Graves' disease in a five-month-old boy with an unusual treatment course.

OBJECTIVES: Graves' disease (GD) is rare in children under age five years. Antithyroid drugs are typically first-line therapy but carry the risks of agranulocytosis and liver dysfunction. CASE PRESENTATION: A male infant with multiple congenital anomalies, left ventricular hypertrophy, and neurologic dysfunction developed GD at five months of life. The presence of chronic hepatitis complicated medical management. Potassium iodide was effective temporarily, but urgent thyroidectomy was required at nine months of age. Postoperatively, the patient developed a thyroid function pattern consistent

Novel Pharmacotherapy in Hypertrophic Cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is an inherited disease characterized by unexplained left ventricular hypertrophy. Although it is estimated to affect 1 out of 500 people, the HCM gene carrier prevalence is much more common, probably as high as 1 in 200 people. Most affected individuals have a normal life expectancy, whereas some patients may develop sudden cardiac death or end-stage heart failure. Despite significant developments in the treatment of HCM with surgical, interventional, and device-based procedures, the main focus of current pharmacological therapy has not evolved from the basic

Transforming growth factor beta (TGF-β) mediates cardiac fibrosis and induces diabetic cardiomyopathy.

Cardiovascular diseases account for the major cause of morbidity and mortality among individuals with diabetes. The diabetic cardiomyopathy (DCM) is a type of diabetic cardiovascular disease, which further directs to the heart failure. The researchers found that diabetes induced cardiac fibrosis plays a vital role in several of the pathological changes that associated with DCM, causing left ventricular hypertrophy (LVH), diastolic dysfunction and systolic dysfunction. However, the mechanisms involved in the pathogenesis of DCM are still elusive. Many studies have demonstrated that the transfor

The role of the paracrine/autocrine mediator endothelin-1 in regulation of cardiac contractility and growth.

UNLABELLED: Endothelin-1 (ET-1) is a critical autocrine and paracrine regulator of cardiac physiology and pathology. Produced locally within the myocardium in response to diverse mechanical and neurohormonal stimuli, ET-1 acutely modulates cardiac contractility. During pathological cardiovascular conditions such as ischaemia, left ventricular hypertrophy and heart failure, myocyte expression and activity of the entire ET-1 system is enhanced, allowing the peptide to both initiate and maintain maladaptive cellular responses. Both the acute and chronic effects of ET-1 are dependent on the activa

Ischemic heart disease in patients undergoing dialysis.

Cardiovascular disease is the most common cause of death in patients with end-stage renal disease (ESRD) who are undergoing chronic dialysis. Diabetes and hypertension, the 2 leading causes of ESRD, contribute to the pathogenesis of ischemic heart disease (IHD) in these patients, as do other traditional risk factors (eg, dyslipidemias, smoking, and sedentary lifestyle). However, patients with ESRD are subject to several unique risk factors that contribute to the development and progression of IHD. Chronic volume overload and anemia, leading to left ventricular hypertrophy, and deranged calcium

Current status and future prospects for a vaccine against American trypanosomiasis.

The clinically relevant pathognomonic consequences of human infection by Trypanosoma cruzi are dilation and hypertrophy of the left ventricle walls and thinning of the apex. The major complications and debilitating evolutionary outcomes of chronic infection include ventricular fibrillation, thromboembolism and congestive heart failure. American trypanosomiasis (Chagas disease) poses serious public healthcare and budgetary concerns. The currently available drugs, although effective against acute infection, are highly toxic and ineffective in arresting or attenuating clinical disease symptoms in

[Cardiovascular mortality predictors in the oldest old].

This article investigates the association between cardiovascular risk factors and cardiovascular morbidity and mortality in the oldest old. In 1996, 91% of the population > or = 80 years of age from Veranópolis, Rio Grande do Sul State, Brazil, were evaluated to detect cardiovascular risk factors and morbidity. The sample was followed up for three years, with the assessment of deaths. The analysis was done using univariate statistics and multivariate logistic regression analysis. There were 41 deaths (21%): 20 men and 21 women. Deaths were distributed by year as follow: 03 (7.3%) in the first

Cellular hypertrophy in cardiomyopathic patients is associated with lower creatine-stimulated mitochondrial respiration.

The mitochondrial respiration rate and morphometric indices in endomyocardial biopsy samples were measured in 43 patients with dilated cardiomyopathy selected in accordance to WHO criteria by endomyocardial biopsy studies after excluding of various forms of myocarditis, alcoholic cardiomyopathy and other specific diseases of the heart. A group of 13 patients with unusually high mean myocyte diameter, 30 +/- 4 microns, and nuclear size, 57 +/- 5 microns, was selected. The remainder of patients (n = 30) had significantly lower mean myocyte diameter and nuclear size, 23 +/- 3 and 42 +/- 6 microns

Cardiac catheterization and other imaging modalities in the evaluation of valvular heart disease.

This review describes recent publications using cardiac catheterization and diagnostic imaging modalities, excluding transesophageal and transthoracic echocardiography, to evaluate valvular heart disease. Radionuclide techniques and magnetic resonance imaging continue to be studied as to whether they can improve the quantification of valvular regurgitation. As yet, however, they are not justified in the routine evaluation of patients with valvular heart disease. Cardiac catheterization has been used by several authors to assess more completely the obstructive physiology of valve stenosis, incl

The body surface QRST isointegral maps in infants with right ventricular overload.

