Patient-reported outcomes with durvalumab after chemoradiotherapy in stage III, unresectable non-small-cell lung cancer (PACIFIC): a randomised, controlled, phase 3 study.

BACKGROUND: In the ongoing, phase 3 PACIFIC trial, durvalumab improved the primary endpoints of progression-free survival and overall survival compared with that for placebo, with similar safety, in patients with unresectable, stage III non-small-cell lung cancer. In this analysis, we aimed to evaluate one of the secondary endpoints, patient-reported outcomes (PROs). METHODS: PACIFIC is an ongoing, international, multicentre, double-blind, randomised, controlled, phase 3 trial. Eligible patients were aged at least 18 years, had a WHO performance status of 0 or 1, with histologically or cytolo

Feasibility and physiological effects of a combined exercise and nutritional intervention in older adults with cancer under catabolic stress.

BACKGROUND: Ageing and cancer are associated with accelerated skeletal muscle catabolism, leading to sarcopenia, adverse body composition changes, and functional decline. Exercise and nutritional support are established physiological countermeasures against muscle loss; however, their feasibility and physiological effects in older adults exposed to profound catabolic stress remain insufficiently characterized. METHODS: Between 2021 and 2023, adults aged ≥65 years with advanced cancer undergoing systemic therapy participated in a 12-week combined exercise and nutritional intervention program.

Differences in Post-Cardiac Rehabilitation Quality of Life in Western Sydney by Country of Birth, Employment Status, Socioeconomic Status of Area of Residence, and Remoteness of Area of Residence.

BACKGROUND: Cardiac rehabilitation improves recovery after cardiovascular events, but changes in health-related quality of life (HRQoL) may differ by socio-economic subgroups. This study investigated whether changes in HRQoL differed by country of birth, employment status, socio-economic status of area of residence, and remoteness. METHOD: Data were analysed from individuals who completed pre- and post-program assessments at a publicly funded cardiac rehabilitation program in Western Sydney, Australia. Changes in domain scores of the 36-Item Short Form Health Survey (SF-36) were assessed usin

Case report: Changes in defense mechanisms, personality functioning, and body mass index during psychotherapy with patients with anorexia nervosa.

Eating disorders (EDs) are difficult to treat in psychotherapy due to their pervasive symptomatology and frequent and rapid relapses. Restrictive anorexia nervosa (AN) is the most challenging ED, often associated with severe physical and mental conditions. Perceived as an ego-syntonic syndrome that somehow protects the patient from a number of developmental tasks, treating AN requires extensive multidisciplinary long-term intervention. As with other emotion regulation strategies, defense mechanisms mediate an individual's reaction to internal or external stressors, including those related to E

A comparison of two methods to ascertain dietary intake: the CARDIA Study.

Data on dietary intake were collected in the Coronary Artery Risk Development in Young Adults (CARDIA) Study at the baseline examination in 1985-86 and again at the second examination 2 years later. At baseline, a diet history questionnaire developed for the CARDIA study was used; at the second exam the NCI (Block) food frequency questionnaire was used. The purpose of the present report is to compare the estimated nutrient intakes obtained with the two instruments; to compare correlations of nutrient intakes obtained at the two exams with those observed for other lifestyle and physiological va

MISTOSUS: a phase II trial of Viscum album extract adjuvant therapy for relapsed resectable osteosarcoma.

Relapsed resectable osteosarcoma carries a dismal prognosis of 30% 12-month post-relapse event-free survival (PREFS-12). Surgery remains the primary therapy for metastatic pulmonary osteosarcoma with the role for adjuvant therapy still undefined. There is an urgent need for effective nontoxic adjuvant therapies to improve PREFS following complete resection of metastases. Viscum album extract (Iscador® P, Iscador AG, Arlesheim, Switzerland) is a fermented aqueous extract manufactured from the leaves, stems, and berries of the white-berried hemiparasitic plant from the European mistletoe (Viscum

Case Report: Adebrelimab-associated suspected immune-related myocarditis with myositis/myasthenia gravis overlap syndrome (IM3OS): A rapidly fatal case and clinical implications.

This article reports a case of a 72-year-old male with intrahepatic cholangiocarcinoma who developed acute symptoms-including chest tightness, muscle weakness, ophthalmoplegia, and dyspnea-3 days after receiving the second cycle of adebrelimab, following combination therapy with adebrelimab, apatinib, and tegafur. These complications proved fatal. Laboratory tests showed significant elevation of myocardial injury markers (high-sensitivity troponin T, N-terminal pro-B-type natriuretic peptide, myoglobin, and creatine kinase). Electrocardiogram showed multi-lead ST-T changes and conduction abnor

Practical management of rheumatic immune-related adverse events occurring with immune checkpoint inhibitors: A narrative review.

Immune checkpoint inhibitors (ICIs) have dramatically changed the management of cancer and their indications are expanding, leading to an increase in immune-related adverse events (irAEs). Recently, attention has focused on previously underestimated rheumatic irAEs, including inflammatory arthritis (IA), polymyalgia rheumatica-like phenotypes and Sicca syndrome, but also fasciitis, and rare but severe immune mediated myositis, with a possible association with myasthenia-like symptoms or myocarditis. Rheumatic irAEs may have a prolonged course and affect a patient's quality of life. Patients wi

Reducing the risk of atherosclerotic cardiovascular disease in people with hemophilia: the importance of primary prevention.

