-part proximal humerus fractures and fracture dislocations were treated with humeral head replacement between 1989 and 1995. At followup (mean, 37 months; range, 12-48 months) the patients were evaluated for postoperative pain, active range of motion, muscular strength, overall function in every day activities, and patient satisfaction. Complications developed in five patients and consisted of humeral component malposition (one shoulder), rotator cuff insufficiency (two shoulders), and heterotopic ossification (two shoulders). According to the Neer criteria, the …

Subtalar distraction bone block fusion: an assessment of outcome.

Twenty-nine feet in 28 patients who underwent subtalar distraction bone block fusion for the treatment of subtalar deformities associated with symptomatic arthrosis were evaluated. All patients were assessed with the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale score. Eighteen men and 10 women with a mean age of 44 years were evaluated. The mean follow-up was 33 months. Subtalar arthrosis was secondary to trauma in 27 of 29 feet (93%) with os calcis fractures in 19, subtalar dislocations in 5, and talus fractures in 3. Eighteen of the 28 patients (64%) returned to e

Extensile Lateral Approach for the Operative Management of a Displaced Intra-Articular Calcaneus Fracture.

Calcaneal fractures account for approximately 1.2% of all fractures and 60% of all tarsal bone fractures. Almost 75% present as displaced, intra-articular fractures. Because of the complex articular and osseous anatomy, the vulnerable soft-tissue envelope, and the technically challenging approach needed for fixation, these fractures are often treated nonoperatively, resulting in poor outcomes. These poor outcomes can include entrapment of the posterior tibial and sural nerves, impingement, dislocation or entrapment of peroneal tendons, a widened heel with a loss of height, hindfoot varus or va
