The localization and biological roles of the multifunctional cell type mast cells remain unclear in subacute thyroiditis that is characterized by both epithelioid granuloma formation and thyroid tissue repair. We examined their immunolocalization with tryptase of a mast cell marker, using the biopsy specimens from 12 cases. In the epithelioid granuloma, no mast cells were detected in any of the cases, although a small number of them (4.6 +/- 2.4) were seen at the fibrous stroma around the granuloma in all cases. Elective surgery for peptic ulcer is becoming rare with the use of more effective medical therapy. However, life threatening complications have not reduced in number. In several cutaneous disorders, pruritus is both the primary symptom and the cause of perpetuating factor. Therefore, one focus of therapy should be on treating pruritus and thereby preventing or aiding in the resolution of skin lesions. Several treatment options are available, including nonpharmacologic methods; use of emollients, tar preparations, and other topical agents; ultraviolet B therapy; and use of oral antihistamines or corticosteroids. A new topical tricyclic agent is now available that adds a promising therapeutic approach.

We describe the first case of drug-induced solar urticaria due to repirinast, an antiallergic drug developed and introduced into the market in Japan in 1987. The patient was a 72-year-old woman who had been on repirinast for 1 year and 8 months. She developed urticaria immediately after an irradiation with 1.5 J/cm2 of UVA, and the provocation test confirmed that repirinast was responsible for the urticarial reaction. Data of complex clinico-instrumental and morpho-functional examination of 168 patients with transpyloric prolapse are presented. The authors suggest its great significance in the development and progress of the inflammatory-destructive changes of the stomach and duodenum. The detected features of the clinical course show the importance of a dynamic observation and rational treatment of patients with transpyloric prolapse.