Electrocardiographic criteria for right ventricular (RV) hypertrophy in infants generally exhibit low sensitivity in terms of diagnostic accuracy. We studied the QRST isointegral map (QRST-Imap) of body surface potential distribution for the diagnosis of RV overload in patients less than 2 years old. Patients with atrial septal defect (ASD), pulmonary stenosis (PS) and tetralogy of Fallot (TOF) were examined (RV overload group) and the findings of their QRST-Imaps were compared to those of age-matched healthy infants (NOR). QRST-Imaps in RV overload showed abnormal findings, with two maxima or

[Disorders of the cardiovascular system in Turner's syndrome].

A group of 22 adults with Turner's syndrome, mean age 29.6 years, was subjected to a careful examination by one-dimensional, two-dimensional, pulsed and coloured Doppler echocardiography. The purpose was to assess the incidence and character of congenital and acquired abnormalities of the cardiovascular system which occur within the framework of this defined genetic syndrome. A quite normal echocardiographic finding was recorded in 13 patients, i. e. in 59.1%. In the remainder a wide spectrum of abnormalities was found such as prolapse of the mitral valve (in 13.6%), bicuspid aortal valve with

Preoperative evaluation of the thoracic aorta using MRI and angiography.

The ability of ECG-gated magnetic resonance imaging (MRI) to evaluate disease of the thoracic aorta compared with angiography was prospectively assessed in 28 patients. MRI identified abnormalities in all patients, with confirmation at operation in 22 (79%) and by angiography alone in all 28. In 20 of the patients, MRI correctly diagnosed 20 of 21 aneurysms of the thoracic aorta (6 dissecting, 4 saccular, 10 fusiform), but 1 surgically proven fusiform aneurysm was categorized as an enlarged aortic dissection based on both MRI and angiographic findings. One dissection and 1 fusiform aneurysm we

Epidemiologic features of chronic atrial fibrillation: the Framingham study.

In the Framingham Study 2325 men and 2866 women 30 to 62 years old at entry were followed biennially over 22 years for the development of chronic atrial fibrillation in relation to antecedent cardiovascular disease and risk factors. During surveillance, atrial fibrillation developed in 49 men and 49 women. The incidence rose sharply with age but did not differ significantly between the sexes. Overall, there was a 2.0 per cent chance that the disorder would develop in two decades. Atrial fibrillation usually followed the development of overt cardiovascular disease. Only 18 men and 12 women (31

Echocardiographic differentiation of hypertensive heart disease and hypertrophic cardiomyopathy.

The clinical differentiation of hypertensive heart disease from hypertrophic cardiomyopathy usually presents no problem but it is less clear whether an echocardiographic distinction can always be made and, if so, what those echocardiographic criteria of difference are. It can be inferred from recent publications that when echocardiographic criteria for hypertrophic cardiomyopathy are met in hypertensive subjects, both diagnoses may be made. This may be unjustified, and in order to clarify this problem that M-mode echocardiographic features of 37 patients with severe systemic hypertension were

Aortic valve replacement with frame-supported autologous fascia lata grafts. II. Clinical and laboratory findings.

Thirty-five patients with a frame-supported autologous fascia lata graft implanted in the aortic annulus were investigated 11 to 36 months after operation. The group comprised 7 patients with pure aortic stenosis, 11 with combined stenosis and incompetence and 17 with pure aortic incompetence. Seven patients had concommitant mitral valve disease. The follow-up investigation included ECG, a work test on a bicycle ergometer, dynamic spirometry, roentgenological heart volume determination and haematological "screening tests" for intravascular haemolysis. Concomitant with a marked subjective impro

The significance of intercalated discs in the pathogenesis of Friedreich cardiomyopathy.

Friedreich ataxia (FRDA) is an autosomal recessive disorder with a complex clinical and neuropathological phenotype, but the most frequent cause of death is cardiomyopathy. The principal autopsy findings in FRDA hearts are concentric hypertrophy, enlargement of cardiomyocytes, myofiber necrosis, inflammatory infiltration, scarring, and random accumulation of iron. In addition, the myocardium shows generalized disorganization of intercalated discs (ICD), the Velcro-like end-to-end connections of heart fibers that provide mechanical cohesion and ionic coupling. The principal components of ICD ar

I1-imidazoline-receptor agonists in the treatment of hypertension: an appraisal of clinical experience.

Although essential hypertension is usually defined as a hemodynamic disorder, it is expressed differently among individuals and varies during progression of the disease state. Therefore, various types of treatment can be envisioned. The use of selective I1-imidazoline-receptor agonists to modulate I1-imidazoline receptors involved in the central regulation of blood pressure has led to the introduction of a novel class of centrally acting antihypertensive drugs. Moxonidine, a representative molecule of this class, dissociates between a 10% alpha 2-adrenoceptor-agonist action linked with side ef

Ectopic Adrenocortical Adenoma Causing Malignant Hypertension and Hypokalemia.

BACKGROUND Ectopic adrenocortical adenomas are rare and can produce aldosterone autonomously, causing resistant hypertension and hypokalemia. Atypical locations pose diagnostic challenges. This report describes a 45-year-old man with malignant hypertension and hypokalemia due to an aldosterone-producing ectopic adrenal adenoma located between the pancreas and left adrenal gland. CASE REPORT A 45-year-old man with a 9-year history of poorly controlled hypertension presented with dizziness, vomiting, and fatigue. On admission, blood pressure was 192/110 mmHg and serum potassium was 2.07 mmol/L.
