A case of a persistent parotid fistula and sialocele following temporomandibular joint surgery is described. This rarely reported complication is postulated to result from pre-existing bucco-masseteric obstruction of parotid outflow causing raised intraductal pressure in the patient. Following unsuccessful conservative management, the patient was cured by a 5-day course of restricted oral intake and propantheline bromide.

A young scientist reported severe headaches emanating from behind one eye with an associated intermittent monocular blur. The monocular near cross-cylinder test revealed an accommodative fluctuation of one eye alone with a range of approximately 3 D. Referral to a neuro-ophthalmologist failed to produce a diagnosis for the disorder. A subsequent routine visit to a dentist provided a diagnosis of Costen's temporomandibular joint (TMJ) syndrome, a condition that is commonly misdiagnosed as a variety of neurological and vascular disorders.

Twenty-seven patients with echocardiographic evidence of primary mitral valve prolapse (MVP) were tested for the presence of joint hypermobility using the criteria of Beighton and Horan. Thumb metacarpophalangeal (MCP) fusion to treat posttraumatic arthritis is retrospectively reviewed in 18 patients, 16 of whom were examined and functionally tested, with an average follow-up of 18 months. All patients were satisfied. There was 100% fusion in an average of 59.9 days. Preoperative disabling MCP pain was present in all patients, and mild postoperative pain and difficulty in picking up small objects were present in 78%. A 63-year-old woman was unable to actively flex the interphalangeal joint of her right thumb after a hyperextension injury. Surgical exploration revealed an avulsion of the distal insertion of the palmar plate, allowing the bulk of the plate to block the joint during attempted active flexion. Resection of the palmar plate restored active flexion and caused no joint instability. We do not believe this lesion, which resulted in this disability, has been previously described.

Autologous whole blood was injected into the knee joints of rabbits 3 times each week for 12 weeks. The resulting destructive arthritis showed macroscopic and microscopic changes similar to those described in hemophilic arthritis in humans. The clinical and radiological findings in a patient who developed a spontaneous fistula between the right temporo-mandibular joint and the external auditory canal are presented. A high resolution CT scan revealed that the fistula had developed through a congenital dehiscence in the anterior wall of the tympanic bone.

A dislocation of the interphalangeal joint of a thumb was treated without problems. The simultaneous dislocation of the lunate was not diagnosed or treated until the plaster cast was removed. Even in cases with an obvious lesion like a finger joint dislocation it is necessary to investigate all other fingers and the wrist to exclude other lesions and to avoid delay in treatment.

The authors advocate an elective approach to the lower part of the humeral (shoulder) joint by an original axillary route. This route is external to the short head of the biceps brachii muscle which is retracted together with the neurovascular bundle.
