Comparative evaluation of ambient digital scribe systems in clinical documentation.

OBJECTIVES: To demonstrate a replicable in-house framework for evaluating ambient digital scribe (ADS) systems and, applying it, to assess 3 commercial systems and examine alignment between clinician Likert ratings and a manual transcript-level error audit. MATERIALS AND METHODS: Two standardized clinical encounters-a brief problem-focused visit and a comprehensive annual visit-were simulated and audio-recorded. Three ADS vendors generated notes from identical recordings. A multidisciplinary panel of 56 physicians rated the notes on 7-point Likert scales across 4 domains: relevance, accuracy,

Artificial intelligence-based diagnostic decision support for primary care of older adults using collective intelligence and imitation learning.

OBJECTIVE: Older adults face disproportionate risk of diagnostic errors in primary care, yet effective diagnostic decision support systems (DDSSs) remain lacking. Existing artificial intelligence (AI)-based DDSSs rely on gold-standard labels infeasible to obtain, have narrow diagnostic scope, and fail to reflect real-world uncertainty. We developed and validated a deep learning DDSS using imitation learning and collective clinician intelligence to generate diagnostic and order recommendations. MATERIALS AND METHODS: We trained a multi-label neural network on structured and unstructured electr

Causal modeling of chronic kidney disease in a participatory framework for informing the inclusion of social drivers in health algorithms.

OBJECTIVES: Incomplete or incorrect causal theories are a key source of bias in machine learning (ML) algorithms. Community-engaged methodologies provide an avenue for mitigating this bias through incorporating causal insights from community stakeholders into ML development. In health applications, community-engaged approaches can enable the study of social drivers of health (SDOH), which are known to shape health inequities. However, it remains challenging for SDOH to inform ML algorithms, partially because SDOH variables are known to be interrelated, yet it is difficult to elucidate the caus

Predicting intracranial pressure monitor placement in children with traumatic brain injury: a prospective cohort study to develop a clinical decision support tool.

OBJECTIVE: Clinicians currently make decisions about placing an intracranial pressure (ICP) monitor in children with traumatic brain injury (TBI) without the benefit of an accurate clinical decision support tool. The goal of this study was to develop and validate a model that predicts placement of an ICP monitor and updates as new information becomes available. MATERIALS AND METHODS: A prospective observational cohort study was conducted from September 2014 to January 2024. The setting included one US hospital designated as an American College of Surgeons Level 1 Pediatric Trauma Center. Part

Transporting a pediatric surgical site infection prediction model to the electronic health record: external validation, harmonization, and recalibration.

OBJECTIVES: To externally validate a pediatric surgical site infection (SSI) prediction model, assess institutional and temporal transportability, and evaluate recalibration strategies to support adaptation of registry-derived predictions for clinical decision support (CDS). MATERIALS AND METHODS: An elastic-net logistic regression model developed using a national pediatric surgical registry (2012-2022) was validated in an institutional electronic health record (EHR)-derived cohort (10 450 procedures) and in a newly released, temporally distinct cohort from the same registry (301 999 procedur

Knowledge graph-augmented large language models for reconstructing life course risk pathways: a gestational diabetes mellitus-to-dementia case study.

OBJECTIVES: To develop and evaluate a knowledge graph-augmented large language model (LLM) framework that synthesizes epidemiological evidence to infer life-course exposure-outcome pathways, using gestational diabetes mellitus (GDM) and dementia as a case study. MATERIALS AND METHODS: We constructed a causal knowledge graph by extracting empirical epidemiological associations from scientific literature, excluding hypothetical assertions. The graph was integrated with GPT-4 through four graph retrieval-augmented generation (GRAG) strategies to infer bridging variables between early-life exposu

Evaluating large-scale propensity score adjustment when sample size is small.

BACKGROUND: Large-scale propensity score (LSPS) models are increasingly used to control confounding in observational studies, but their reliability in small-sample settings is unclear. Small samples can limit a study's ability to estimate propensity scores accurately and achieve adequate covariate balance, raising concerns about insufficient confounding adjustment. Resulting in small datasets being excluded, despite potentially containing valid information. METHODS: We evaluated the balancing and bias reduction performance of LSPS models using six target-comparator pairs. To emulate a distrib

Orchestrator multi-agent clinical decision support system for secondary headache diagnosis in primary care.

OBJECTIVE: Secondary headaches require urgent recognition due to potentially devastating consequences if untreated. Despite established clinical "red flag" criteria, identifying patients needing immediate evaluation remains challenging in primary care. This study developed and evaluated a large language model (LLM)-based multi-agent clinical decision support system for interpretable secondary headache diagnosis. MATERIALS AND METHODS: We first established 7 clinically relevant secondary headache red flag domains through manual review and synthesis of clinical guidelines. Based on these domain

PhenoSS: phenotype semantic similarity-based approach for rare disease prediction and patient clustering.

OBJECTIVES: Systematic clinical phenotyping using Human Phenotype Ontology (HPO) is central to rare disease diagnosis. However, current disease prioritization (ranking candidate diseases from HPO for a patient) methods face key challenges: they often fail to account for the hierarchical structure of HPO terms, ignore dependencies among correlated terms, and do not adjust for batch effects arising from systematic differences in phenotype documentation across cohorts, institutions, or clinicians. We aim to develop a scalable and statistically principled framework to address these limitations for

A lossless one-shot distributed algorithm for addressing heterogeneity in multi-site generalized linear models.

OBJECTIVE: We propose Heterogeneity-aware Collaborative One-shot Lossless Algorithm for Generalized Linear Model (COLA-GLM-H), a novel one-shot lossless distributed algorithm that enables the integration of heterogeneous multi-institutional data while relying solely on instituion-level summary information rather than patient-level data. MATERIALS AND METHODS: Generalized Linear Models (GLMs) are widely used in medical research for analyzing diverse outcome types. In multi-institution settings, we demonstrated that the global likelihood can be reconstructed using only institution-level summary