Revolutionary advances in the treatment of hemophilia has led to a significant improvement in life expectancy. Associated with this has been an increase in age-related diseases especially atherosclerotic cardiovascular disease (CVD). While people with hemophilia (PWH) develop atherosclerosis at rates similar to those of the general population, rates of atherothrombosis and mortality related to CVD have been much lower, due to their hypocoagulable state. Changing treatment paradigms, aimed at reducing the risk of bleeding by improving hemostasis to levels approaching normality, has meant that t

A Protocol for Comprehensive Analysis of Gait in Individuals with Incomplete Spinal Cord Injury.

This is a protocol for comprehensive analysis of gait and affecting factors in individuals with incomplete paraplegia due to spinal cord injury (SCI). A SCI is a devastating event affecting both sensory and motor functions. Due to better care, the SCI population is changing, with a greater proportion retaining impaired ambulatory function. Optimizing ambulatory function after SCI remains challenging. To investigate factors influencing optimal ambulation, a multi-professional research project was grounded with expertise from clinical rehabilitation, neurophysiology, and biomechanical engineerin

HeartMate III Autonomy Assistant in a Hemiplegic Left Ventricular Assist Device Patient: A Case Report.

Terminal heart failure presents challenges, requiring cardiac transplantation, or mechanical circulatory support. Limited donor organ availability has made mechanical support crucial. Advances in centrifugal-flow systems, compared to axial-flow, have improved patient outcomes by reducing adverse events. Rehabilitation is vital for patient independence postimplantation. We developed a tool to aid hemiplegic patients in managing power sources. A 44 year old male with tritroncular ischemic heart disease and a 15% ejection fraction. He underwent a double bypass and left ventricular assist device (

[Recent changes in paradigms in the management of adulthood chronic idiopathic thrombocytopenic purpura].

The scenario of immune pathomechanism-based first- and second-line therapies of immune thrombocytopenia (i.e., immunosuppression, splenectomy) substantially changed approximately more than 20 years ago, giving place and important role for thrombopoietin analogs in steroid refractory cases, positioning splenectomy as third-line intervention. Although this approach became dominant and powerful, refractory cases urged the search for further medications. More recently, the traditional immunological approach received more attention again, and new targets were determined: (1) shortening selectively

Rare case of postoperative mediastinitis following thyroidectomy.

Descending necrotizing mediastinitis (DNM) is a life-threatening complication secondary to oropharyngeal abscesses, cervical esophageal perforation, or neck infections spreading along the fascial planes into the mediastinum. Post-thyroidectomy surgical site infection is a highly unusual complication that is typically localized around the incision, but may be propagated into the mediastinum, causing DNM. We encountered a case of this rare complication after thyroidectomy. The patient was a 33-year-old woman who was admitted to our center for left hemithyroidectomy. The surgery was performed wit

Management of Adverse Events and Supportive Therapy in Relapsed/Refractory Multiple Myeloma.

Relapsed/refractory (RR) multiple myeloma (MM) patients are a fragile population because of prolonged drug exposure and advanced age. Preserving a good quality of life is of high priority for these patients and the treatment of disease- and treatment-related complications plays a key role in their management. By preventing and limiting MM-induced complications, supportive care improves patients' outcome. Erythropoietin-stimulating agents and bisphosphonates are well-established supportive strategies, yet novel agents are under investigation, such as anabolic bone agents and activin receptor-li

Changes in clinical scenarios, management, and perspectives of patients with chronic hepatitis C after viral clearance by direct-acting antivirals.

INTRODUCTION: Hepatitis C virus (HCV) causes a systemic infection inducing hepatic and extrahepatic diseases. These latter involve cardiovascular system, kidney, brain, endocrine, glucose, and lipid metabolism, and the immune system. HCV infection is associated with an increased risk of morbidity and mortality for both hepatic and extrahepatic events. Direct-acting antivirals (DAA), introduced in the most recent years for HCV treatment, are effective in up to 99% of cases and have changed the clinical scenarios and management of these patients. AREAS COVERED: The literature on the impact of H

Non-Hematologic Toxicity of Bortezomib in Multiple Myeloma: The Neuromuscular and Cardiovascular Adverse Effects.

The overall approach to the treatment of multiple myeloma (MM) has undergone several changes during the past decade. and proteasome inhibitors (PIs) including bortezomib, carfilzomib, and ixazomib have considerably improved the outcomes in affected patients. The first-in-class selective PI bortezomib has been initially approved for the refractory forms of the disease but has now become, in combination with other drugs, the backbone of the frontline therapy for newly diagnosed MM patients, as well as in the maintenance therapy and relapsed/refractory setting. Despite being among the most widely

Lights Out! The Body Needs Sleep: Electronic Devices and Sleep Deficiency.

Sleep hygiene in children and young adults has been a topic of interest in scientific studies geared towards understanding metabolism, mental health, neuroscience, and in reference to the quality of life. There are multiple factors that may contribute to poor sleep hygiene in children, many of these include environmental factors and genetic components. This review article will pay particular focus on environmental factors which as of late, have been increasing contributors to poor sleep hygiene in children. Ultimately, these factors lead to unhealthy habits that transform into unhealthy lifest

[Rheumatoid arthritis in elderly people].

Rheumatoid arthritis (RA) is a chronic, systemic connective tissue disease, characterized by progressive, destructive polyarthritis with internal organs involvement due to active, systemic inflammation. The onset of disease occurs usually in 4th or 5th decade of life. Since the general population is ageing, beginning of RA in older age is more and more common. The term elderly onset of rheumatoid arthritis (EORA) describes the disease with onset at age over 60. Several observational studies indicated, that proportion of women and men is comparable in EORA. Clinical course of the disease is cha
