Classification of surgical approaches to the mandibular condyle.

Published articles on surgical approaches to the mandibular condyle seem to be confusing. We present a classification system that describes and differentiates between surgical approaches to the mandibular condyle and is based on the relation to the facial nerve, height of the approach, and choice of skin incision.

[Arthroscopic technique using crossed K-wires for avulsion fractures of intercondylar eminence in children].

PURPOSE OF THE STUDY: Avulsion fractures of the intercondylar eminence (ICE) of the tibia have become increasingly frequent in older school age. Due to recent advances in arthroscopic techniques, it is now possible to treat them by minimally invasive methods. This retrospective study presents evaluation of clinical outcomes of an arthroscopic technique using crossed K-wires. MATERIAL: Thirty-five patients with ICE avulsion fracture, 17 girls and 18 boys at an average age of 13 years (7-18), were included in this study. All were indicated for surgical treatment on the basis of radiographic evi

Radius morphology and its effects on rotation with contoured and noncontoured plating of the proximal radius.

BACKGROUND: The radius has a sagittal bow and a coronal bow. Fractures are often treated with volar anterior plating. However, the sagittal bow is often overlooked when plating. This study looks at radial morphology and the effect of plating the proximal radius with straight plates and then contoured plates bowed in the sagittal plane. We report our findings and their effect on forearm rotation. MATERIALS AND METHODS: Morphology was investigated in 14 radii. Attention was paid to the proximal shaft of the radius and its sagittal bow; from this, 6-, 7-, and 8-hole plates were contoured to fit

[Patellar osteochondral injury as onset of patellar instability].

Patellar osteochondral fractures with no dislocation are uncommon and usually affect the centromedial facet of the patella. We present the case of a 10 year-old, overweight, female patient. She was seen in the emergency room after suffering an accidental fall, assessed as an osteochondral fracture-dislocation of the right patella with upper-outer displaced free fragments. By patient interview, she referred to no previous episodes of patellar dislocation. To complete the study, we performed an MRI which showed a medial facet patellar fracture, with two osteochondral fragments located in the sub

Anatomical variation of mental foramen. A case report.

We have reported a case of triple mental foramina at the right side of the premolar region, which was discovered during reposition and ostheosynthesis of a mandible fracture. A review of the literature, was performed which disclosed no previous clinical cases reported but an incidence of 1.2% of triple foramina after investigating dry skulls or radiographics. The discussion stresses the importance of adequate preoperative radiological examination in the clinical situation especially when closed surgery is planned.

Partial avascular necrosis after talar neck fracture.

BACKGROUND: Recently, it has been shown that avascular necrosis of the talus can occur in only a portion of the talar body. There is little information regarding the geographic location of the avascular segment and the clinical significance of an incomplete avascular process. METHODS: Seven patients with partial avascular necrosis after Hawkins type II or III fracture dislocations were evaluated with magnetic resonance scans. The precise anatomic location of the avascular segment was determined and assigned to a specific quadrant of the talar body. The operative exposure, incidence of collaps

The triceps-flexor carpi ulnaris (TRIFCU) approach to the elbow.

INTRODUCTION: Intra-articular fractures of the distal humerus frequently require internal fixation. Several approaches have been described, with the posterior approaches being most common. We present a new approach to the distal humerus via the lateral border of the triceps muscle. PATIENTS AND METHODS: The senior author has used this technique for fixation of intra-articular fractures of the distal humerus in 12 patients. RESULTS: The approach is equally useful for intra- and extra-articular fractures. No cases of postoperative ulna nerve neuropraxia have been encountered. There have been n

Influence of an additional locking screw on fracture reduction after volar fixed-angle plating-introduction of the "protection screw" in an extra-articular distal radius fracture model.

BACKGROUND: Though, volar fixed-angle plating has become increasingly popular to treat distal radius fractures no studies are available comparing the mechanical properties of different screw configurations to fix the plate to the shaft. The aim of the present study was to evaluate the effect of an additional locking screw directly proximal to the fracture site and to assess if such a screw might be protective against secondary loss of reduction after volar fixed-angle plating. METHODS: Ten Sawbones radii were used to simulate an extra-articular distal radius fracture model (AO/OTA 23-A3). In

Resorbable versus titanium plates for facial fractures.

BACKGROUND: Rigid internal fixation of the jaw bones is a routine procedure for the management of facial fractures. Titanium plates and screws are routinely used for this purpose. The limitations of this system has led to the development of plates manufactured from bioresorbable materials which, in some cases, omits the necessity for the second surgery. However, concerns remain about the stability of fixation and the length of time required for their degradation and the possibility of foreign body reactions. OBJECTIVES: To compare the effectiveness of bioresorbable fixation systems with titan

Arthroscopic management of scaphoid fractures in athletes.

Fractures of the scaphoid are a common athletic injury. In this article the indications and treatment strategy for arthroscopic management of scaphoid fractures and nonunions in athletes are reviewed. Various arthroscopic assisted and percutaneous techniques for the fixation of fractures of the scaphoid are discussed, including the volar and dorsal percutaneous approaches, and arthroscopic reduction by the Geissler technique. In general, these techniques include a small amount of wrist arthroscopy and a significant amount of fluoroscopy.

Carpal fractures in athletes excluding the scaphoid.

A wide range of hand and wrist injuries occur in today's recreational and elite athletes and account for 3% to 9% of all sports injuries. The onus is on the physician to discriminate between injuries that can be managed with an early return to sport, and those injuries that place the athlete at risk of further injury if they are not managed aggressively from the outset. The physician and the athlete must understand the balance between safe, early return to sport, and prompt surgical treatment that prevents late disability.

Biomechanical comparison of two headless compression screws for scaphoid fixation.

This study compared compression generation between two headless compression screws: the Synthes 3.0-mm and the Acutrak standard. Twenty scaphoids were harvested from 10 pairs of fresh cadaveric forearms. A washer-shaped load cell was inserted between the halves of each scaphoid created by a simulated fracture via osteotomy. One scaphoid of each pair was tested with the Synthes and the other with the Acutrak. Parameters of interest were peak screw torque and fracture site compression. Differences in parameters of interest were checked for significance (p < .05) with paired t tests. No significa

Delayed appearance of diplopia due to orbital emphysema after repair of orbital fractures.

A 38-year-old caucasian male, after continuous nose blowing caused by common flu, developed left diplopia, especially in downward gaze, and complained of orbital dull pain during the preceding 48 hours. Clinical signs included left lower eyelid crepitant edema, mild exophthalmos and restriction of the eye movement. The patient had been operated on for orbital fractures 12 days previously. Open reduction had been performed, stabilization had been achieved with osteosynthesis miniplates, and the orbital floor defect was reconstructed with a piece of Liodura. Orbital computerized tomographic scan

Pedicle screw fixation (T1, T2, and T3).

The indications for thoracic pedicle screw fixation have expanded over the past decade. Thoracic pedicle screws are now being used in the treatment of degenerative, traumatic, neoplastic, congenital, and developmental disorders. The pedicles of T1, T2, and T3 are typically large and ovoid in shape and amenable to pedicle screw fixation in most instances. The placement of thoracic pedicle screws requires knowledge of the topographic and deep bony anatomy of the thoracic spine as well as an appreciation of the surrounding visceral structures at risk. With strict adherence to the surgical techniq
